Human parvovirus (HPV) infection has recently been implicated as the cause of aplastic crisis in patients with hemolytic anemias such as congenital spherocytosis and sickle cell anemia. The virus causes a transient red cell aplasia which, in patients with a shortened red cell life span, is manifested as a rapid worsening of the anemia and an absence of peripheral reticulocytosis. Recovery is associated with the presence of giant pronormoblasts in the bone marrow, and several days later, a brisk peripheral reticulocytosis. In normal subjects, HPV causes erythema infectiosum (fifth disease) but is not associated with symptomatic anemia, probably because of the duration of the normal red blood cell life span. A case of HPV infection producing severe anemia in an immunocompromised patient without an underlying hemolytic anemia is presented here. Infection in this patient, a 3-year-old boy with acute lymphoblastic leukemia in remission, may have been prolonged by immunosuppression, leading over a 4-week period to a severe anemia. The immunosuppressed appear to be another group of patients at risk of developing symptomatic anemia when infected by HPV.

As chemotherapy has become more effective, patterns of relapse have begun to change. We report a patient with T-cell acute lymphoblastic leukemia (ALL) who had an isolated mandibular relapse that was first manifested as dental pain. Eight other recently reported patients with acute leukemia who relapsed in the mandible are reviewed. In four the relapse was isolated to the mandible; all four had ALL. Local control of disease was achieved with radiotherapy, often combined with chemotherapy. However, our patient and two of the other four subsequently relapsed and died of refractory leukemia.

Pediatrie residents are exposed to a massive amount of scientific information and data from each pediatrie sub-specialty discipline. The residents may or may not know what is important and what is relevant from a general pediatrician's point of view. Educational objectives have been written by three pediatric hematologists/oncologists to guide the resident specifically as to the minimal expectations of knowledge and performance in this discipline. Other pediatrie residency programs may wish to use the objectives as a starting point or an outline for their own objectives.

The treatment of neuroblastoma must take into account the prognosis as determined at the time of diagnosis. The stage is an important prognostic factor. In Evans's Stage I neuroblastoma, surgery is curative in itself and adjuvant treatment is not needed. In Evans' Stage II, it is our opinion that the presence of involved lymph nodes worsens the prognosis. Therefore, we suggest that patients with Stage II should be divided into Stage IIA (without lymph node involvement) and Stage IIB (with lymph node involvement). The treatment of Stage IIA patients may be the same as for Stage I patients, i.e., surgery alone. However, Stage IIB patients should be treated with intensive combined chemotherapy of short duration following surgery.

Ataxia-telangiectasia (AT) is a rare multisystem disorder characterized by progressive cerebellar ataxia, frequent sinopulmonary infections, and telangiectasia of bulbar conjunctiva and/or skin (1). Frequency of various malignant neoplasms in patients with AT has been estimated to be more than 120 times greater than that of age-matched controls (2). More than 80% of the reported malignant neoplasms in patients with AT were of lymphoreticular origin (2).We present a case of an 18-year-old female with AT who developed papillary carcinoma of thyroid.

The most common endocrinologic abnormalities in his-tiocytosis X are diabetes insipidus and growth hormone deficiency resulting from histiocytic infiltration of the hypothalamus (1). The development of goiter, hypothyroidism, and thyroid involvement is quite uncommon, having been reported in only 11 publications (2–12). Our purpose is to report a patient who not only had thyroid involvement, but had such massive involvement that he developed sudden respiratory arrest secondary to upper airway obstruction.

The association of osteogenic sarcoma developing either synchronously or metachronously with acute lymphocytic leukemia is not known (1,2). We report an interesting case of osteogenic sarcoma developing metachronously in a young girl of 7 years after she was treated successfully for acute lymphocytic leukemia.

The association of Klinefelter's syndrome with acute nonlymphocytic and lymphocytic leukemia has recently been described. Klinefelter's syndrome, a disorder marked by a 47 XXY karyotype, has also been associated with male breast cancer, embryonal carcinoma, teratoma, and primary mediastinal germ cell tumor.Acute megakaryoblastic leukemia in childhood has primarily been reported to involve chromosome 21, including five cases of trisomy 21 and one constitutional ring 21 chromosome. We here report a 10-month-old boy with acute megakaryoblastic leukemia and an XXY karyotype in both his bone marrow and somatic cells.

Significant discrepancies between the hemoglobin and hematocrit by an electronic counter such as the Coulter counter do not usually occur. The percentage value in the hematocrit in grams per deciliter is approximately three times the value of hemoglobin. Recently, we observed a patient with autoimmune hemolytic anemia secondary to infectious mononucleosis who had a hematocrit value (in %) lower than the hemoglobin value (in g/dl) by the Coulter counter. The lower hematocrit occurred because of in vitro hemolysis.
