SummaryUlcerative colitis confined to the rectum and sigmoid flexure was studied in 124 patients who had normal roentgenograms of the colon. Follow‐up information on 120 patients was obtained; 63 were followed for periods longer than five years, the mean period being 6.2 years.The major symptom was rectal bleeding, which was the primary complaint of all but one patient. Sigmoidoscopic findings were indistinguishable from those of ulcerative colitis affecting more extensive portions of the colon. Biopsy specimens of rectal mucosa of 21 patients were indistinguishable from those of patients with ulcerative colitis. (No case of Crohn's disease was included in the study.)Extension to more proximal portions of the colon occurred in 20 patients and the entire colon was involved in 14. Ten patients required operations, of which three were abdominoperineal resections with colostomy; five were surgical procedures for toxic ulcerative colitis, and two were for carcinoma. No patient required surgical treatment solely for chronic symptoms of colitis. Ulcerative colitis confined to the rectum and sigmoid flexure is more benign and has a better prognosis than colitis involving the entire colon.

SummaryIn a number of cases, during resection of rectosigmoid carcinoma, malignant cells implanted at the site of the anastomosis cause recurrence. This can be prevented by transproctoscopic irrigation of the rectal stump during the operation, using a solution of perchloride of mercury. However, many surgeons are deterred from using this technic because they find that it is somewhat awkward. In this communication, a convenient method for performing the irrigation, using a specially designed flexible rectal tube, is described. No special skill is required to use it and it can be applied easily whenever restorative resection is performed. Thus, many recurrences should be prevented.

Summary and ConclusionsA case of spontaneous internal biliary fistula involving the gallbladder and colon has been presented. Unusual features included massive bleeding from the rectum due to erosion of the cystic artery and the presence of a granulomatous mass in the colon adjacent to the fistula. Treatment consisted of ligation of the bleeding artery, tube cholecystostomy and removal of the involved segment of colon, which resulted in a fecal fistula which closed spontaneously. Subsequently the patient recovered fully.Massive bleeding of unknown origin from the rectum should arouse suspicion of biliary tract disease. Gastrointestinal bleeding manifested as melena is uncommon in cholecystoenteric fistulas.

SummaryA method has been described by means of which cancerous tissue has been made visible in ordinary flat‐plate x‐rays. Polyvinylpyrrolidone (PVP), a blood plasma expander, has been used as the injection medium because its large molecules are trapped or delayed in their passage through primary or metastatic carcinomatous tissues. For this purpose, PVP with bismuth and PVP with iron have been injected alternately into veins of the arm. Three to six weeks later, opacities which can be seen in flat‐plate x‐rays identify and reveal the sharply‐outlined borders of carcinomatous lesions.Exact details of the nature of the chemical reactions involved are not known. It may have something to do with precipitation of bismuth by the more active element, iron, in the cancer tissue. Continuation of this study, probably utilizing radioisotopes in future experiments, is intended.

SummaryTwo cases of rectal oleogranuloma, which simulated carcinoma, have been presented. In each instance, biopsy of an associated lesion suggested the need for radical surgical treatment. The presence of an extensive, possibly constricting but nonulcerated, mass in the rectum, even when high above the dentate line, should arouse suspicion of an iatrogenic oleogranuloma.

SummaryAn unusual instance of a patient with a sigmoidal colostomy of 27 years' duration is presented. Although the anus was completely healed and closed, sphincteric musculature was present and active. The levator ani muscles were present. Apull‐throughprocedure was performed, in which the descending colon was delivered through the perineum and a satisfactorily functioning “anus” was reconstructed. During the period of 31/2 years, since the operation, the patient has enjoyed normal bowel function.

Summary and ConclusionsImportant advances in clinical and experimental studies of fecal continence have been reviewed. In some respects, the results of these studies are contradictory, and much remains to be learned. Although complete clinical success remains elusive, methods for the restoration of both reservoir continence and sphincteric continence appear to be at hand.
