SummaryAn account is given of all known cases of haemolytic disease occurring in two Counties in the North East of England during the last decade.The value of cord blood estimations in assessing severity and in selecting for exchange transfusion is discussed. The criteria for exchange transfusion which have been successfully employed are given, together with the results of 822 liveborn cases treated in Newcastle hospitals since 1952.When cord samples are not available reliance must be placed on other criteria, and the importance of the early clinical picture is discussed with particular emphasis on jaundice which is often the feature calling attention to the diagnosis. The help which can be obtained from haemoglobin, bilirubin and reticulocyte levels in the first hours of life is assessed.RésuméAnalyse de tous les cas connus de maladie hémolytique du nouveauné survenus au cours des 10 dernières années dans 2 comptés du Nord‐Est de l'Angleterre. L'utilité de divers examens de sang, quand il s'agit de juger de la gravité de la maladie et de choisir les cas à traiter par l'exsanguino‐transfusion, est discutée. Les critères dont les auteurs se sont servis avec succès pour l'exsanguino‐transfusion sont présentés, ainsi que les résultats de ce traitement chez 822 cas nés vivants dans les hôpitaux de Newcastle depuis 1952.Quand on n'a pas d'échantillon de sang, on est obligé d'avoir recours à d'autres critères, et l'importance, dans ces cas, du tableau chinique initial, et tout particulièrement de la jaunisse qui est souvent le symptôme qui conduit au diagnostic, est discutée.Les service rendus par les taux d'hémoglobine, de bilirubine et des réticulocytes, dans les premières heures après la naissance, sont évalués.ZusammenfassungEs werden alle Fälle von Morbus haemolyticus neonatorum, welche in den letzten zehn Jahren in zwei Grafschaften Nordostenglands erkannt wurden, zusammenfassend dargestellt. Die Brauchbarkeit verschiedener Blutwerte für die Einschätzung der Schwere der Krankheit und für die Indikationsstellung zur Austauschtransfusion wird besprochen. Die Kriterien, derer sich die Autoren bei letzterer mit Erfolg bedienten, werden angegeben, sowie die Ergebnisse der Behandlung von 822 Fällen, die seit 1952 in den Spitälern von Newcastle lebend geboren wurden. SummaryModels of the liver and spleen were made with agar containing a known amount of radioactive chromium. These models were placed in corpses and “surface counts” were then made with a scintillation counter. In this way the counter was calibrated and the calibrations were used to interpret counts made in living subjects following the injection of Cr51‐labelled incompatible red cells.When most of the red cells (average 85%) were removed from the circulation with a half time of 2–6 minutes, most (average 74%) of the injected radioactivity was found in the liver. When approximately 80% of the red cells were removed from the circulation with a half time of 15–22 minutes, or 56–75 minutes, about 80% of the injected radioactivity was found in the spleen.RésuméDes modèles du foie et de la rate ont été fabriqués en agar contenant une quantité connue de chrome radioactif. Ces modéles furent placés dans des cadavres et des «comptes de surface» furent faits avec un compteur à scintillation. De cette façon le compteur fut calibré et ce calibrage servit de base pour interpréter des comptes faits sur des sujets vivants auxquels des hématies incompatibles marquées au Cr51avaient été injectées.Quand la plupart des hématies (en moyenne 85%) quittent la circulation avec un demi‐temps de 2 à 6 minutes, on retrouve la plus grande partie (en moyenne 74%) de la radioactivité dans le foie. Quand environ 80% des hématies quittent la circulation avec un demi‐temps de 15 à 22 minutes, ou de 56 à 75 minutes, environ 80% de la radioactivité injectée se retrouve dans la rate.ZusammenfassungEs wurden Agarmodelle der Leber und der Milz hergestellt, welche eine bekannte Menge radioaktives Chrom enthielten. Diese Organmodelle wurden in Körpermodelle