AbstractWe evaluated three filters used for leukocyte removal from platelet concentrates: Imugard IG 500, Pall PL100 and Sepacell PL‐10A. Filter performance, platelet activation and expression of membrane glycoproteins Ib and IIb‐IIIa were evaluated. Imugard, Pall and Sepacell showed median postfiltration in vitro platelet recoveries of 88, 84 and 80%, and total residual leukocyte counts of 16.1, 7.5 and 0.6 times 106/pool of 8 platelet concentrates, respectively. Mean platelet volume was reduced after filtration with all filters. Postfiltration values of glycoproteins Ib and IIb‐IIIa, and of activation markers GMP 140 and gp 53 were not significantly different from prefiltration values. Filtration through Imugard, Pall and Sepacell did not induce significant platelet activation or modifications of platelet membrane glycoproteins Ib and IIb

Abstract‘Top‐and‐bottom’ bags have an outlet at the top and at the bottom of the collecting bag, allowing simultaneous expression of plasma and red cells, whereas the buffy coat remains in the collecting bag. The composition of blood components was investigated following manual separation of whole blood in a conventional 4‐bag system (A) or automated separation in a ‘top‐and‐bottom’ system (B). To overcome inter‐donor differences, two units of whole blood were pooled and redistributed into the original bags (A and B) prior to centrifugation. Leukocyte‐poor platelet concentrates (LPPC) were manufactured from both types of buffy coat (A and B). The volumes of plasma, red cell concentrates (RCC) and buffy coat were similar in both methods. However, the residual leukocytes and platelets in the RCC from the top‐and‐bottom system were significantly lower than in the conventional system, 140 ± 59 times 106(mean ± SD) versus 762 ± 228 times 106, respectively (p<0.01). Both types of LPPC contained less than 107leukocytes and could be stored for 7 days maintaining a pH above 6.5. AbstractThe pretreatment of LEW (RT‐11) rat recipients with antibody‐coated BN (RT‐1n) blood cells resulted in the suppression of the LEW antibody response against subsequent BN platelet and leukocyte transfusions. Both coating with homologous LEW‐anti‐BN serum or heterologous rabbit‐anti‐rat lymphocyte serum was effective. Repeated boosting with uncoated platelets or leukocytes did not abrogate the suppression. The degree of suppression was dependent on the amount of antibody used for cell coating. AbstractThe case of a 58‐year‐old male with severe anemia after hemicolectomy is described. The patient proved to be Kp(b–), and the serum contained anti‐Kpb. Because no Kp(b–) donors were available, two incompatible units are administered after intravenous gammaglobulin (400 mg/kg/day) and hydrocortisone (500 mg). AbstractA 65‐year‐old man with bladder outlet obstruction due to prostatic hypertrophy was incidentally discovered to have cold‐antibody autoimmune hemolytic anemia (cold‐aggluthinin syndrome; CAS) due to autoanti‐I (titer 1,024 at 4°C and 64 at 30°C), and a biclonal gammopathy. Immunofixation electrophoresis of serum and a red blood cell eluate revealed the patient's autoantibody to be biclonal IgMx and IgAx. No underlying cause could be determined to explain the development of either the biclonal gammopathy or the CAS. To our knowledge, this is the first reported case of CAS associated with a biclonal gammopathy and biclonal cold autoagglutinins of the IgMx

AbstractThe introduction of a 7.5% hypertonic saline/6% dextran 70 (HSD) solution into clinical trials for the treatment of hypovolemic states, and the past concerns regarding the possible interference of dextran with blood serology, prompted us to investigate the effects of HSD on human red‐cell typing and stability. HSD was evaluated with fresh and 35‐day stored CPDA‐1 red cells from 12 healthy donors. A 1:5 mixture of HSD to blood in vitro had no effect on ABO, Rh, and MN typing in both fresh and stored blood. HSD produced no significant lysis with fresh cells and a minimal level with stored blood. No evidence of metabolic or morphologic changes was seen after HSD treatment. The results of this study suggest that the clinical use of HSD for the treatment of hemorrhagic shock will not affect blood group determinations or red‐cell stability from stored blood which may be infused after the HSD‐treated patient is transported to a

AbstractThe antigens Fl and Vo, recognized by cold agglutinins, are protease‐resistant differentiation antigens like I and i antigens. In contrast to Ii, Fl and Vo are sialidase‐susceptible antigens like the protease‐resistant Gd antigens. Because I/i antigens are branched/linear type‐2 (poly‐N‐acetyllactosamine or neolacto series) chains which are cleaved from red cells by endo‐β‐galactosidase, the effect of the enzyme on Fl, Vo and Gd antigens was studied. Fl and Gd antigens, previously shown to be sialylated branched (Fl), and linear and branched (Gd) type‐2 sequences, were not inactivated on red cells by the enzyme, whereas Vo antigenicity was completely abolished. AbstractA case of mild hemolytic disease of the newborn is presented which was caused by an antibody to a hitherto unknown antigen of low incidence. This antigen, now designated as HOFM (ISBT number 700050) was detected in 6 relatives, and in all of them, it was associated with an unusually weakened expression of C antigen. The serological data indicate that HOFM may be part of the Rh system, but the genetic data, although supportive of this interpretation, are inconclusive.
