In a study of 227 platelet concentrates the average number of lymphocytes in each unit was 66 times 106. There was good viability of the mononuclear cells up to 10 days and the percentage of T and B cells and the OKT4+ to OKT8+ ratio appeared unchanged with storage. At day 8 the mononuclear cells were able to respond to phytohaemagglutinin as assessed by the [H3] thymidine uptake at 72 h. The number of mononuclear cells in the concentrates varied with the technique of separation. This emphasises the need for standardisation of optimum methods of separation for platelet concentrates.

IgG in factor VIII preparations was studied as a possible cause of adverse reactions following infusion of hemophiliacs. Thin‐layer immuno‐gel filtration analysis of nine products from seven firms demonstrated aggregated and monomeric IgG at highly variable amounts and proportions. These factor VIII products contained from 20 to 700 mg IgG per 1000 IU of factor VIII; IgG‐fibrinogen complexes were demonstrated by double‐antibody testing.A relationship is suggested between aggregated IgG and/or IgG complexes in factor VIII concentrates and adverse clinical reactions. The requirement for hepatitis B immunoglobulin for passive immunisation persists despite the availability of a safe hepatitis B vaccine, and there is therefore a need for immune plasma for immunoglobulin production. We have screened a large number of blood donors to identify potentially suitable hepatitis B immune plasma donors. 24 previously non‐transfused patients were given three transfusions of 200 times 108platelets at 14‐day intervals. Group I (12 patients) received leucocyte‐poor platelet suspensions with a mean contamination of 15 times 106leucocytes per transfusion. Group II (12 patients) received platelets with<5 times 106leucocytes per transfusion. 5 patients in group I and no patients in group II developed lymphocytotoxic antibodies (p == 0.037). A delayed hemolytic transfusion reaction (DHTR) occurring in a 30‐year‐old Caucasian man is described. His blood groups were B, CDe/cDE (R1R2, f‐) K‐, S‐s+, Fy (a + b‐), and Jk (a + b +). Eight days after receiving two units of packed red blood cells, his urine was strongly positive for hemoglobin; serum‐free hemoglobin was 125 mg/dl.
