SummaryThe alpha‐2‐globulin previously reported as 'PAG' has been renamed Pa 1. It appears to be inherited by means of an autosomal gene, possibly recessive. There is no evidence indicating a genetic relation to 16 other independent grouping systems. The appearance of this protein seems to be under hormonal control.RésuméL'alpha‐2‐globuline précédemment appelée »PAG« a été renommée Pa 1. Il semble que cette alpha‐2‐globuline soit conditionnée par un gène autosomal, peut‐être récessif. SummaryActive post‐transfusion shock in rabbits caused by isoprecipitins against γ‐globulin allotypes is described.Rabbits in which immunization with selected homologous globulins resulted in production of isoprecipitins against γ‐globulin allotypes were given transfusions of heteroallotypic homologous serum. After injection of about 20 ml of the serum distinct symptoms of shock were observed as manifested by drop in blood pressure, motor restlessness, convulsions and in several cases death of the animal.The serum isoprecipitin titers in the rabbits ranged from 1/16 to 1/256.Immunized rabbits which failed to respond to antibody production and unimmunized rabbits did not react, or did so very slightly after the injection of 20 ml of group‐incompatible serum.The result of the experiments indicate that differentiation of the serum proteins within the same species and presence of antibodies, likewise the incompatibility in the erythrocytic group system, can lead to post‐transfusion shock.RésuméLes chocs post‐transfusionnels actifs chez le lapin ont été causés par des isoprécipitines contre les allotypes de γ‐globuline et sont décrits.Les lapins immunisés par des γ‐globulines homologues sélectionnées et ayant produit des isoprécipitines contre des allotypes de γ‐globuline ont reçu des transfusions de sérums homologues hétéroallotypiques. Après injection d'environ 20 ml de sérum, des symptomes de choc ont été observés qui se manifestaient par une diminution de la tension artérielle, une agitation motrice évidente, des convulsions, et dans de nombreux cas, la mort de l'animal.Le titre des isoprécipitines sériques du lapin se situait entre 1/16 et 1/256.Les lapins qui ne réagissaient pas et ne répondaient pas par une production d'anticorps et les lapins non immunisés ne présentaient aucune réaction, ou réagissaient très faiblement. après l'injection de 20 ml de sérum de groupe incompatible.Le résultat de cette expérimentation démontre qu'il existe des différences entre les protéines sériques de sujets de la même espèce et qu'il existe des anticorps —au même titre que dans l'incompatibilité érythrocytaire — qui peuvent produire des chocs post‐transfusionnels.ZusammenfassungEs werden posttransfusionelle Schockzustände beim Kaninchen, bedingt durch Isopräzipitine gegen Gamma‐Globulin‐Allotypen, beschrieben.Kaninchen, die mit ausgewählten homologen Gamma‐Globulinen bis zur Bildung von Isopräzipitinen immunisiert worden waren, erhielten Transfusionen von heteroallotypischem homologern Serum. SummaryA single intraperitoneal injection of mice with complete Freund's adjuvant, with and without bovine serum albumin, resulted in transient involution of the thymus and spleen, a wasting syndrome, and an altered peripheral blood picture characterized by lymphopenia and neutrophil granulocytosis. Interestingly, an increased production of antibody was observed during a period of reduced lymphocyte numbers. An analogy has been drawn here to the wasting disease observed in neonatally thymectomized mice.RésuméLorsqu'on pratique chez les souris une seule injection intrapéritonéale de l'adjuvant complet de Freund, avec ou sans albumine bovine, on provoque transitoirement une atropine du thymus et de la rate, un syndrome de cachexie thymoprive et une modification de l'image du sang périphérique caractérisée par une lymphopénie et une neutrophilic. Durant cette diminution des lymphocytes, il est intcressant de noter une augmentation de production des anticorps. Une analogie existe entre cette maladie cachectique et l'image que donnent les souris chez lesquelles une ablation du thymus fut pratiquée à la naissance.ZusammenfassungEine einmalige intraperitoneale Einspritzung von komplettem Freundschen Adjuvans mit und ohne Rinder‐Serumalbumin bewirkte bei Mäusen eine vorübergehende Involution des Thymus und der Milz, ein »Wasting Syndrom« sowie ein verändertes peripheres Blutbild, gekennzeichnet durch Lymphopénie und neutrophile Granulozytose. SummaryThe conditions under which nonhemolytic febrile transfusion reactions developed in eight afebrile patients were investigated. Recipients invariably developed a reaction on transfusion of incompatible white cells if a sufficient quantity of leukocytes was infused. The sensitivity of each patient differed with respect to the number of incompatible white cells which could be tolerated without a reaction. In general, thein vivoreaction to blood from an individual donor could be predicted by the routine leukocyte agglutination test. Mild febrile reactions were inconstantly observed in three patients transfused with presumably compatible leukocytes. Further investigation suggested that the cells inducing reaction were probably incompatible, and thatin vivoreactions were a more sensitive index than the invitrotest. Granulocytes, lymphocytes or platelets, separately administered, all induced febrile reactions, provided that the routine leukocyte agglutinin test demonstrated an incompatibility. There was no direct evidence that febrile reactions had occurred in response to plasma infusion. It is concluded that white cell antibodies were the primary cause of nonhemolytic febrile transfusion reactions in this series, and that the detection of white cell antibodies and preparation of