&NA;After an explanation of alternative treatment modalities, 30 patients with unresectable liver metastases from colorectal adenocarcinoma chose to have no treatment. Fourteen patients (47 percent) had synchronous liver metastases and 16 (53 percent) had metachronous lesions. Thirteen of the 30 patients (43 percent) had histologic documentation of liver metastases, with the remaining 17 patients diagnosed by liver scan, ultrasonography, or computerized axial tomography. Twenty‐four of 30 patients (80%) had less than 25 percent liver involvement (Stage I), 23 of 24 patients had alkaline phosphatase levels less than twice normal, and 20 of 24 patients were asymptomatic from their liver metastases. The remaining six patients had liver stages II or III at the time of diagnosis. Overall mean survival of the 30 reported patients with untreated metastatic colorectal cancer to liver is 16 months (range, 2 to 58 months; median survival, 12 months).

&NA;From 1978 to 1980, 68 patients with rectal cancer were randomly allocated to either preoperative irradiation plus surgery or surgical treatment without any preoperative measures. The primary aim of the trial was to investigate the 5‐year survival rate in both groups; a secondary aim was to analyze the local recurrence rate and finally the anatomopathologic tumoral classification after surgery. All patients were followed at least 8 years. The preoperative irradiation group (Group A) was submitted to 4000 cGy for 4 weeks and surgery was performed 1 week after irradiation. All tumors were classified anatomically and pathologically according to Broders' and Dukes' classifications. The results indicated that there is a significant difference in the five‐year survival rates in both groups: group A had a corrected survival rate of 80 percent; group B (nonirradiated) had a corrected survival rate of 34.4 percent. The local recurrence rate was 2.9 percent in group A and 23.5 percent in group B. Regarding tumor regression, before radiotherapy 64.6 percent of the tumors were Broders' Grades 3 and 4; after radiotherapy these were reduced to 20.5 percent. As to Dukes' classification, 26.4 percent of the tumors were type C in group A and, in group B, 47 percent were considered as Dukes' C.

&NA;Changes of bile composition in gallbladder bile and serum chemistries were investigated in nine dogs after proctocolectomy and ileoanal anastomosis, using an interposed jejunal segment as a neorectum. A significant decrease in cholic acid and an increase in deoxycholic acid were observed 24 weeks after the operation, although there was no significant change of total bile acid and phospholipids in the bile. Concentration of cholesterol and cholesterol saturation index in bile increased after total proctocolectomy. Although serum triglyceride levels became lower compared with the nontreated control group, concentrations of total protein, serum cholesterol, total lipids, blood sugar, and electrolytes showed no significant difference. This operative procedure apparently did not impair intestinal absorption of bile acid, but did increase the biliary cholesterol saturation index.

&NA;This paper presents two cases of adenosquamous carcinoma of the colon and brings to 39 the total number documented in medical literature. The concurrent glandular and squamous differentiation of the tumor cells was demonstrated by immunocytochemistry and electron microscopy. Evaluation of the biologic characteristics of all the reported cases suggests that malignant squamous elements in colonic carcinomas behave more aggressively than their glandular counter‐parts. In contradistinction from the pure squamous‐cell carcinoma of the colon, adenosquamous carcinoma does not show the same predilection for the right colon.
