SummaryThe effect of plasma and of fraction I on the bleeding time of a patient withvon Willebrand'sdisease has been presented. Some fractions were highly effective, others showed little or no effect. The differences in their preparation which may have contributed to this are discussed. Fresh plasma, from blood collected in plastic bags, was also effective. The use of glass bottles appeared markedly to decrease the activity of the “anti‐bleeding” factor. The possible relationship between the deficiency of this factor and of AHG invon Willebrand'sdisease is discussed.RésuméL'auteur dérrit les effets du plasma et de la fraction I sur le temps de saignement d'un patient atteint de la maladiede Willebrand. Certaines fractions ont un effet conidbrable; d'autres n'ont que très peu d'effet, voire aucun. La cause de ce phénomène peut résider dans les différents modes de préparations de substances injectées. Les plasma frais provenant de sang prélevé dans des sacs en plastic ont un bon effet. Par contre, I'utilisation de flacons en verre semhle diminuer d'une manière assez marquée I'activité du facteur «anti‐saignement». SummaryThe present concept of paraproteins includes myeloma globulins, Waldenström's macroglobulins, and Bence‐Jones' uroproteins. Their appearance in blood or urine is a characteristic symptom of neoplastic diseases of the lympho‐plasmo‐reticular system. Paraproteins are defined as proteins from the immune globulin group with an abnormal primary structure. Paraproteins lack some antigenic determinants of the corresponding immune globulins, yet they possess individual specific, and partly, group specific antigenic determinants which are not present in the corresponding normal immune globulins. According to their antigenic structure, serum paraproteins are classified into the γ‐globulin type, the β2A‐globulin type, the β2M‐globulin type, and the intermediary type. The γ‐ and β2A‐paraproteins are myeloma proteins, the β2M‐parapruteins are Waldenström's macroglobulins. The rarely found intermediary type cannot be attached firmly to one of the three immune globulins, and consists of paraproteins of low molecular weight which are similar to the Bence‐Jones' uroproteins.Having analyzed the sera of 206 patients, we came to the conclusion that by means of paper electrophoretical findings the potential diagnosis of a paraproteinemia can be made, while only immunoelectrophoretical investigation permits the identification of the type of paraproteins in question. In the whole, we found 109 cases of γ‐, 34 cases of β2A‐, 62 cases of β2M‐paraproteinemia, and 1 case of paraproteinemia of the intermediary type. In the discussion of our findings, we have dealt especially with the interpretation of the paraprotein‐linked changes of the immunoelectrophoretical diagrams.RésuméLes paraprotéines sont des protéines inertes de structure primaire anormale appartenant au groupe des irnrnuno‐globulines. Il leur manque une partie des déterminants antigéniques des imrnuno‐globulines rorrespondantes. D'autre part les paraprotéines portent des déterminants antigéniques de spécificité individuelle ou en partie de spécificité de groupe qui manque aux imrnuno‐globulines normales correspondantes. D'après leur structure antigénique on distingue actuellement quatre types de paraprotéines sériques: paraprotéines du type γ, du type β2A, du type β2Met enfin parsprotéines du type intermédiaire. Les paraprotéines γ et β2Asont des protéines de myélomes, les paraprotéines β2Msont des macroglobulines de Waldenstrom. Le type intermédiaire, du reste très rare, ne peut être attribué définitivement à une des trois immuno‐globulines; il contient des paraprotéines de poids moléculaire peu élevé; il e'agit de protéines du type de Bence‐Jones.L'analyse des séra de 206 malades autorise les conclusions suivantes: Les résultats de l'électrophorèse sur papier ne donnent que le diagnostic potentiel d'une para‐protéinémie; seul I'exarnen immuno‐électrophorétique perrnet d'identifier le type de paraprotéine présent. Nous ayons trouvé 109 paraprotéinémies du type γ, 34 du type β2A, 62 du type β2Met en outre I paraprotéinémie du type intermédiaire. Les risultats sont discutés dans leur ensemble en tenant particulièrement compte de I'interprétation des changernents du diagramme imrnuno‐électrophorétique