Expression of the carcinoma‐associated antigens CA 19‐9 and CA‐50 in inflammatory bowel disease |

&NA;The expression of gastrointestinal cancer antigen, CA 19‐9, and of carcinoma‐associated antigen, CA‐50, was studied in formalin‐fixed and paraffin‐embedded tissue from 18 patients with ulcerative colitis, 29 with Crohn's disease in the colon, four with diverticular disease, and eight with sigmoid volvulus. None of the patients with inflammatory bowel disease showed strong dysplasia or had manifest carcinoma. Both antigens were expressed frequently in patients with inflammatory bowel disease. Of the 18 patients with ulcerative colitis, 17 were positive for both CA 19‐9 and CA‐50, and of the 29 with Crohn's colitis, 21 were positive for CA 19‐9 and 22 for CA‐50. No distinct differences in antigenic expression were found between Crohn's disease and ulcerative colitis. CA‐50 was expressed in normal colonic mucosa from 10 of 12 patients with sigmoid volvulus or diverticular disease, and such mucosa was positive for CA 19‐9 in three of the four patients with diverticular disease and in two of the eight patients with sigmoid volvulus. It is concluded that immunodetection of CA‐50 or CA 19‐9 is of limited value in the differential diagnosis of inflammatory bowel disease. The usefulness of these antigens as markers for precancerous changes in inflammatory bowel disease is also doubtful, since the expression is also frequent in cases of inflammatory lesions, with no obviously increased risk of malignancy.

&NA;Postoperative morbidity after colorectal surgery was retrospectively investigated, comparing 32 alcoholics (≥60 gm of alcohol a day) to 32 controls (<25 gm of alcohol a day). The material originated from 260 male patients and the two groups were paired with respect to operative procedure, diagnosis, age‐complicating cardiopulmonary diseases, weight, and smoking habits. Postoperative morbidity was higher (P<.01) in the alcohol group (59 percent with major complications, 75 percent with major and minor complications) compared with the control group (19 percent with major, 33 percent with major and minor). The alcoholics also stayed longer in the hospital (26 daysvs.17 days) (P>.01). This apparent increased surgical risk in alcoholics is reevaluated in prospective investigations.

&NA;Samples of maximally diseased and adjacent macroscopically normal intestine from 52 patients undergoing surgery for Crohn's disease were analyzed for ascorbate content; 26 of the patients had intestinal fistulas and 26 did not. Ascorbate analyses were also performed on samples of normal intestine from ten control patients. Diseased intestine from both groups of patients with Crohn;s disease contained significantly more ascorbate than the adjacent macroscopically normal intestine. Their diseased intestine also contained significantly more ascorbate than normal intestine from controls. Whereas diseased intestine from patients without fistulas contained 47 percent more ascorbate than their normal intestine (P<.001*), the diseased intestine from patients with fistulas contained only 23 percent more ascorbate than their normal intestine (P<.02*). Patients with fistulas appear unable to concentrate as much ascorbate in their diseased intestine as patients without fistulas. This difference may be a factor in the pathogenesis of fistula formation in Crohn's disease because of the importance of ascorbate in collagen production.

&NA;Retroperitoneal cysts have been classified into traumatic, infective, degenerative, neoplastic, and developmental, according to their origin. This paper focuses on the developmental variety of retroperitoneal pelvic cysts, particularly those of mesothelial, mesonephric, or paramesonephric origin. Their clinical presentations depend on their location; they may be mesenteric, paravorian, or vaginal. The pathogenesis and embryologic, diagnostic, and therapeutic aspects are reviewed and a case report is presented.

&NA;Percivall Pott was born in London on January 6, 1714, the son of a scrivener, in a house on Threadneedle Street where the Bank of England now sits. His father died three years later, but with the assistance of a relative, the Bishop of Rochester, Pott was able to attend a private school in Kent. At the age of 16, he demonstrated an interest in surgery and was apprenticed to Edward Nourse, an assistant surgeon at St. Bartholomew's Hospital. It was Pott's responsibility to prepare anatomical subjects for Nourse's lectures. In September 1736, Percivall Pott received his diploma and was admitted to the Company of Barber‐Surgeons. Following dissolution of the Company in 1745, Pott allied himself with the surgeons, having been appointed the previous year as assistant surgeon to St. Bartholomew's Hospital.Pott's reputation as a compassionate and gentle physician can be readily appreciated in his writing on fistula, the subject for thisClassicspresentation. He is harshly critical of the ubiquitous cautery and escharotic dressings. At the age of 43, he published the first of a large number of books. His wirints actually revolutionized the practice of British surgery. His style was direct, simple, and devoid of affectation.Two sections from one of his books are reproduced. The first presentation deals with the treatment of anorectal abscess, and next month, it will be directed to the management of anal fistula.
