&NA;Incidental prophylactic inferior vena cava clipping (IVCC) has been used in 30 patients undergoing colonic operations. The results obtained compare quite favorably with other methods of preventing postoperative pulmonary embolism (PE). This procedure would be justified in patients with a postoperative PE risk greater than 5 to 10 per cent. Practical criteria to identify this group of patients are presented, and the value of utilizing a risk profile is emphasized. There is no mortality from the procedure itself, and the morbidity was limited to lower‐extremity edema in three patients who otherwise could have been expected to develop PE. The edema lasted two months in one patient and cleared rapidly in the other two. Attesting to the procedure's effectiveness, there were no cases of recurrent PE. Caval partition is an appealing mode of prophylaxis in high‐risk patients because of its safety, efficacy, and permanence.

&NA;A total of 116 one‐layer end‐to‐end anastomoses of the colon and the rectum in 112 patients were studied. Three patients died postoperatively (2.6 per cent). Significant disruption of anastomosis requiring a diverting colostomy occurred in five patients. The study is presented to draw attention to the safety of the single‐layer anastomosis.

&NA;Forty‐eight patients treated for chronic incapacitating constipation by surgical resection were reviewed. Thirty‐seven had subtotal colectomy, eight had left colectomy, two had right hemicolectomy, and one had ileostomy, colectomy, and a Hartmann pouch. There were significant complications (21 per cent) and one mortality (2 per cent). A long‐term successful outcome was found in 81 per cent of the patients on follow‐up. Physiologic studies are recommended to improve the selection of patients.

&NA;Fifty‐six patients who had hemorrhoidal crisis underwent urgent radical hemorrhoidectomy with anoderm‐flap repair. There was no significant increase in complications as compared with elective hemorrhoidectomy. One patient (1.8 per cent) had one of his three flaps separate. Anal packing with a central‐tubed cotton plaque is of benefit to ensure takes of anoderm flaps and in reducting postoperative hemorrhage.

&NA;Anorectal manometry under anesthesia was performed in 18 children, 15 of whom had long histories of constipation and had been on regular regimens of laxatives and enemas with no success. In these 15 patients, Hirschsprung's disease had not been demonstrated by barium enema. Anesthesia was induced and maintained with ketamine (intravenously or intramuscularly) in 16 patients, while halothane was used in the other two. It was found that anesthesia did not affect the events that are usually documented during anorectal manometry in the awake patient. In patients with a history of constipation, after mamometry, a rectal suction biopsy was taken, followed by a therapeutic anal stretch. Results of biopsies confirmed the manometric findings in all patients. Results of the present study demonstrate that ketamine may be used to anesthetize young patients suffering from chronic constipation for the purpose of performing anorectal manometry and taking a rectal suction biopsy (to exclude Hirschsprung's disease), followed by a therapeutic anal stretch.