dus aux paraprotéines.ZusammenfassungUnter dem Begriff der Paraproteine werden beute die Myelomproteine, die Waldenströmschen Makroglobuline und die Bence‐Jonesschen Uroproteine zusammengefaßt, deren humorales Auftreten ein charakteristisches Symptom neoplastischer Erkrankungen des lympho‐plasmo‐reticulären Systems darstellt. Es handelt sich bei den Paraproteinen urn Eiweißkörper nus der Gruppe der Immunglobuline, welche eine abnorme Primärstruktur aufweisen. Es fehlt ihnen ein Teil der Antigendeterminanten der entsprechenden Immunglobuline. Andererseits aber verfügen sie über individualspezifische und zum Teil über gruppenspezifische Antigendeterminanten, welche die entsprechenden normalen Immunglobuline nicht besitzen. Auf Grund ihrer Antigenstruktur werden deshalb die Serumparaproteine heute in folgende Typen eingeteilt: γ‐Globulin‐Typ, β2A‐Globulin‐Typ, β2M‐Globulin‐Typ und Intermediärtyp. Bei den γ‐ und β2A‐Paraproteinen handelt es sich um Myelomproteine, bei den β2M‐Paraproteinen urn Waldenströmsche Makroglobuline. Zum seltenen Intermediärtyp, der antigenanalytisch keinem der drei Immunglobuline fest zugeordnet werden kann, gehören niederrnolekulare Paraproteine vorn Typ der Bence‐Jonesschen Uroproteine.Bei der Analyse der Seren von 206 Patienten zeigte es sich, daß auf Grund des papierelektrophoretischen Befundes nur die Diagnose bzw. Verdacbtsdiagnose einer Paraproteinämie möglich ist, während es der immunelektrophoretischen Untersuchung vorbehalten bleibt, die Art des vorliegenden Paraproteins zu identifizieren. SummaryBy means of immunoelectrophoresis a study on the ρ‐proteins (pre‐albumins) of patients' sera was performed. In different diseases the two ρ‐proteins demonstrated appeared to undergo important quantitative changes. Decrease of both ρ‐proteins was observed in the serum of patients who suffered from protein lack in the food, cirrhosis of the liver or important tissue breakdown. The ρ‐levels returned to normal in recovering patients. No lowering of the ϱ‐proteins was observed in the nephrotic syndrome.The significance of the ϱ‐protein deficiency and the possible connection between the ϱ‐protein level and the state of protein balance are discussed.RésuméUne étude des ϱ‐protéines (préalbuniines) a été pratiquée sur des sérums de malades au moyen de l'analyse immuno‐électrophorétique. Dans différentes maladies, les deux ϱ‐protéines mises en évidence semblent subir d'importantes modifications d'ordre quantitatif. Une diminution des deux ϱ‐protéines a été observée dans le sérum de patients ayant souffert d'une alimentation pauvre en protéines, de cirrhose du foie ou d'importantes destructions tissulaires. Le taux des ϱ‐protéines redevenait normal chez des patients guéris. Il n'a pas été observé de diminution des ϱ‐prottines dam Ies syndromes néphrotiques. La signification de la déhcience en ϱ‐protéines et le rapport possible entre le taux des ϱ‐protéines et l'état d'équilibre protéinique est discuté.ZusammenfassungMittels der Immunoelektrophorese wurden die ϱ‐Proteine (Präalbumin) in Pacientenseren untersucht. Es wurde gezeigt, daß bei verschiedenen Krankheiten ausgeprägte Gehaltsverschiebungen beider ϱ‐Proteine vorkommen. Ein Schuund beider ϱ‐Proteine wurde bei Patienten mit eiweißarmer Ernährung, Lebercirrhose und allgemein gewebsdestruierenden Krankheiten beobachtet. SummaryA family in which the same‐D‐Rh complex occurs within different Rh genotypes is described. The amount of D antigen detected varied in different combinations and some variation was observed between individuals of the same type in the case ofede/‐D‐. Some effect on the amount of E and e antigens, estimated by dosage titrations, was detected. It was also concluded that‐D‐heterozygotes may well be more common in populations of European origin than the results of random tests would suggest.RésuméLes auteurs décrivent une famille dans laquelle le même complexe Rhésus‐D‐se présente parmi différents génotypes. La quantité d'antigène D mise en évidence vane dans les différentes combinaisons; on observe des variations parmi les individus du même type lorsqu'ils sontcde/‐D‐.On a pu mettre en évidence que la quantité d'antigène E et e déterminée par titration produit un certain effet. On conclut donc que les hétérozygotes ‐D‐peuvent être plus fréquents dans la population