Within 3 weeks of beginning continuous daily isoniazid and rifampin therapy for pulmonary tuberculosis, a patient developed acute renal failure. Renal biopsy demonstrated lesions characteristic of rapidly progressive glomerulonephritis with minor interstitial changes. Prior reports have emphasized the tubular and interstitial lesions associated with intermittent or discontinuous rifampin therapy for tuberculosis. This report documents the occurrence of severe glomerular pathology associated with continuous, daily antituberculous therapy. The spectrum of renal lesions occurring during antituberculous therapy, particularly in association with rifampin, may be wider than previously suspected.

317 patients with suspected or documented infections other than cystitis were randomly assigned to receive gentamicin or tobramycin dosed according to the Sawchuk/Zaske method or a modification of the McHenry method. 196 patients completed 6 days of therapy, had serial determinations of serum creatinine and were evaluated for nephrotoxicity. Within each dosing method group those patients receiving gentamicin and tobramycin had a similar mean age, initial serum creatinine level, total dose, duration of therapy and trough serum aminoglycoside concentration. Nephrotoxicity developed in 5 of 49 (10.2%) given gentamicin via the McHenry method compared to 9 of 49 (18.4%) given tobramycin. Similarity, 4 of 50 (8.0%) given gentamicin via the Sawchuk/Zaske method developed nephrotoxicity compared to 8 of 48 (16.7%) given tobramycin. Within the Sawchuk/Zaske and the modified McHenry dosing method groups, no significant difference was noted in the incidence of nephrotoxicity associated with gentamicin and tobramycin. 34% of patients with elevated trough serum aminoglycoside concentrations developed nephrotoxicity compared to 3.7% of those with nonelevated troughs (p < 0.0005).

A detailed survey of dialysis facilities in Texas (ESRD Network No. 11) evaluated the health care delivery to all ESRD patients under 21 year of age. 38 units reported data on 115 patients (50 males, 65 females) ranging in age from 7 months to 20 years. Home peritoneal dialysis was used in 20% of the patients with the rest treated by in-center hemodialysis. 50% of the patients were active transplant candidates at the time of survey; of the remaining 58 patients, there were medical reasons for the nontransplant status of 33 but 25 patients elected to stay on dialysis in the absence of medical contraindications to transplant. The units provided excellent support for the psychosocial, educational, nutritional and growth needs of the children and adolescents surveyed.

To investigate the role of circulating immune complexes (CIC) in the pathogenesis of acute poststreptococcal glomerulonephritis (AGN), sera were obtained serially from 13 patients with biopsy-proven AGN, 16 patients with group A streptococcal infection, and 20 age- and sex-matched controls. Samples were analysed for Clq-binding activity (Clq-BA), levels of IgG, IgA, IgM, C3 and C4, and antibody titres to streptococcal enzymes. Significant elevation of Clq-BA was observed in 11 patients (84.5%) with AGN and 7 patients (44%) with streptococcal infection alone. The data suggest that CIC do not necessarily cause glomerular damage, but rather represent a systemic inflammatory response in patients with group A streptococcal infection.

Marked declines in platelet numbers were noted in association with hemodialysis. This resulted in gastrointestinal bleeding and the need for packed RBC and platelet transfusions. This hemodialysis-associated thrombocytopenia was ameliorated by changing the dialyzer in use. The gastrointestinal bleeding stopped and the need for platelet transfusions was obviated. The degree of platelet loss during hemodialysis is probably affected by the composition of the dialyzer membrane used as well as other factors. Hemodialysis-associated thrombocytopenia may be a contributing factor in the increased bleeding tendency noted in hemodialysis patients.

A case of successful cadaveric renal transplantation is reported in a patient with irreversible renal failure due to myeloma kidney. The clinical features of this case along with those of the 5 previously reported cases are summarized, as are available details of patient management and follow-up. In selected patients with irreversible renal failure due to myeloma kidney, renal transplantation represents an excellent therapeutic alternative.

The occurrence of renal insufficiency associated with heavy glomerular proteinuria during the course of continuous rifampin therapy in a patient with pulmonary tuberculosis is reported. Renal biopsy demonstrated acute interstitial nephritis, normal glomerular histology, effacement of glomerular epithelial cell foot processes, and electron-dense deposits in mesangial matrix and subendothelial and paramesangial sites. Discontinuation of rifampin was followed by improvement in renal function and abatement of proteinuria. We call attention to the broad clinical and morphologic spectrum of nephrotoxicity which may be induced by rifampin and a variety of other drugs.
