Summary and ConclusionsThe markedly improved results of the surgical treatment of nonspecific ulcerative colitis over the past 20 years have been owing largely to the elimination of ileostomy complications and a better understanding of the physiologic changes which occur after operation. Complete rehabilitation of patients after appropriate surgical procedures may be expected in the great majority of these cases.Indications for surgical therapy are usually clearly defined and have been discussed.Routine use of proctocolectomy in one stage for all cases of ulcerative colitis is to be deprecated, mainly, because this removes forever, the possibility of re‐establishment of intestinal continuity.The problem of reconnection, i.e., ileorectal anastomosis, demands serious consideration. It is extremely doubtful whether this operation should be carried out in the presence of active disease in the rectum or any rectal complications, such as fistula, anal incontinence, polyposis and stricture. There is urgent need for individualization and careful appraisal of each patient.

Summary and ConclusionsTranssacral resection of the rectum carries a low mortality rate and much less morbidity than resections performed through the abdominal approach. It can be advantageously combined with the classical abdominal procedure in the treatment of cancer or when added length of colon is necessary to establish continuity with the anal canal in benign lesion treatment.It should be combined with a defunctionizing colostomy when most inflammatory strictures are removed. In the treatment of benign neoplasms or small, low‐grade types of malignancy, a colostomy is seldom necessary.In our hands it has been possible in the treatment of lesions of the middle and lower portions of the rectum to re‐establish continuity and preserve function of the bowel more effectively by transsacral resection and end‐to‐end anastomosis than by any other method of which we know.

Summary and ConclusionsThe perisphincteric wiring operation is an efficient, safe and relatively simple procedure which has yielded results in a much wider field of application than was anticipated when we first began its use ten years ago. It has served as the sole operation in extensive prolapse, especially in the aged and infirm, and we consider it to be a valuable adjunct in the combined abdominoperineal approach to complete rectal prolapse in younger, more active, patients.The technic, as herein described, represents an improvement in both method and materials in the performance of this operation and has thereby given it greater scope. It is our opinion that our results to date justify wider application of this operation.

Summary1. Thirty‐seven patients, undergoing colon surgery, were studied clinically and bacteriologically, to determine the efficacy of a 72‐hour preoperative preparation with nystatin‐neomycin and mechanical cleansing of the colon.2. Thirteen additional patients were studied clinically, to determine the efficacy of these methods.3. In this series of 50 patients there were no deaths, wound infections, anastomotic leaks or instances of staphylococcal enterocolitis.4. The bacterial and yeast flora were satisfactorily controlled.5. Mechanical cleansing of the large bowel was satisfactory, in 46 of 50 patients.6. No adverse side effects were noted after administration of the nystatin‐neomycin combination.