caucasienne que ne le laissent supposer les résultats des déterminations Rh pratiquées en série.ZusammenfassungEs wird eine Familie beschrieben, bei welcher der Rh‐Komplex‐D‐in mehreren unterschiedlichen Rh‐Genotypen vorkommt. Bei den verschiedenen Rh‐Kombinationen wurde ein ungleicher Gehalt an D‐Antigen gefunden; sogar bei Individuen mit demselben Genotyp z. B.cde/‐D‐wurde eine gewisse Variabilität festgestellt. SummaryLeaand Lebsubstances are shown to occur in seminal fluids. Single tube inhibition tests and titration studies show that the amounts and specificities of these substances in seminal fluids parallel those found in serum and plasma by other workers. These findings are briefly compared with what is known about the appearance of Lewis substances in saliva, milk and ovarian cyst fluids.RésuméOn trouve dans le sperme des substances Leaet Leb. Des tests d'inhibition en tube et les titrations démontrent que la quantité et la spécificité des substances Lewis présentes dane le sperme correspondent aux valeurs observées par d'autres auteurs dans le sérum et le plasma. SummaryThe reaction of human globulins with the erythrocyte surface was studied using I131labeled human globulin fractions containing incomplete anti‐D (Rho). These studies indicate that globulin normally fixes to erythrocytes in a nonimmunologic fashion. Quantitative studies on the nature of the nonimmunologic binding of human globulin were performed with the following results: (1) Fixation of human globulin to the erythrocyte occurred independently of the antigenic nature of the cells. (2) The number of molecules which fix to the surface of an erythrocyte bears the power relationship y2/y1= (x2/x1)Kto the globulin concentration of the reacting solution. (3) The linkage of globulin to the erythrocyte is relatively firm; approximately 50% is still fixed after 15 washings. (4) Globulin molecules fixed to the erythrocyte possess the ability to exchange with globulin molecules in a suspending medium. (5) Human globulin molecules fixed to erythrocytes give a negative agglutination reaction with rabbit antihuman globulin sera.RésuméLa réaction des globulines humaines à la surface des érythrocytes a été étudiée au moyen de fraction de globuline humaine contenant de I'anti‐D (Rho) incomplet et marquée au I131. Ces études démontrent que les globulines ee fixent aux érythrocytes d'une manière non immunologique. Dee ètudes quantitatives sur la nature de la liaison non immunologique dee globulines humaines out été pratiquées et ont donné les résultats suivants:1. La fixation des globulines humaines aux érythrocytes se produit indépendamment de la nature antigénique des érythrocytae. 2. Le nombre des molécules qui se fixent à la surface des érythrocytes est en rapport avec la concentration en globuline de la solution de suspension obéissant à la fonction y2/y1= (xl/xl)K. 3. La fixation de la globuline à I'érythrocyte est relativement solide: en effet, à peu près 50% des globdines sont encore fixées aprb 15 lavages. 4. Les molécules de globulines fixées aux érythrocytes peuvent e'interchanger avec des molécules de globuline se trowant dans le milieu de suspension. 5. Lee molécules de globuline humaine fixées aux érythrocytes donnent une réaction d'agglutination négative avec des sérums de lapin antiglobuline humaine.ZusammenfassungDie Reaktion menschlicher Globuline mit der Erythrozytenoberfläche wurde untersucht, Dabei wurden I131‐markierte, menschliche Globulinfraktionen, welche inkomplette Anti‐D(Rho)‐Antikörper enthielten, verwendet. Die Untersuchungen zeigten, daß Globuline normalerweise in einer nicht immunologischen Bindung an die Erythrozytenoberfläche fixiert werden. Die quantitative Analyse dieser nicht immunologischen Human‐Globulinbindung ergab folgendes:1. Die Fixation des Humanglobulins an die Erythrozytenoberflache steht in keiner Beziehung zur Antigenstruktur der Zellen. 2. Die Anzahl der an die Erythrozytenoberfläche gebundenen Moleküle steht in einer y2/y1= (x2/x1)K‐Beziehung zur Globulinkonzentration der Lösung. 3. SummaryThe reaction of antiglobulin serum with globulin fixed to erythrocytes in a non‐immunologic fashion was studied using I131labeled antiglobulin sera. These studies revealed: 1. Rabbit globulin fractions containing anti‐human globulin antibody fixed to erythrocytes irrespective of prior exposure to anti‐D (Rho) and the D (Rho) positive or negative state of the erythrocytes. 