SummaryA study was undertaken to determine the accuracy of the barium‐enema examination in detecting carcinoma of the colon. It was divided into two parts, one to determine the accuracy of barium‐enema examinations in proved cases of carcinoma of the colon and the second to determine the likelihood of false‐positive reports of carcinoma based on such examinations.Of 707 cases in which lesions histologically proved to be carcinoma of the colon were located above the region of proctoscopic visualization, the lesion was either missed or misdiagnosed in 49 cases — an over‐all diagnostic error of 6.9 per cent. If one excludes those carcinomas that were found but misinterpreted, those associated with chronic ulcerative colitis, and those recurring at anastomoses, the diagnostic accuracy in this series was 95.9 per cent.Of 577 cases in which a roentgenologic diagnosis of carcinoma of the colon was made, false‐positive diagnoses were made in five cases—an error of 0.87 per cent.The difficulties in detecting carcinoma of the colon by means of the barium‐enema examination are discussed. The overwhelming majority of the lesions missed were located either in the cecum or the sigmoid colon.

SummaryA report is made of an inguinal hernia which mimicked a carcinoma of the sigmoid flexure on x‐ray study after administration of a barium enema. A simple technic of examination is suggested whereby error of interpretation might be avoided in the routine preoperative sigmoidoscopic and roentgenologic examination of the colon in patients undergoing elective inguinal herniorrhaphy.

Summary and ConclusionsA case of carcinoma of the colon complicating chronic ulcerative colitis of long duration in a 31‐year‐old patient has been presented. It has been stated once more that there is a much higher incidence of cancer in patients with this disease than in the normal population. The incidence becomes extremely great as the duration of the disease increases. The virulence of malignant disease in chronic ulcerative colitis has been stressed and the poor prognosis emphasized. It is the authors' considered opinion that the patient with ulcerative colitis should have periodic proctoscopic and roentgenologic examinations and that surgery should be performed whenever localized changes are revealed on roentgenologic examination after the patient has harbored the disease for 5 years or more.

ConclusionsLocal treatment of carcinomain situof the rectum is the procedure of choice. It is also preferable when there is a focal malignant change in a polyp with a pedicle that is not invaded by the cancer, and when a sessile carcinoma of low grade is less than 2 cm. in diameter.These indications may be extended in some cases when the general condition of the patient precludes a radical surgical procedure, when the patient, because of a poor mental condition, would not be able to manage the colostomy himself, and when a colostomy is refused.When a carcinoma of the rectum is inoperable, or when there is distant metastasis, local treatment may provide gratifying palliation.

SummaryMicro‐angiography has been carried out on two resected specimens of chronic ulcerative colitis. The vasculature of the diseased colon is much altered from that of the normal colon; the predominant difference being manifested by an increase in the number of vessels. The diameter of vessels of the diseased mucosa and muscularis was uniformly greater than that of the normal colon. In areas where the mucosa remained intact, the vasculature retained a normal pattern.The results of this study do not add support to the hypothesis that vascular lesions are a pathogenetic feature of chronic ulcerative colitis.
