ConclusionsEndoscopic removal of 343 colonic polyps more than 0.5 cm in size, located from the cecum to the sigmoid colon and beyond the reach of rigid sigmoidoscopes, is reported.Diagnostic colonofiberscopy is a valuable procedure permitting direct visual examination of the entire colon with a high degree of safety. More than 1,800diagnosticcolonofiberscopies have been performed in our unit without a single complication.The polypectomy procedures via the colonofiberscope were done without mortality and with only a single immediate complication, which responded to conservative measures. Endoscopic removal of colonic polyps is proposed as a safe, practical alternative to either laparotomy and colotomy or repeated barium‐enema studies in the management of the patient with a colonic polyp. The endoscopic approach allows differentiation of malignant from nonmalignant polyps by complete excision, thus permitting definitive cancer therapy measures to be undertaken promptly and with assurance.Endoscopic removal of colonic polyps is not advised for endoscopists who have not first achieved considerable experience and dexterity in diagnostic colonoscopy. Endoscopic removal of colonic polyps is best accomplished by an experienced endoscopist supported by the full spectrum of general hospital resources, after the patient has been thoroughly evaluated medically. It should be carried out on an inpatient basis.When a patient has a colonic polyp, an alert should be maintained for unsuspected colonic cancer unrelated to the polyp, lest the less serious lesion monopolize the clinical situation.

SummaryIn a retrospective study, 352 cases of carcinoma of the cecum registered with the Central Cancer Registry of the State of West Virginia during the years 1960‐1970 were analyzed. It was found that cancer of the cecum is more prevalent in women and occurs more frequently after the fourth decade of life, with a tendency for the incidence to remain high until the age of 75 years or more in both men and women.The cases of all patients with cancer of the cecum admitted over a 12‐year period (1959‐1971) to West Virginia University Hospital and the affiliated Veterans Administration Hospital, Clarksburg, West Virginia, were also analyzed. For localized lesions treated with right hemicolectomy, the five‐year survival rate without evidence of disease was 91 per cent. When there was involvement of regional lymph nodes, 50 per cent died of the disease. With disseminated disease, no patient survived more than two years.

SummaryA method of transcecal long intestinal tube decompression of the small intestine is described. The procedure is simple and very useful for intestinal obstruction which does not respond to proximal decompression. It is also suitable in the management of recurrent intestinal obstruction, and may be helpful as an adjunctive procedure with a Noble plication.

SummaryThe 24 deaths among 137 cases in which abdominoperineal resection was done at Touro Infirmary and Charity Hospital in New Orleans from 1964 through 1969 are reviewed. Sixty‐two per cent of these deaths were those of patients more than 70 years old, and 16 per cent were those of patients more than 80 years old. As evaluated by cardiovascular, pulmonary, and renal status, 50 per cent of the patients who died are classified as having been poor risks preoperatively. Eight of the 24 (33.3 per cent) are evaluated as definitely not candidates for abdominoperineal resection. These patients should have been treated by more conservative measures, such as coagulation of the tumor or radiation therapy. Careful preoperative evaluation of operative risk and determination of the curative or palliative potential treatment are essential. No longer is abdominoperineal resection necessarily the best procedure for palliation in incurable cases of adenocarcinoma of the rectum.

SummaryThe clinical and roentgenologic findings typical of annular carcinoma of the colon are well known, and the diagnosis of this lesion is usually straightforward. However, when there is perforation of the neoplasm through the intestinal wall with the development of secondary inflammatory changes, both the clinical and roentgenologic features may be altered, and the correct diagnosis becomes more difficult to determine preoperatively. Some clinical and roentgenologic aspects of this type of chronic perforating carcinoma of the colon are discussed, using five selected cases as examples.