2. Rabbit anti‐human globulin antibody fixed to human erythrocytes coated nonimmunologically with human globulin. This is a nonagglutinating reaction. 3. Human globulin fixed in a nonimmunologic fashion to erythrocytes could be eluted and would react with anti‐human globulin antibody. 4. Rabbit anti‐human globulin antibody fixed to erythrocytes was unable to induce agglutination when incomplete anti‐D (Rho) was subsequently fixed to the cell.These studies indicate that the agglutination of erythrocytes coated with incomplete antibody by antiglobulin sera is dependent on the molecular conformation of incomplete antibody. The site of primary importance in inducing agglutination appeared to be the incomplete antibody‐erythrocyte linkage.RésuméLa réaction du sérum anti‐globuline avec la globuline fixée non immunologiquement aux érythrocytes a été étudiée au moyen de sérums anti‐globuline marqués B l'iode131, Ces études ont révélé:1. L'anticorps de lapin anti‐globuline humaine se fixe aux érythrocytes humahs chargés non immunologiquement avec des globulines humaines. Il n'y a pas de réaction d'agglutination. 3. Les globulines bumaines fixées non immunologiquement aux érythrocytes peuvent être élués et réagiesent avec l'anticorps anti‐globuline humaine. 4. L'anti‐corps de lapin anti‐globuline humaine fixé aux érythrocytes est incapable de provoquer une agglutination quand de l'anti‐D (Rho) incomplet est ensuite mis en contact avec ces érythrocytes.Ces études démontrent que l'agglutination par des sérums anti‐globuline d'érythrocytes chargés par des anticorps incomplets est dépendant de la constitution moléculaire de I'anticorps incomplet. Kaninchenglobulinfraktionen, welche Anti‐Human‐Globulin‐Antikörper enthielten, wurden an die Erythrozytenoberfiäche fixiert; dabei war es gleichgültig, ob die Zellen vorgängig mit Anti‐D(Rbo) behandelt worden waren oder ob es sich um D(Rho) positive oder negative Erythrozyten handelte. 2. Anti‐Human‐Globulinseren vom Kaninchen wurden von menschlicben Erythrozyten, die nicht‐immunologisch mit menschlichem Gammaglobulin beladen worden waren, ebenfalls fixiert.3. Nicht immunologisch‐gebundenes Human‐Globulin konnte von der Erythrozytenobedäche eluiert werden; es reagierte mit Anti‐Human‐Glohulinseren.4. Anti‐Human‐Globulin‐Antikörper vom Kaninchen, welche an der Erythrozytenoberfläche fixiert waren, vermochten nachträglich gebundene inkomplette Anti‐D(Rbo)‐Antikörper nicbt zu agglutinieren.Diese Untersuchungen zeigen, daß Erytbrozyten, die mit inkompletten Antikörpern beladen sind, durch Antiglobulinseren nur beim Vorliegen einer bestimmten Orientierung der Antikörper‐Mo

SummaryAs shown by determinations in 1088 patients suffering from various diseases of the blood, the frequency of the Gm(a) factor was lower in these patients than in the general population; this was particulary striking in cases of lymphatic leukemia (25,8%) and acute leukemia (39,1%).The determination of Gm(a) and Gm(x) in 89 cases of leukemia revealed, in addition to the known Gm phenotypes, the phenotype Gm(a‐b‐x‐) in 22 patients; the occurrence of this phenotype was not due to hypogammaglobulinemia.RésuméLa recherche du facteur Gm(a) dans 1088 cas d'hémopathies diverses a montré une diminution de sa fréquence par rapport à la population normale, surtout au cours de la leucémie lymphofde (25,8 %) et des leucoses aiguës (39,l %).L'étude des facteurs Gm(a), Gm(b) et Gm(x) dans 89 cas de leucoses a révélé l'existence, en dehors des phénotypes Gm connus dans les populations normales, d'un phénotype Gm(a‐b‐x‐). Ce phénotype a été rencontré chez 22 malades, son apparition ne peut pas être attribué à une hypo‐gammaglobulinémie globale.ZusammenfassungAnläßlich der Gm(a)‐Bestimmung bei 1088 Patienten mit verschiedenen hämatologischen Krankheiten wurde festgestellt, daß bei mehreren Kranheitsbildern insbesondere der lymphatischen Leukämie (25,8%) und den akuten Leukosen (39,1%) die Frequenz Gm(a)‐positiver Individuen geringer ist als in der Durchschnittsbevölkerung.Bei der Bestimmung der Faktoren Gm(a), Gm(b) und Gm(x) bei Leukosen 89 fand sich, nebst den bekannten Gm‐Phanotypen, bei 22 Patienten der außergewöhnliche Phänotypus Gm(a‐b‐x‐).
