SummaryThree serological methods, the agglutination test, the anti‐human globulin consumption test and the complement fixation test for the detection of platelet antibodies are described and the results given. Special attention is directed to some factors concerning the anti‐human globulin consumption test. The frequency and clinical significance of platelet iso‐ and auto‐antibodies was investigated in patients with a history of blood transfusion, in pregnant women and in patients with an idiopathic or secondary thrombocytopenia. Some of the results are compared with those obtained by thrombelastography.In the serum of 6 or 0.92% out of 650 normal persons agglutinins were found. In all these cases the agglutinins proved to be aspecific cold auto‐antibodies. The AHGC test and the CF test were negative with all normal sera. Platelet suspensions from 10 normal persons were investigated but we did not succeed in finding specific platelet groups by means of incomplete antisera.The AHGC test was positive with the serum of 58 or 14.3% out of 406 patients with a history of blood transfusion. The frequency of the incomplete antibodies correlated with the number of blood transfusions received.With the CF test a somewhat smaller number of positive results were obtained. In most sera with platelet antibodies white cell antibodies were also found. To our experience platelet iso‐antibodies rarely give rise to febrile transfusion reactions.The direct AHGC test was positive on the platelets of 13 or 56.5% out of 23 patients with idiopathic thrombocytopenia and of 22 or 27.2% out of 81 patients with secondary thrombocytopenia. In both groups free circulating platelet antibodies were found in a much lower frequency.The CF test proved to be less sensitive for the recognition of auto‐antibodies. Thrombelastography was positive with the serum of 22 out of 49 patients with a history of blood transfusion and of 31 out of 96 thrombocytopenic patients. The results of thrombelastography did not correlate with the other methods used.It is still uncertain, whether platelet auto‐antibodies are the primary cause of thrombocytopenia or whether they are secondary formed, their significance being limited to an aggravation of the thrombocytopenia.RésuméLes auteurs décrivent 3 méthodes sérologiques, le test d'agglutination, le test de consommation de l'anti‐globuline humaine et le test de fixation du complément, destinées à mettre en évidence les anticorps antiplaquettaires. Ils donnent les résultats obtenus au moyen de ces trois méthodes. Une attention toute particulière a été consacrée à l'étude de certains facteurs concernant le test de consommation de l'antiglobuline humaine. La fréquence et la signification cliniques des iso‐ et des autoanticorps antiplaquettaires ont été recherchées chez des patients ayant eu des transfusions de sang, chez des femmes enceintes et chez des patients atteints de thrombocytopénics idiopathique et secondaire. Certains des résultats obtenus sont comparés avec ceux obtenus par la thromboélastographie.Dans le sérum de six (0,92%) personnes normales sur 650 des agglutinines ont été découvertes. Dans tous ces cas, il a été prouvé qu'il s'agissait d'auto‐anticorps froids aspécifiques. Les tests de consommation de l'anti‐globuline humaine et de fixation du complément étaient négatifs avec tous les sérums normaux. Des suspensions plaquettaires de dix personnes normales ont été examinées, mais il n'a pas été possible de mettre en évidence des groupes spécifiques de plaquettes au moyen d'anti‐sérums incomplets.Le test de consommation de l'anti‐globuline humaine était positif chez 58 (14,3%) patients sur 406 ayant eu des transfusions de sang. La fréquence des anticorps incomplets est en rapport avec le nombre de transfusion reçues.Avec le test de fixation du complément, on obtient un nombre plus petit de résultats positifs. Dans de nombreux sérums contenant des anticorps antiplaquettaires, des anticorps antileucocytaires ont été découverts. D'après notre expérience les isoanticorps antiplaquettaires ne provoquent que rarement des réactions fébriles lors de transfusions.Le test direct de consommation de l'anti‐globuline humaine est positif avec les plaquettes de 13 (56,5%) patients sur 23 atteints de thrombocytopénie idiopathique et de 22 (27,2%) patients sur 81 atteints de thrombocytopénie secondaire. Dans les deux groupes, la fréquence des anticorps antiplaquettaires