leukocyte‐poor blood continue to be procedures of practical importance in modern transfusion therapy.RésuméLes conditions sous lesquelles des réactions fébriles transfusionnelles non‐hémolytiques se sont développées chez huit patients afébriles ont été recherchées. Les receveurs ont développé invariablement une réaction transfusionnelle à l'égard de leucocytes incompatibles si la quantité de leucocytes infusés était suffisante. La sensibilité de chaque patient différait selon le nombre de leucocytes incompatibles qui pouvait être toléré sans provoquer de réaction. En général, les réactionsin vivoà l'égard du sang de tel ou tel donneur pouvait être prédite en pratiquant le test d'agglutination leucocytaire de routine. Des réactions fébriles moyennes ont été observées d'une manière inconstante chez trois patients transfusés vraisemblablement avec des leucocytes compatibles. D'autres investigations laissent suggérer que les cellules ayant provoqué la réaction étaient probablement incompatibles et que les réactions observéesin vivoétaient un index plus sensible que les réactions obtenuesin vitro. Les granulocytes, les lymphocytes ou les plaquettes, administrés séparément, peuvent tous provoquer des réactions fébriles lorsque le test d'agglutination leucocytaire de routine démontrait une incompatibilité. Il n'y a pas eu de démonstration évidente directe que les réactions fébriles soient provoquées à la suite d'une infusion de plasma. On conclut que les anticorps anti‐leucocytaires sont la cause primaire des réactions transfusionnelles fébriles nonhémolytiques dans cette recherche et que la mise en évidence des anticorps anti‐leucocytaires et la préparation de sang pauvre en leucocytes constituent un mode de faire d'une importance pratique non négligeable dans la thérapeutique transfusionnelle moderne.ZusammenfassungDie Bedingungen, unter welchen bei acht afebrilen Patienten nicht‐hämolytische febrile Transfusionsreaktionen zustande kamen, wurden genau untersucht. Die Empfänger zeigten ausnahmslos eine Reaktion im Anschluß an die Transfusion von inkompatiblen weißen Blutkörperchen, sofern ihnen solche Zellen in genügender Menge infundiert wurden. Die Sensibilität hinsichtlich der Menge weißer Zellen, die eben noch reaktionslos ertragen wurden, wechselte von Patient zu Patient. Im allgemeinen gelang es, diein vivo‐Reaktion auf Blut eines Einzelspenders an Hand des Leukozytenagglutinationstestes voraus zu sagen. Bei drei Patienten wurden bei Transfusion von augenscheinlich kompatiblen Leukozyten mitunter leichte febrile Reaktionen beobachtet. Bei der weiteren Abklärung zeigte sich, daß es sich höchstwahrscheinlich auch in diesen Fällen um inkompatible Leukozyten gehandelt hat. Diein vivo‐Reaktionen erwiesen sich als empfindlicher als diein vitro‐Tests. Falls der Leukoagglutinintest positiv war, traten nach separater Transfusion von Granulozyten, Lymphozyten und Plättchen Fieberreaktionen auf. Direkte Hinweise, daß auch durch Plasma solche Fieberreaktionen erzeugt wurden, konnten nicht beigebracht werden. Nachdem auf Grund dieser Untersuchungsreihe anzunehmen ist, daß Leukozytenantikörper die Hauptursache für nicht‐hämolytische Fieberreaktionen darstellen, erweist sich der Nachweis von Leukozytenantikör

SummaryA case of severe haemolytic reaction following ingestion of Dipyrone (Novaldin) is described. Serological investigation revealed an antibody that reacted with human red cells in the antiglobulin test. The reaction occurred only in the presence of Dipyrone. The antibody was specific for the drug and had no blood‐group activity. The antibody appears to be a thermolabile gamma globulin (IgG). It is suggested that the antibody has partial specificity for red cells and the drug.RésuméUn cas d'une réaction hémolytique grave faisant suite à l'ingestion de Dipyrone (Novaldin) est décrit. Les investigations sérologiques ont révélé la présence d'un anticorps qui réagissait avec les érythrocytes humains au test à l'anti‐globuline. La réaction se produisait uniquement en présence de Dipyrone. L'anticorps était spécifique pour le médicament et n'avait aucune activité de groupe sanguin. L'anticorps est apparu comme étant une gamma‐globuline thermolabile (IgG). On suggère que l'anticorps a une spécificité partielle pour l'érythrocyte et pour le médicament.ZusammenfassungEs wird eine schwere hämolytische Reaktion nach Einnahme von Dipyrone (Novaldin) beschrieben. Anläßlich der serologischen Untersuchung fand sich ein Antikörper, der mit menschlichen Erythrozyten einen positiven Antiglobulintest zeigte.Diese Reaktion war nur in Anwesenheit von Dipyrone positiv. Der Antikörper war spezifisch für das Medikament und zeigte keine Blutgruppenaktivität. Der Antikörper erwies sich als thermolabiles Gamma‐globulin (IgG). SummaryThe second example of anti‐Ku has been identified in a patient possessing the phenotypeKo. The serological and genetical investigations are described. It was considered likely that the patient had only one antibody in her serum. It was subsequently postulated that McLeod cells which failed to react with the original anti‐Ku, triple eluate, in fact possess a weak Ku antigen, as well as weak k, Kpband Jsbantigens.RésuméLe second exemple d'anti‐Ku a été identifié chez un patient possédant le phénotype Ko. Les recherches sérologiques génétiques sont décrites. Il a pu être démontré que le patient n'avait qu'un seul anticorps dans son sérum. Il a été ensuite postulé que les érythrocytes McLeod qui ne réagissaient pas avec l'anti‐Ku original, après trois élutions, ces érythrocytes possèdent cependant, en fait, un antigène Ku faible ainsi qu'un antigène k, Kpbet Jsbfaibles.ZusammenfassungDas zweite Beispiel eines Anti‐Ku‐Antikörpers wurde bei einer Patientin mit dem Phänotyp Kogefunden.
