&NA;A case of perianal Paget's disease associated with a cloacogenic carcinoma is presented. There was no clinical evidence of the Paget's disease, which was confined to a very small area. The points of particular interest were (1) the association of Paget's disease with a cloacogenic carcinoma, and (2) the dissimilarity between the pagetoid cells and the carcinoma cells. The histologic differences between the two cell types support the view that perianal Paget's disease and underlying carcinomas represent a multicentric reaction to an unknown stimulus

&NA;Two patients with advanced rectal cancer, women aged 73 and 49 years, have survived more than nine years with chemotherapy only and no surgical removal of the disease foci. In one patient, the tumor completely disappeared, and in the other the tumor stopped growing and its size remained unchanged. The reason for this long‐term survival appears to be a high susceptibility to anticancer agents and persistence of enhanced host immunity.

&NA;Marlex®mesh is an excellent prosthetic material for closure of major abdominal defects. Most of its complications are seroma and infections. We have used Marlex mesh intraperitoneally for closure of burst abdomen in a patient who had a gastrectomy for recurrent duodenal ulcer. A year and a half later, this patient developed a fecal fistula to the skin due to incorporation of the Marlex mesh into the splenic flexure of the colon. The patient underwent a second operation during which the fistula was resected and the Marlex removed. We concluded that intraperitoneal placement of Marlex mesh is not recommended.

&NA;A 57‐year‐old man administered an enema to himself, preparatory to intravenous pyelography. He left after the films were taken and could not be reached when retroperitoneal emphysema was detected. He was located 12 days later and found in good health. Abdominal x‐ray films and rectosigmoidoscopy were normal. The patient refused further investigations. It is assumed that a small perforation occurred at the rectosigmoid junction during the self‐administration of the enema. This assumption is borne out by the pattern of air distribution. The total absence of complaints and physical signs is unusual, although not unique; similar cases have been reported previously. The cause of such injury is mechanical, since the high pressures necessary to rupture the rectum are not usually attained in ordinary enemas. Caution is called for in intrarectal instrumentation, especially in older patients.

This development is rare, as rectal carcinoids are more often associated with other carcinomas of the gastrointestinal tract. &NA;Evanescent colitis was first reported in 1971. This clinical entity is manifested by abrupt onset of colicky abdominal pain usually out of proportion to the physical findings, loose stools progressing to hematochezia, and segmental colonic involvement with spontaneous resolution in a matter of days. The diagnosis can be suggested by abdominal flat plate; confirmation depends upon barium‐enerna examination early in the course of the illness. The clinical presentation is identical to that of colonic ischemia with one remarkable exception; while colonic ischemia has come to be regarded as a disease of the elderly, usually with underlying vascular disease, evanescent colitis occurs in young people who are otherwise free of disease.In this report the authors present nine cases whose course is classic for colonic ischemia except that they are all less than 50 years of age and free of underlying vascular disease. Two of the patients were on oral contraceptive medication. A review of the literature revealed 15 additional cases. Five of these cases were associated with oral contraceptives.Conditions to be excluded in the differential diagnosis of this disease are the specific infectious colitides, idiopathic ulcerative colitis, granulomatous colitis and antibiotic‐related pseudomembranous colitis

&NA;Mucinous adenocarcinoma developing in a chronic anal fistula is a rare tumor of the anus of which there are less than 150 reported cases. There has been some debate as to whether the fistula is the source of the tumor, or whether the fistula is the presenting feature of a slow‐growing, indolent carcinoma. Two recent cases seen at our hospital are presented, along with a review of the literature and what we feel to be strong evidence that the fistula and associated anal glands are indeed the source of this unusual tumor.

&NA;We have employed the operative plan described upon 7 patients with cancer of the rectum. This method is itself not without undesirable features but, as a two‐stage operation, has so far appeared to offer a nearer approach to the ideal one‐stage abdominosacral removal of cancer of the rectum than have other procedures.As much delay and preparation as desired are possible between the first and second stages of the operation.The duration of the steps is better divided, the greater part being done in the second stage when the patient is in the best state of preparation.The necessity of implanting dead bowel in the pelvis is overcome.The second stage involves the removal of a clean, empty rectum.Good posterior drainage is established immediately after the extensive pelvic dissection.