eingesetzt. Hernach wurde der Scintillationszähler an der Oberfläche dieser Körpermodelle geeicht. Die Eichergebnisse wurden benutzt, um die Meßergebnisse bei Menschen, denen Cr51markierte inkompatible Erythrozyten injiziert worden waren, zu deuten.Bei denjenigen Fällen, bei welchen die Hauptmenge der injizierten Erythrozyten mit einer Halbwertzeit von zwei bis sechs Minuten aus dem Kreislauf eliminiert wurde, fand sich der Hauptanteil der verabreichten Radioaktivität (im Mittel 74%) in der Leber. SummaryAccording to the generally accepted view, the function of the secretor gene consists in conditioning the secretion of ABH substance and in producing, together with theLeagene, the Lebcharacter on the blood cells. By means of a special irregular anti‐A in an A2Le(a‐b+) person (“Magard”) a further function of the secretor gene was demonstrated, viz. the function of modifying the A substance on the blood cells of persons who lack theLeagene. The secretor gene gives the A substance a special form which makes the blood cells react with Magard serum as well as with anti‐A. In other words: In addition to A, the blood cells of persons possessing the gene constellation characteristic of Magard positive persons, develop a special blood group character, provisionally called the Magard factor. This factor is thus the result of an interaction between 3 gene systems, viz. the Lewis system, the secretor system, and the ABO system.The evident analogy with the Lebcharacter leads to the suggestion that the Lebcharacter too depends on an interaction between these three gene systems. This implicates the ABO system into the genetic systems which determine the Lewis phenotype.The hypothesis advanced above affords an explanation of the following three phenomena: (1) the parallelism between the H character and the Lebcharacter, (2) the fact that all Ohpersons tested so far have been Le(a+) and non‐secretors, and (3) the departure from the genetic rules of the Lewis system in the Ohfamily published byLevineet al.RésuméIl est généralement admis aujourd'hui que le gène sécréteur a pour fonction de régler la sécrétion des substances ABH et de produire, en combinaison avec le gène Lea, le caractère Lebdes eérythrocytes. A l'aide d'un anti‐A irrégulier spécial trouvé chez un individu (“Magard”) de groupe A2Le(a‐b+) il a été possible de mettre en évidence une troisième fonction du gène sécréteur, celle de modifier la substance A sur les érythrocytes de personnes ne possédant pas le gène Lea. Le gène sécréteur donne à la substance A une forme spéciale qui fait que les hématies réagissent aussi bien avec l'anti‐A qu'avec le sérum Magard. Autrement dit: les hématies de personnes ayant la constellation de gènes caractéristique de personnes Magard positives possèdent, en plus de l'antigène A, une spécificité spéciale provisoirement appelée «facteur Magard». Ce facteur est donc la résultante de trois systèmes de gènes: du système Lewis, du système sécréteur et du système ABO.L'analogie évidente avec le caractère Lebfait penser que ce dernier, lui aussi, dépend des trois systèmes susmentionnés. Ceci implique le système ABO dans la détermination du phenotype Lewis.L'hypothèse proposée permettrait d'expliquer les trois phénomènes suivants: 1. le parallélisme entre le caractère H et le caractère Leb, 2. le fait que tous les individus Ohéxaminés jusqu'ici étaient Le(a+) et non‐sécréteurs, et 3. l'apparente exception aux lois génétiques du système ABO rencontrée dans la famille Ohpubliée parLevineet coll.ZusammenfassungEntsprechend der allgemein anerkannten Auffassung ist das Sekretorgan verantwortlich für die Ausscheidung ABH‐Substanz. Im weiteren bewirkt es zusammen mit dem Lea‐Gen die Ausbildung der Leb‐Eigenschaft der Erythrozyten. Mit Hilfe eines besonderen irregulären Anti‐A‐Antikörpers eines A2Le(a‐b+)‐Individuums (« Magard ») gelang es, eine weitere Funktion des Sekretorgans nachzuweisen. Dieses bewirkt bei Individuen, welchen das Gen Leafehlt, eine Änderung des A‐Antigens der Erythrozyten. Solche Zellen zeigen