SummaryFour hundred patients were studied with 485 examinations by colonofiberscopy. It was found that any type of colonofiberscope would pass the upper part of the sigmoid colon in almost every patient except those with advanced carcinomas. In patients studied for the purpose of examining the cecum, a success rate of more than 40 per cent was obtained. This procedure is very useful for preoperative diagnosis, cauterysnare resection, and careful long‐term follow‐up studies. It is also advisable for the determination of medical therapy for inflammatory changes, for the determination of surgical procedures for neoplasms, and to help decrease the number of unnecessary laparotomies. Specifications for these colonofiberscopes, examination technics, and some points of improvement are presented. A cooperative understanding from the radiology department which provides fluoroscopic assistance is necessary to ensure safety during the procedure: fluoroscopy permits observation of the position of the fiberscope, and facilitates its insertion, withdrawal, and rotation.

Summary and ConclusionsA simple rapid method for the correction of rectal procidentia by the use of polypropylene mesh (Marlex) is described. The excellent results of this procedure make it preferable to any other. The material used gives the impression that an external sphincter is present immediately postoperatively. It is hoped that this technic will permit treatment of the elderly patient without the need for extensive surgical procedures.

SummaryPseudomyxoma peritonei is a rare condition that develops following rupture of a mucocele of the appendix or the ovary. Twelve cases have been reviewed. Early diagnosis and aggressive surgery offer the best chance for survival of the patient. Recurrent intestinal obstruction is a part of the natural course of the disease, and should be treated surgically by lysis of adhesions and removal of peritoneal implants when they arise. Radiation and systemic chemotherapy do not affect the natural course of the disease or the prognosis.

SummaryMy experience with fulguration of rectal cancers began in 1946. Twenty‐seven patients are reported. These represent only 3.8 per cent of the total number of patients treated.All patients treated were considered to have invasive cancer. Diagnosis was made primarily on the basis of the gross characteristics, such as firmness and fixation to the muscle. According to the pathologist, the diagnosis of “adenocarcinoma” from the biopsy specimen implies invasive carcinoma. None of these cancers was adenocarcinomain situor adenocarcinoma in an adenoma. Tumors of the rectum are not graded in our laboratory. In 11 of the 27 patients the cancers were ulcerating. In 14 “infiltrating” or “invasive” appeared on the laboratory report. Seventeen patients (63.7 per cent) were alive at the end of five years. At the present time, fulguration should be considered as experimental treatment for cancer of the rectum, and should be performed only by those who have had extensive experience.

SummaryThe records of 110 patients with recurrent and metastatic cancer of the colon and rectum are reviewed. Most patients had evidence of recurrence within two years of initial treatment. The abdomen was the most common site of recurrence. The “pelvic syndrome,” an expression of local recurrence in the lower abdomen, was the most frequent clinical indication for treatment.The effect of radiation therapy was assessed by assigning definite criteria of success or failure. Treatment prescriptions were expressed in Nominal Single Dose Equivalent (NSDE) units, relating the total dose given to the number of fractions in which it was given and the time of the whole treatment course. There was a significant correlation between the effective dose, expressed in NSDE Rets, and improvement noted. It is concluded that a moderately aggressive course of radiation therapy often ameliorates symptoms and improves the quality of survival of these patients.
