&NA;Cryogenic rectal surgery is a valuable addition to the therapeutic armamentarium of the proctologist. It permits operative intervention in the poor‐surgical‐risk patient because of reduced anesthetic hazard; ease of postoperative defecation; relative freedom from pain; and immediate ambulation. The same factors warrant its use in a broad spectrum of patients.Excisional surgery, where indicated, was combined with cryodestruction in a single operative session. The postoperative course in these patients was less painful than might be anticipated.A nitrous‐oxide‐cooled cryoprobe (KRY‐med) with nonelectric defrost action was successfully used in the destruction of hemorrhoids, fissure‐in‐ano, condylomas, and fibroepithelial anal polyps in 142 of 145 patients. Complications were minimal.

&NA;In conclusion, it has been shown that cryosurgery equals the anatomic results of excisional surgery, and exceeds excisional surgery in patient acceptance and patient comfort. Of the media available, LN2is the most destructive; N2O provides adequate therapy if the time to reach tissue destruction is remembered before starting the freezing; CO2has too small a temperature range to be dependable as an universal agent for use in proctology.

&NA;Eight patients with ileal‐resection diarrhea who were being treated with cholestyramine took part in a trial comparing the effectiveness of cholestyramine with that of a hydrophilic colloid. Both drugs reduced overall stool frequency and improved consistency, but a larger dose of colloid was required. One patient planned to use the hydrophilic colloid instead of cholestyramine.

&NA;A study of 41 cases where a single layer of 5‐0 monofilament stainless steel wire is used for low end‐to‐side rectosigmoid or ileorectal anastomosis has shown satisfactory results. The procedure should be considered particularly where there is gross disparity between the sizes of the approximated cut ends.
