ABSTRACTThe effect of co-trimoxazole on methotrexate (MTX) pharmacokinetics was studied in seven children with ALL in remission receiving weekly maintenance therapy. Each child received MTX alone during one course, and MTX with co-trimoxazole during the next. Co-trimoxazole did not cause any significant alteration in either intestinal absorption of MTX, the degree of MTX plasma protein binding, or the average concentration x time exposure to MTX. These results indicate that low doses of co-trimoxazole do not perturb the serum pharmacokinetics of MTX. However, interaction of these drugs at the cellular level still remains a distinct possibility.

ABSTRACTErythroblastic islands are distinct anatomical units allowing direct transfer of ferritin molecule into developing erythroblasts. A patient with Fanconi's anemia who developed acute erythroleukemia and hypotransferrincmia exhibited numerous erythroblastic islands in the bone marrow. This unusual finding in our patient is thought to be in response to an increased demand for iron caused by neoplastic crythropoietic proliferation in the absence of an adequate amount of transferrin.

ABSTRACTA family history encompassing four generations is presented. Definite cyclic neutropenia has been noted in two generations. In one member, the symptoms abated as she grew older. One 12-year-old member died of clostridial septicemia secondary to gangrene of the terminal ileum. At autopsy he was found to have amyloidosis. A trial of lithium carbonate therapy was carried out in a 7-year-old male but a positive hemato-logic response was not obtained.

ABSTRACTSiblings of pediatric oncology patients in active therapy met as a group for 5 consecutive weeks with two oncology team members. The parents met simultaneously in another location in the hospital. The goals were to provide (1) a support group for the siblings, (2) information regarding the various treatment regimens, and (3) specific knowledge regarding the particular stresses of specific therapy programs. The following problem areas were identified: (1) reiteration of resentment toward the patient due to the amount of attention focused on them by family and friends, (2) embarrassment in talking about the patient's illness, and (3) the relationship between the parent's willingness to discuss the patient's illness at home and the sibling's comprehension of the illness.

ABSTRACTThe histiocytosis syndromes consist of a heterogeneous group of disorders referred to as histiocytosis X or differentiated histiocytosis. Although a large proportion of cases of histiocytosis remains to be of an idiopathic nature, there are cases in which the etiologic mechanism appears to be a defect in the immune system. Provided is a brief review of the literature regarding the immune system in histiocytic disease and a review of the experience at the University of Minnesota with immunologic evaluation of children with histiocytic reactions. Two general hypotheses on the relationship between the immune system and histiocytosis syndromes are discussed.

ABSTRACTEosinophilic granuloma in children occurs in descending frequency in the skull, femur, ribs, pelvis, and spine. The varying degrees of radiographic diagnostic certainty indicate that biopsy is frequently required for diagnosis. The orthopedist should extend his involvement in such patients from the biopsy to prevention of deformity during the subsequent treatment and healing of the lesion.

This paper summarizes the results of clinical trials with both single-agent and multiple-agent treatment regimens and describes the prognostic variables which must be considered in designing future clinical trials.

ABSTRACTExcellent results can be obtained with radiation therapy in the primary management of localized histiocytosis, usually found in bone. When the disease is more widespread, radiation therapy is an effective adjunct to systemic chemotherapy, particularly in control of local symptoms. Doses in the range of 600–1000 rads are effective in producing local control in most instances.

ABSTRACTResidual disabilities are seen in more than half of children who survive histiocytosis. These problems are secondary to either continuously active disease or scarring of previously affected tissues.Fatal outcome from disabilities are particularly seen with pulmonary disease, either from progressive fibrosis or complicating opportunistic infections. “Second” tumors have been seen in association with radiation: brain tumors, osteosarcoma, and thyroid carcinoma. No cases of chemotherapy-related malignancy have been reported.

ABSTRACTScrum α-hydroxybutyrate, dehydrogenase (α-HBDH) levels were determined in 28 children with homozygous sickle cell disease (SCD) during the steady state, crisis, and postcrisis periods. Serum α-HBDH levels were also studied in 20 patients with infection. In patients during the steady state, serum α-HBDH levels were significantly above control values. During the vaso-òcclusivc crisis there was a significant further increase in the α-HBDH levels. In patients with infection, however, there was no significant increase in serum α-HBDH levels. These findings clearly indicate that the level of serum α-HBDH in patients with SCD permits differentiation between vaso-occlusive crises, infection, and spurious symptoms.