libres circulants est beaucoup plus basse.Le test de fixation du complément s'est montré le moins sensible pour la mise en évidence des auto‐anticorps. La thromboélastographie était positive avec le sérum de 22 patients sur 49 ayant reçu des transfusions de sang et de 31 patients thrombocytopéniques sur 96. Les résultats obtenus avec la thromboélastographie ne concordent pas avec ceux obtenus au moyen des autres méthodes.On ne sait pas encore si les auto‐anticorps antiplaquettaires sont la cause pri‐maire de la thrombocytopénie ou s'ils sont formés secondairement. Leur signification se limite à une aggravation de la thrombocytopénie.ZusammenfassungDrei serologische Methoden, der Agglutinationsversuch, der Antihumanglobulinkonsumptionstest (AHGC‐Test) und der Komplementbindungsversuch (CF‐Test) zum Nachweis von Plättchenantikörpern werden beschrieben und die mit diesen Methoden erzielten Ergebnisse werden mitgeteilt. Spezielle Aufmerksamkeit wurde einigen Faktoren des AHGC gewidmet. Die Häufigkeit des Vorkommens von Plättcheniso‐ und ‐autoantikörpern sowie ihre klinische Bedeutung wurde bei Patienten mit früheren Bluttransfusionen, bei schwangeren Frauen sowie bei Patienten mit idiopathischer und sekundärer Thrombozytopenie untersucht. Einige dieser Ergebnisse wurden mit denjenigen die durch Thrombelastographie erhalten worden waren, verglichen.Bei 6, d. h. 0,92% von insgesamt 650 Seren gesunder Individuen wurden Agglutinine gefunden. In all diesen Fällen erwiesen sich die Agglutinine als unspezifische Kälteautoantikörper. Der AHGC‐Test und der CF‐Test war mit alien Normalseren ausnahmslos negativ. Plättchenaufschwemmungen von 10 gesunden Individuen wurden untersucht; es war uns unmöglich, mit Hilfe inkompletter Antiseren spezifische Plättchengruppen nachzuweisen.Bei 58, d. h. 14,3% von insgesamt 406 Seren von Patienten mit Bluttransfusionen in der Vorgeschichte, war der AHGC‐Test positiv. Die Häufigkeit des Vorkommens inkompletter Antikörper stand in direkter Beziehung zur Zahl der verabreichten Bluttransfusionen.Mit dem CF‐Test wurden etwas weniger häufig positive Reaktionen beobachtet. Die meisten Seren, mit Plättchenantikörpern enthielten auch Leukozytenantikör‐per. Nach unserer Erfahrung verursachen Plättchenisoantikörper nur selten febrile Transfusionsreaktionen.Der direkte AHGC‐Test war positiv mit den Plättchen von 13, d. h. 56,5% von 23 Patienten mit idiopathischer Thrombozytopenie und 22, d. h. 27,2% von 81 Patienten mit sekundärer Thrombozytopenie. In beiden Gruppen wurden frei zirkulierende Plättchenantikörper erheblich seltener beobachtet.Der CF‐Test erwies sich als weniger empfindlich zum Nachweis von Autoantikörpern. SummaryHeterospecific antiplatelet sera were demonstrated to have a direct action on coagulation, as manifest by the inhibitory effect on thromboplastin generation. This effect is distinct from the previously demonstrated ability to agglutinate platelets and inhibit clot retraction. Platelet phospholipid represents the antigenic platelet constituent responsible for this phenomenon. This constituent is not unique to platelets, but is also present in red cells since erythrocyte phospholipid cross reacts with antiserum directed against platelet phospholipid. Brain cephalin, which contains another phospholipid active in thromboplastin generation, exhibited no cross reaction.Fibrinogen is another antigenic substance intricately associated with platelets. It was confirmed that the precipitin action with sonicated platelets is due to fibrinogen.A platelet protein constitutes a third antigenic constituent which was found to be responsible for the immunological specificity of platelets. Platelet agglutination and inhibition of clot retraction are the result of an immune reaction involving this platelet protein as the specific substrate.RésuméLes sérums anti‐plaquettaires hétérospécifiques ont une action directe sur la coagulation se manifestant par un effet inhibiteur de la thromboplastinogénèse. Cet effet est différent de celui déjà démontré qui se manifeste par une tendance à agglutiner les plaquettes et à inhiber la rétraction du caillot. Left phospholipides plaquettaires représentent le constituent plaquettaire antigénique responsable de ce phénomène. Ce constituant n'est pas le seul apanage des plaquettes, mais il est aussi présent sur les érythrocytes puisque les phospholipides érythrocytaires présentent une réaction croisée