ConclusionI have tried to present a comprehensive review of medical literature pertaining to this interesting and confusing disease. Although many theories have been postulated, its etiology is still unknown. Several unusual characteristics are stressed.Histologically, giant‐cell systems may predominate, and often they are associated with an absence of the typical granulomatous changes.Fever may be the only presenting symptom; gastrointestinal symptoms may be completely absent. We should consider regional enteritis in the differential diagnosis of any fever of unknown origin.X‐rays may not show the classical string sign. One must also be able to recognize other x‐ray changes such as loop separation, internal fistulization, and irregularities of the mucosal wall.A case has been reported which reveals these unusual features.

Summary and ConclusionsA report has been made of an electromyographic study of the external sphincter ani muscle, utilizing monoterminal and bipolar technics and standardized technic using eight channels. A group study has been made of 28 subjects, 15 of whom had no evidence of anorectal disease; the remainder had diverse anorectal disorders.Evaluation of the findings observed during our study leads us to several conclusions. Perhaps the most important is that concerning the electric behavior of the external sphincter muscle, in which activity does not occur simultaneously in all sectors when pathologic changes affect the structural integrity, or when they indicate a physiologic response which we have classified as segmentary. Lesions that affect motor innervation at any level in the reflex arc, or along its cortical pathways, always register tracings that differ considerably from those that are considered to be normal.We believe that the electromyographic method, so extensively used in the study of other portions of the human body, can also find wide application in the anorectal area. Its relative simplicity and accuracy warrant further investigation and utilization in the diagnosis and pathogenesis of diverse proctologic disorders.We cannot yet assert that the phenomenon of pain is parallel to the increased electric activity of the external sphincter ani muscle, but we believe that further studies will clarify this and certain clinical problems.

SummaryA fatal case of nontraumatic, clostridial, gas gangrene of the right arm is presented. The probable entry site for the organisms was an ulcerating, cecal adenocarcinoma which was found at postmortem examination. If future experience with high‐pressure oxygen proves to be beneficial in the treatment of clostridial myositis, we may see an increase in the survival rate of this disease. It is important to bear in mind that there exists a relationship between nontraumatic, clostridial, gas gangrene and intestinal cancer.

SummaryMalignant tumors of the large intestine and anal canal, excluding lymphomas, account for about 3 per cent of all malignant tumors in patients in Ibadan. This paper analyzes 166 histologically confirmed cases, seen in biopsy and autopsy material over a period of eight years. The most common tumors encountered in the large intestines and anal canal are well‐differentiated adenocarcinomas and squamous‐cell carcinomas. The youngest patient with carcinoma of the colon was 17 years old and the youngest with anal carcinoma was 25 years old. The average age of patients with these tumors seems to be lower than those reported by others. In Ibadan, intestinal tumors are not as common as tumors of the cervix uteri, breast and liver, but are more common than prostatic cancer. In our experience with hospitalized patients, they do not account for a significant proportion of cancer deaths. Analysis of the segmental distribution of colonic and rectal tumors reveals no predilection for the right half of the colon in women. The colon/rectum ratio is about 1.2 to 1, and the sex ratio for both sites is about 1:1. There is a preponderance of women with anal carcinoma, the sex ratio being 12:1.It is suggested that the low incidence of intestinal neoplasms may be attributable to a low incidence of potentially malignant intestinal conditions such as polyposis coli and ulcerative colitis.

SummaryWe have described three patients who had the triad of Gardner's syndrome, with the unusual added feature of skin pigmentation limited to the torso. To lead into a discussion of Gardner's syndrome with skin pigmentation, the history of all types of familial polyposis was first reviewed. It is a familial disease with Mendelian autosomal dominant inheritance. The colonic polyposis in this syndrome is pathologically similar to, and behaves like that of, familial polyposis. However, the appearance of colonic polyps and subsequent cancer occurs approximately six years later in life. Furthermore, in this syndrome there are obvious external manifestations which should lead one to suspect the presence or possibility of colonic polyposis. A new manifestation with Gardner's syndrome is the finding of pigmentation confined to the skin of the torso but not to buccal mucosa, lips, face or digits. Is it possible that in these cases there were multiple mutations of separate but closely‐linked dominant genes, thus, perhaps, adding another variation to Gardner's syndrome?
