Between May 1971 and June 1978, 72 children received bone marrow transplants as treatment for their aplastic anemia. Although graft rejection was a major problem for 12 patients, 44 are surviving with complete hematologic reconstitution.A total of 131 children with leukemia were transplanted between August 1970 and December 1977. Although recurrent leukemia is a major problem, there appears to be an improved overall survival for those children transplanted while in remission. Fifteen of 96 children transplanted in relapse are surviving and 19 of 35 children transplanted in remission are surviving.All these patients received marrow from HLA-matched sibling donors. Future trends in marrow transplantation includes utilization of donors other than matched siblings and transplantation for other marrow diseases.This report will summarize the experience of the Seattle marrow transplant team with marrow transplantation in children with aplastic anemia or acute leukemia.

摘要:
Four preterm infants with severe hemolytic disease of the newborn developed marked leukopenia during the course of repeated exchange transfusions. In all patients leukopenia was transient, with a return toward normal values well established within 1 day following the last exchange. Our data suggest that profound leukopenia may be a more common complication of multiple exchange transfusions than has previously been appreciated.

Possible genetic and environmental determinants of multifocal nodular renal blastema (NRB) were investigated by interviewing parents of affected children and matched controls. We found significant associations with frequent use of coffee or tea and exposure to synthetic progestins during index pregnancies. Associated congenital anomalies, particularly hemihypertrophy, were unusually frequent in patients with Wilms' tumor and NRB. NRB alone may be specifically linked to isolated congenital heart disease in infants. Sibs of index cases were similar to controls in number and type of anomalies and rarity of cancer to date, but the families of index children have excess cancer prevalence compared to controls. The data suggest that environmental factors may be important co-determinants of Wilms' tumor. A genetic determinant, if it exists, may have a general, rather than tumor-specific, effect.

This study investigates how the classroom teacher views the functioning of the children with cancer in the regular classroom. The population under study included all school-aged children, grades K–12, who receive treatment at two local pediatric cancer centers, and did not focus solely on those children who manifested problems. A 34-item questionnaire was developed as a forced-choice instrument detailing school-related behaviors. The teachers of the 42 children with cancer filled out the questionnaires, both for the child with cancer and for a typical student in the same classroom, one who represented the general characteristics of the class population.Results were as follows. The children with cancer scored at a level demonstrating behaviors significantly at variance with those of the controls. The children with cancer did not differ from controls on items such as willingness to attend school, apprehension regarding school, play habits, clinging behavior, or physical appearance. They did differ from controls in lower attendance, in having difficulties concentrating, and in being underactive and lacking in energy. The children with cancer appeared inhibited, less active, less willing to try new things. Finally, the children with cancer were seen as less able or willing to express emotions, either positive or negative. Comments are drawn regarding how to help the children to fulfill their wishes to participate actively in school and be as normal as possible.
