&NA;Geographic location of a population plays a significant role in the interpretation of symptoms of some diseases of the colon. Because diverticular disease of the colon is very rare in the tropics, frank rectal bleeding, which may be suggestive of bleeding diverticulosis when it occurs in a temperate region, is suggestive of a bleeding ileal typhoid ulcer eroding terminal branches of the superior mesenteric artery. Similarly, symptoms suggestive of ulcerative colitis in a temperate region are suggestive of amebic colitis in the tropics. Anatomic variation also plays a part in the nature of some disease processes. The high incidence of freely mobile cecum and ascending colon and the freely mobile redundant sigmoid colon, seen in a tropical population, play a part in the frequent occurrence of nontumid intussusception, relatively high incidence of sigmoid volvulus, and relatively high incidence of rectal prolapse seen in the area.

&NA;The results of subcutaneous lateral internal sphincterotomy for chronic fissure‐in‐ano were analyzed in a series of 60 patients operated upon over a period of two years. Immediate relief from pain with painless first bowel movement was accomplished in 97 per cent of the patients with very low morbidity rates. All patients were treated on an outpatient basis and had a minimum follow‐up of two years without any recurrences. The incidence of sphincter dysfunction is very low and is usually only temporary.

&NA;The patient requiring ileostomy may have the opportunity to choose from four options. The Brooke ileostomy has afforded thousands of patients a secure, comfortable life. Appliances adhere well, and complications are few. The continent ileostomy or Kock pouch permits a patient to forego the wearing of an appliance. It is about 90 per cent successful but has a significant complication rate. Crohn's disease and physical frailties preclude its use. In “endorectal pull‐through”, the colon and rectal mucous membrane are removed to cure polyposis or ulcerative colitis. The ileum is led through the rectal muscular cylinder and anastomosed to the anus. The operation is tedious and not uniformly successful. A period of training must follow. It is unfit for people with Crohn's disease. Ileal anastomosis to a rectal segment is simpler but leaves disease behind. It may be used in Crohn's disease but may expose the patient to the hazard of recurrent disease or cancer. We must understand and ask the right questions if we are to give our patients the right answers.

&NA;Crohn's disease confined to the appendix is a rare entity, less than 50 cases having been reported. The present study reports on another 12 cases representing 6 per cent of all 194 patients operated upon for Crohn's disease in a total, unselected series. The indications for surgery were appendicitis in eight patients, appendiceal abscess in two, suspected pyosalpinx in one, and an ovarian cyst in one. The appendices were in all cases strikingly enlarged. Giant‐cell granulomas, without microabscesses, were detected in all but one patient. Two patients had early septic postoperative complications. Fistulization from the cecum did not occur. The median observation time after operation was 13.8 years. Since none of the patients had further manifestations of the disease, it is concluded that patients with Crohn's disease confined to the appendix have a favorable prognosis.

&NA;Twelve patients with anatomic anal stricture were treated with C‐anoplasty in the past six years. Anal stricture was caused by previous hemorrhoidectomy in ten, fistulectomy in one, and fissurectomy in one. All patients had had conservative treatment from four to 22 years but failed to respond. Eleven patients obtained satisfactory results from C‐anoplasty; one required three anal dilatations postoperatively because of restricture. C‐anoplasty is advantageous because it extends the pedicle without compromising vascular supplies; suture‐line tension can be controlled by extending the incision; and the size of the graft is easily adjustable to anal size.

&NA;Cystic lymphangiomas are rare benign tumors of the gastrointestinal tract. Such a lesion was found in the colon of a man who presented with diarrhea and rectal bleeding. The colonoscopic appearances of a smooth, soft polypoid lesion on a broad base should alert the clinician to suspect such a lesion. Histologically the lesion is characterized by submucosal lymphatic spaces with a smooth muscle component in the wall. Though colonoscopic excision, injection, or rupture of the cyst have been advocated as possible modes of treatment, surgical excision is still considered to be the treatment of choice.