eine positive Reaktion mit Anti‐A‐Seren und dem Magardserum. Dies bedeutet mit anderen Worten, daß Individuen mit der typischen Magard‐positiven Genkonstellation über ein besonderes Blutgruppenantigen verfügen, welches vorläufig als Magardfaktor bezeichnet wurde. Dieser Faktor beruht auf dem Zusammenwirken von drei Gensystemen, nämlich dem ABO‐, dem Lewis‐ und dem Sekretorsystem.Die offensichtliche Analogie zur Leb‐Eigenschaft läßt die Annahme als wahrscheinlich erscheinen, daß auch diese auf dem Zusammenwirken dieser drei Gensysteme beruhen dürfte. Falls diese Annahme den Tatsachen entspricht, würde das ABO‐System zu denjenigen Gensystemen gehören, welche für die phänotypische Ausprägung der Lewiseigenschaften verantwortlich sind.Diese Hypothese würde die folgenden drei Phänomene erklären: 1. Die Koppelung der H‐ mit der Leb‐Eigenschaft, 2. die Tatsache, daß alle bis heute untersucht

The antigen is rare: three examples were found in testing the blood of 1200 random Dutch donors. The antigen is inherited as a dominant character. SummaryIt has been possible to demonstrate the Rh antigen D on platelets from Rh positive individuals by means of the antiglobulin consumption test, the technique of which is described in detail. To obtain positive reactions, it proved necessary to use twice as much platelets as are needed for detecting pathological anti‐platelets antibodies.RésuméPar le test de consommation d'antiglobuline (dont la technique est décrite), les auteurs ont montré la présence de l'antigène D sur les plaquettes des individus Rh‐Positif. Pour mettre en évidence cet antiène, il fallut accroître la sensibilité de la réaction par une très grande quantité de plaquettes (deux fois plus de plaquettes que la dose nécessaire à la détection des anticorps pathologiques antiplaquettes).ZusammenfassungEs gelang mit Hilfe des Antiglobulinkonsumptionstestes, dessen Technik eingehend beschrieben wird, an den Plättchen rhesuspositiver Individuen das Antigen D nachzuweisen. SummaryThe usefulness of red cells separated from plasma in blood transfusion is well established. The indications for these have been considerably extended: whereas in 1948 washed cells were asked for only in case of Marchiafava's nocturnal hemoglobinuria, almost half the transfusions of whole blood given at the present time might reasonably and profitably be replaced by transfusions of concentrated (or washed) cells.There are, however, certain technical drawbacks: separated cells must be prepared ex tempore and they must be administered as soon as possible. These preparations will therefore be used extensively only when it becomes possible to store them. That is why we are now studying, by means of51Cr, the viability of concentrated cells stored up to 7 days. Satisfactory results would make it possible to constitute “red cell banks”, the need for which is already being felt, since the demand for concentrated or washed cells is steadily increasing.ZusammenfassungEs ist unnötig, auf das klinische Interesse an plasmafreien Erythrozytensedimenten hinzuweisen. Ihr Indikationsbereich hat sich ständig erweitert: 1948 war die Marchiafava‐Micheli'sche paroxysmale nächtliche Hämoglobinurie die einzige Krankheit, bei welcher ein Bedarf an gewaschenen Erythrozyten vorlag, während heute etwa die Hälfte der Vollbluttransfusionen vorteilhaft durch Erythrozytentransfusionen ersetzt werden könnte.Die heute noch notwendige ad hoc‐Herstellung von Erythrozytensuspensionen und die Notwendigkeit der Transfusion kurz nach ihrer Herstellung bereitet noch praktische Schwierigkeiten. Dies würde sich ändern, wenn die Erythrozyten wie Vollblut über eine längere Zeitperiode konserviert werden könnten. Aus diesem Grunde führen wir derzeit mit der Cr51‐Technik Überlebenszeitbestimmungen von Erythrozyten durch, die bis zu sieben Tagen konserviert wurden. Falls diese Untersuchungen positive Ergebnisse zeitigen, würde dies die Gründung von « Erythrozytenbanken » ermöglichen, was im Hinblick auf den ständig steigenden Bedarf an Erythrozytensuspen
