Pasteurella multocida has been reported only once previously as a cause of peritonitis in a patient undergoing chronic peritoneal dialysis. The present report describes findings associated with a case of P. multocida peritonitis in a human immunodeficiency virus (HlV)-positive patient in which renal replacement therapy consisted of continuous cycling peritoneal dialysis. To our knowledge this is the first report of this unique infection in an HIV-positive end-stage renal disease patient. In addition, the recent literature on this unusual organism is reviewed in detail. These findings emphasize the potential for increased susceptibility to zoonoses in immunocompromised patients, particularly with indwelling intraperitoneal catheters which may serve as a portal of entry for unusual organisms.

A 30-year-old black male with end-stage renal disease of 8 years’ duration had undergone 16 prior access operations and was still without an adequate access for dialysis. A spiral saphenous vein graft conduit was constructed from the left saphenous vein and used as a straight arterial conduit graft between the superficial femoral artery at Hunter’s canal and the saphenofemoral vein junction in the groin. A 22-year-old woman presented glomerulonephritis with Schonlein-Henoch-like syndrome and monoclonal abnormality. One month later, she developed a rapidly progressive glomerulonephritis with hypertension and persistent purpura. In the two renal biopsies performed during the first and the second attack, mesangial expansion and thickening of the glomerular capillary walls (associated with 50% of crescents in the second biopsy) were observed on light microscopy. By immunofluorescence faint deposits of immunoglobulins (light and heavy chains) and complement components were found present in the mesangium. Electron microscopy showed tubular microfibrils measuring 19–24 nm in the mesangium, subendothelial and subepithelial areas. A skin biopsy performed during the first attack demonstrated leukocytoclastic skin vasculitis. By immunofluorescence, no deposits were observed.
