ABSTRACTNormal nephrogenesis is reviewed in order to distinguish it from nephroblastomatosis. A number of features of the biology and behavior of Wilms' tumor show the overlapping effects of teratologic, chromosomal, and genetic influences on the genesis of this neoplasm. The age at presentation and histologic types of Wilms' tumors are also reviewed briefly in view of their relevance to nephroblastomatosis.

ABSTRACTAlthough much is known about the hematologic and pathophysiologic aspects of autoimmune hemolytic anemia in man, the pathogenesis of the autoimmune process remains to be elucidated. Toward this goal, several experimental animal models, in particular the New Zealand black mouse, have been extremely important in laying the foundations of our present knowledge of the pathogenesis of the autoimmune process. The interaction of environmental, genetic, and immunologic factors plays an important role in the clinical manifestations of the hemolytic disease and the eventual outcome of the autoimmune process. The increased frequency of autoimmune disorders in patients with primary immunodeficiency disorders emphasizes the importance of disturbances of immunoregulation and immune dysfunction in the pathogenesis of autoimmune hemolytic disease.

ABSTRACTTwenty‐three pediatric patients with severe aplastic anemia were evaluated using the assay for colony forming units in culture. In co‐culture experiments, bone marrow from 5 of the 23 patients suppressed normal bone marrow growth greater than 50%. Of the 11 patients who subsequently received immunosuppressive therapy, consisting of antithymocyte globulin, five patients obtained a clinical response. The presence or absence of suppression in the co‐culture assay was not predictive of the clinical outcome. Although autoimmune mechanisms may indeed be responsible for the aplasia in certain cases of severe aplastic anemia, furtherin vitrostudies need to be developed to identify patients in whom autoimmune mechanisms are operative.

ABSTRACTAdriamycin® has been shown to be a superoxide‐inducing agent and trimethoprim‐sulfamethoxazole (TMP‐SMZ) has been reported to induce hemolysis in some G6PD deficient patients. Many cancer patients are routinely placed on TMP‐SMZ as prophylaxis againstPneumocystis carniiinfection. Two black A‐ variant G6PD deficient adolescents were treated with combination chemotherapy which included 35 or 50 mg/m2of Adriamycin®. Both were on TMP‐SMZ prophylaxis. No evidence for hemolysis was detectable in either patient by serial determination of hemoglobin, hematocrit, reticulocyte count, and haptoglobin level. Whether this combination will produce hemolysis in other variants of G6PD deficiency needs to be tested.
