&NA;We feel that gastrostomy is a safe, effective procedure that can be carried out with dispatch with minimal complications. There was no death or second operation in this series related to the use of this method. Patient comfort and gratitude and avoidance of many problems resulting from the use of nasogastric tubes encouraged us to expand the use of this method in our practice.

&NA;A retrospective study of 100 cases of elective colonic surgery is presented. The effects of antibiotic preparations on the complication rate are discussed, along with the influences of other factors and a review of the recent medical literature. Once more, it appeared that use of an antibiotic preparation was detrimental.

&NA;The object of this paper has been to present an enlightened, much simplified approach to treatment of the hemorrhoid syndrome. The orthodox treatment, especially the teaching, has remained too rigid and stereotyped. The proctologist must strive to develop greater skill in all available techniques, so that he can truly say that no two operations are alike. There is no panacea such as many surgeons imagine that the old stretch operation can now confer on the hemorrhoid problem.Hemorrhoid surgery, as also that of anal fissure, anorectal abscess and fistula, minor plastic operations for anal stenosis, and other associated procedures should be dealt with by experts, usually able to perform such operations using local anesthesia, on a completely ambulant basis. In a purely proctologic practice only about 20 per cent of all hemorrhoid cases require definitive surgery, as most respond satisfactorily to conservative measures.This paper has given much space to discussing the problem of spasm of the anorectal sphincteric mechanism. It is hoped that the surgical conclusions arrived at will help to dispel the mistaken approach of using the old, crude procedure of anal stretch or manual dilatation of the anus to overcome the comparatively simple problem of “the over‐tight back passage.” It is felt that anal stretch has only one true indication, that of its rapid performance at the termination of a low colonic or rectal anastomosis to achieve intraluminar atmospheric pressure at the suture line.The place of cryosurgery in the treatment of hemorrhoids still awaits authoritative analysis.

&NA;The concomitant occurrence total genital prolapse and total rectal prolapse (isochronous) was not rare in our series (5 of 55 female patients with total rectal prolapse, an incidence of 9.27 per cent).In 12 of the 55 cases, an incidence of 21.81 per cent, both disorders occurred, but at different times, total genital prolapse preceding total rectal prolapse that occurred later despite perineal repair and hysterectomy. In four of the 12 heterochronous cases, prolapse of the vaginal vault was present concomitant (isochronous) with total rectal prolapse.It seems that the association of total genital prolapse and total rectal prolapse occurs mainly in women 65 years old or older. Of our five patients, three (60 per cent) were in their 80′ s. (The two patients of Lapeyrere and Grezes‐Rueff were 84 and 88 years old). Parity does not seem to affect the occurrence of total rectal prolapse.In our five cases, the rectal prolapses were surgically corrected first (rectosigmoidectomy by the perineal route), and three to four weeks later, vaginal hysterectomy (four patients) or ample cervical amputation (patient 1) plus a perineal repair was performed. For rectosigmoidectomy, the perineal approach is convenient, using epidural anesthesia. (We have done a couple of cases with local anesthesia, with or without hyaluronidase.)

&NA;Thirty‐eight patients were found to have carcinoid tumors of the rectum in a retrospective study of a 23‐year period at the Ochsner Medical Center. Thirty‐three patients had clinically benign tumors and five had invasive carcinoids with metastasis to regional lymph nodes or the liver. All benign tumors were completely asymptomatic and were discovered on routine rectal and proctoscopic examinations. All patients with malignant tumors had symptoms. The distinction between carcinoids which are clinically benign and those which are malignant can be made only by determining the extent of invasion of the bowel wall or by evidence of metastasis. The gross size of the tumor is important, since most tumors less than 2 cm in diameter are localized, whereas those 2 cm in diameter or larger have invasion and metastasis. Benign rectal carcinoids should be treated with biopsy and thorough fulguration or local excision. However, invasive tumors should be treated by radical resection whenever possible, since the metastatic lesions are often very slowgrowing and the patient may remain relatively asymptomatic for long periods.

&NA;Four cases of the linitis plastica type of carcinoma of the intestine are described. In three, the lesions were regarded as primary, and in the fourth, as metastatic from a primary tumor in the stomach. Two of the four involved the rectum. One was localized to the appendix and the other involved the appendix and adjacent cecum and terminal ileum. Lesions of the right colon manifested symptoms of acute appendicitis. Both rectal lesions produced bleeding, tenesmus, and changes in bowel habits. None of the patients had associated ulcerative colitis. Review of the literature revealed about 100 reported cases of linitis plastica carcinoma of the intestine, of which only 34 were regarded as primary; none occurred in the appendix. The clinical, radiologic and pathologic features are presented, with emphasis on the difficulty in arriving at a correct preoperative diagnosis.
