&NA;A report of an innovative approach to the treatment of locally recurrent rectal cancer following abdominoperineal resection is presented. Curative doses of external beam radiation therapy could not be delivered to an area of locally recurrent cancer because of contiguous small‐bowel loops. The patient received 5040 rads external beam radiation therapy, and then underwent intraoperative electron beam boost to the main tumor‐bearing area. An additional 1500 rads was delivered with 23 MeV electrons. This approach can be safely applied to selected patients with residual, primarily unresectable, or locally recurrent adenocarcinoma of the rectum or rectosigmoid.

&NA;A technical variant of the end‐to‐end colorectal anastomosis performed with the EEA stapler is described. It realizes a side‐to‐end anastomosis, performed via the oral‐aboral route. In this manner, indications for stapled anastomoses can be extended to patients with discrepancy in bowel size, and where operative findings involve low anterior resection, with the patient not in the lithotomy—Trendelenburg position. The technique has been used with safety in 17 patients with colorectal and ileorectal anastomoses.

&NA;A safe and practical procedure for total colectomy and mucosal proctectomy with ileonal anastomosis has been developed and performed by us on 11 patients with adenomatosis coli and two patients with ulcerative colitis. The major features of the operative procedure are 1) total removal of the rectal mucosa to just above the dentate line; 2) preservation of anorectal function by a long rectal cuff procedure achieved by rectal mucosal excision from a level just below the sacral promontory, using a rectal internal stent and gauze packing techniques for rectal mucosal stripping, with, in some patients, an ileal reservoir added; and 3) prevention of pelvic sepsis by intraoperative rectal irrigation, rectal cuff drainage, and a temporary defunctioning loop ileostomy. Of six patients with at least three months of follow‐up after reconstruction, each has returned to normal life, averaging two to seven semiformed stools each day. A side‐to‐end ileoanal anastomosis with a lowlying, loop‐type ileal reservoir provided the best functional results.

&NA;The use of a Dacron®‐reinforced Silastic® graft in the perineal repair of rectal procidentia offers a simple technique for elderly and debilitated patients. Experience with this procedure in nine patients, followed for two years, indicates that this elastic material appears to have substantial advantages over wire or synthetic mesh in the perineal repair of rectal procidentia.

&NA;In an attempt to substantiate the efficacy of Urecholine® and Dibenzyline® in prevention of urinary retention, 263 patients who had rectal surgery for benign anorectal abnormalities were divided into three comparable groups; one group was given Urecholine, another group was given Dibenzyline in 10‐mg doses orally, four and eight hours after surgery and the third group was given nothing as a control. The control group had a higher incidence of urinary retention 28.2 per cent; but Dibenzyline did not prevent urinary retention in 20 per cent of the patients and was no more effective than Urecholine at 22.4 per cent.

&NA;Upper gastrointestinal manifestations of collagen vascular diseases have been well described. Recently our attention has been focused on the colonic complications: fibrosis and stricture resulting in obstruction, severe obstipation and recurrent fecal impactions secondary to pseudo‐obstruction, progressive colonic dilatation resulting in gangrene of the colon, and sigmoid volvulus and diverticulitis in the presence of both wide‐ and narrow‐mouth pseudodiverticula. Patients with these colonic manifestations of collagen vascular disease are disabled, if not severely ill. Recognition of the problem enables the surgeon to plan definitive surgical intervention ranging from segmental resection to total colectomy and ileoproctostomy to restore satisfactory large bowel function.

&NA;Records of 266 patients who had undergone rubber ring ligation for hemorrhoidal complaints from 1969 through 1976 were reviewed to evaluate the long‐term results. The minimum follow‐up period was 36 months, with a mean of 60 months. Of the patients, 80 per cent were improved, with 69 per cent totally free of all symptoms. A subsequent hemorrhoidectomy was required in 7.5 per cent. Results were similar when either bleeding or prolapse was the primary indication for treatment. Patients who had a single band applied fared as well as those with two or more ligations.