avec l'anti‐sérum anti‐phospholipide plaquettaire. Avec la céphaline cérébrale, autre phospholipide active dans la thromboplastinogénèse, on n'observe pas de réaction croisée.Le fibrinogène est une autre substance antigénique intimement associée aux plaquettes. II a été confirmé que la réaction de précipitation des plaquettes traitées par les ultra‐sons est due au fibrinogène.Une protéine plaquettaire forme un troisième constituant antigénique qui s'est avéré comme étant responsable de la spécificité immunologique des plaquettes. L'agglutination plaquettaire et l'inhibition de la rétraction du caillot sont le résultat d'une réaction immunitaire component cette protéine plaquettaire comme substrat spécifique.ZusammenfassungHeterospezifische Antiplättchenseren hemmen die Thromboplastinbildung und beeinflussen auf diese Weise die Blutgerinnung. Diese Wirkung ist verschieden von der Fähigkeit solcher Seren, Plättchen zu agglutinieren und Gerinnselretraktion zu hemmen. Bei dem an diesem Phänomen beteiligten Antigen handelt es sich um Plättchenphospholipid. Diese Substanz kommt nicht nur in Plättchen, sondern auch in Erythrozyten vor, was daraus zu ersehen ist, daß Erythrozytenphospholipid mit Antiseren gegen Plättchenphospholipid Kreuzreaktionen zeigt. Mit Hirnkephalin, einem anderen an der Thromboplastinbildung beteiligten Phospholipid wurden hingegen keine Kreuzreaktionen beobachtet.Fibrinogen ist ein anderes Plättchenantigen. Es wurde bestätigt, daß bei der Präzipitationsreaktion mit Ultraschall zertrümmerten Plättchen Fibrinogen das Antigen darstellt.Ein Plättchenprotein bildet das dritte Antigen; es ist verantwortlich für die immunologische Spezifität der Plättchen. Summary24 rabbits were immunized withE. coliO86:B7. Their sera agglutinatedE. coliO86: B 7 and A, B, and, AB human red cells. In fact, we were able, by means of proper (differential) absorptions, to make these sera monospecific (α‐ or β‐like) as shown by tests of haemagglutination with suitable red cells and by inhibition tests with properly selected salivas of secretors.TheE. coli‐elicited α‐ and β‐like agglutinins are not an essential part of the globalE. coliagglutinating activity; in fact, it is possible to remove them (both by appropriate absorptions and by inhibiting them with salivas) without modifying significantly the anti‐E. colititres of the sera so treated.The presence of the B antigen (and less clearly but definitely) of the A inE. coliO86has been shown also by absorbing human sera of A, B, and O people, with E. coli O86.Résumé24 lapins ont été immunisés avec l'E. SummaryA 20‐year old healthy male was found to have an irregular ABO group. His cells behaved as group O, but anti‐B was the only detectable iso‐agglutinin in the serum. He was a secretor of H substance but not of A substance. Some more or less reasonable explanations of the findings are suggested.RésuméLes auteurs ont découvert chez un jeune homme de 20 ans un groupe ABO irré‐gulier. SummaryThe authors report a blood sample which they have classified as Bm, and which exhibited the following characteristics: (a) the red blood cells were not agglutinated with anti‐B serum, either normal or immune, nor by Group O sera. SummaryA new Rh gene complexDceif (Roi) is traced in two generations of a family and is found in high frequency in an Indian tribe (Ica) of Chibchan affiliation. It is suggested that the mutation formed among the Indians rather than as an introduction by way of non‐Indian populations. It is also suggested that this chromosome could be present in other Indian tribes, especially in those belonging to the Andean Equatorial linguistic family.RésuméLes auteurs ont pu mettre en évidence dans deux générations d'une famille un nouveau complexe de gène Rhésus Dceif (Roi) que l'on trouve à une fréquence élevée chez les Icas, tribu indienne affiliée aux Chibchan.On pense que la présence de ce gène est d'avantage le fait d'une mutation qui a eu lieu parmi les indiens, plutôt que celui de l'introduction du gène acheminé par des populations non indiennes. II est possible que ce chromosome existe dans d'autres tribus indiennes et spécialement parmi celles qui appartiennent à la famille linguistique des Andes équatoriales.ZusammenfassungEin neuer Rh‐Genkomplex Dceif (Roi) wurde bei zwei Generationen einer Familie festgestellt. Bei dem zur Chibcha‐Gruppe gehörigen Ica‐Indianerstamm wird dieser Rh‐Genkomplex häufig gefunden.
