SummaryAn autopsy series of 320 patients who had carcinoma of the rectum or sigmoid flexure has been reviewed and a pattern of metastasis plotted with reference to the duration of survival after operation. It is noteworthy that in many of these patients the tumor remained localized for long periods of time.Regional lymph‐node metastasis did not significantly increase after the immediate postoperative period.Hematogenous metastatic spread rapidly increased during the early months after operation. Various factors which may play a part in hematogenous spread have been discussed and therapeutic implications suggested. Some of the clinical implications resulting from this autopsy study have been reviewed.

SummaryA case of fatal fatty metamorphosis of the liver in a patient with fulminant ulcerative colitis is presented.The patient died in hepatic coma with normal serum ammonia. This unusual finding is related to previous colectomy.The occurrence of liver disease in patients with ulcerative colitis, and the possible factors responsible for these severe histopathologic changes, are discussed.

Summary and ConclusionsIn a series of 99 patients operated upon for ulcerative colitis, 29 had ileal involvement. X‐ray examination is not of great diagnostic value.At operation, gross external evidence of ileal involvement was present in only one third of the patients. Inspection of the opened ileum during surgical exposure is mandatory.The long‐range mortality rate was five times greater in patients with ileal involvement than in those in whom the small intestine was not involved.

SummarySix cases of ischemia of the colon and rectum, secondary to operations on the abdominal aorta, are reported, and added to the 33 cases already published.Changes in the colon varied from mild chronic inflammation to complete gangrene of the bowel wall. Clinically, the symptoms and signs varied from transient diarrhea to fulminating bloody diarrhea with fever and toxicity, and from abdominal cramps and distention to advanced peritonitis and death. The diagnosis and treatment must be made early with prompt removal of the involved segment of colon when indicated.The degree of ischemia of the colon and recovery from it depend upon the anatomy of the collateral circulation, the type and degree of the arterial lesion, the duration of occlusive lesions which may or may not permit the development of adequate collateral circulation, the “occlusion time” at operation, the bacteriologic status of the bowel and the postoperative treatment.The major inflow arcs of collateral circulation are the superior to inferior mesenteric artery connection and the inferior mesenteric to hypogastric artery connection. However, alternate routes may be of importance if these two major arcs are compromised.“Ischemic colitis” resembles acute and idiopathic ulcerative colitis. Sigmoidoscopic examination in the two conditions may reveal almost identical findings. The chronic stages of both conditions are similar. “Ischemic colitis” may respond favorably to the same conservative regimen commonly instituted in the treatment of idiopathic ulcerative colitis.
