AbstractThe clinical and serological records of 20 pregnant patients with erythrocyte autoantibodies were critically reviewed and analysed. This series, the largest reported, confirmed an association between erythrocyte autosensitization and pregnancy, and suggested that this occurred on an average of 1 in 50,000 pregnancies. The clinical presentation varied from severe haemolytic anaemia to serological abnormalities discovered during the routine testing of well patients; overt haemolysis was present in 7 cases. Treatment was only necessary in 3 patients where the haemolysis was severe. 3 infants were mildly affected with haemolytic disease due to the maternal autoantibodies crossing the placenta but no treatment was needed. The risks to the infant were increased when other active autoimmune conditions (e.g., systemic lupus erythematosus) were present. With early diagnosis and appropriate treatment, the outlook for mother and child is good.

AbstractThe Lewis (Lea) blood group substance in alimentary tract liquids is a large molecule. Ileal and jejunal mucosal preparations can act upon it to produce dialysable serologically active Lea. Alimentary tract bacteria cannot produce a similar effect. Plasma, serum and urinary Leaare principally in the form of a dialysable moiety. It is suggested that this dialysable form is produced by mucosal degradation of large‐molecule Leain the alimentary tract. The small‐molecule Leais absorbed, transported in the plasma and then excreted. Both secretor and non‐secretor patients with duodenal ulcer and inflammatory bowel disease have normal urinary Leatitres. Non‐secretor coeliac patients who have not regenerated normal jejunal mucosa on treatment have significantly reduced urinary Leatitres when compared with healthy individuals. AbstractArtificial antigens:Antibodies were raised in rabbits with βDGal(13) βDGlcNAc‐BSA and purified by absorption elution on a column of the synthetic oligosaccharide covalently bound to a silicate instead of BSA. The purified antibodies agglutinated specifically erythrocytes from Le (a‐b‐) nonsecretor donors (Lec).Natural antigens:Antibodies were raised in goats using boiled saliva fromO/O, le/le, se/sedonors. The population of antibodies which was adsorbed on a column consisting of βDGal(1→3) βDGlcNAc‐O‐R (type 1 chain precursor disaccharide) groupings attached to a silicate was collected and then refined by passage through a column which consisted of similarly immobilized αLFuc(1→2)βDGal(1→3)βDGlcNAc‐(H type 1), βDGal(1→3)‐[αLFuc(1→4)]βDGlcNAc‐(Lea), and βDGal(1→4)βDGlcNAc‐ (type 2 core chain precursor) groupings. AbstractBy lowering the pH of the medium, allo‐ and autoantibodies bound to granulocytes and platelets could be eluted from these cells and their presence in the eluate could subsequently be detected with normal donor granulocytes and platelets, respectively, in the immunofluorescence test and/or agglutination test. IgG‐ as well as IgM‐ and IgA‐containing antibodies were detectable in the eluates. AbstractMeasurement of diluted samples of high‐titred antisera to hepatitis B surface antigen (HBs) by radioimmunoassay (RIA) proved that the RIA kit can be used quantitatively in a wider range than stated by the manufacturer. Abstract6 patients with antibody deficiency disease had intravenous immunoglobulin substitution and their serum IgG levels were monitored for at least 6 months. After three IgG infusions of 6–12 g each at intervals of 3 or 4 weeks, the IgG serum concentrations were between 300 and 500 mg/dl in 3 patients. In the 3 other patients, higher serum levels were obtained after considerably more IgG infusions given at intervals of 8 days or less. Maintenance of subnormal levels was achieved by replacement of approximately 10 mg/kg body weight/day. AbstractPlatelet concentrates were prepared in plastic packs of polyvinyl chloride with tri‐(2‐ethylhexyl) trimellitate as plasticizer. They were stored, with gentle shaking, at room temperature for periods up to 7 days before labelling with isotope and reinfusing. In vivo survival studies, platelet counts, pH and electron microscopy indicated that platelet concentrates prepared in the new plastic were superior to those prepared in the standard pack currently in use. Oxygen was found to diffuse through the new pack more rapidly than through the standard pack.
