&NA;The histologic grades of multiple neoplastic lesions in the large intestines of 41 patients were studied. Of the 41 patients, 16 had multiple polyps, 12 each had a carcinoma as well as one or more polyps, and the remaining 13 each had two or more carcinomas. Differences in the grades of histologic atypia among multiple neoplastic lesions were minimal. Therefore, the multiple neoplastic lesions present were considered to have airsen simultaneously or successively within a relatively short period of a given patient's life, rather than randomly in different periods. Of 25 patients who had carcinomain situor infiltrating carcinoma, 17 had accompanying precancerous conditions or carcinoma elsewhere in the large intestine, and the remaining eight had accompanying nonmalignant polyps.

&NA;The plausibility of preoperative chemotherapy and radiotherapy in the treatment of patients with carcinoma of the rectum is demonstrated. The finding of specimens with no tumor along with a higher proportion of Dukes' A lesions than expected was most encouraging. Whether this is a false impression because of the small number of patients, or whether it demonstrates upgrading of pathologic staging by preoperative adjuvant therapy, remains to be determined.

&NA;The exact incidence of colonic anastomotic breakdown is difficult to assess. The low incidence reported on the basis of clinical observation only is misleading, since it has been demonstrated that leakages occur without clinical manifestation. Better evaluation of the rate of incidence will require meticulous postoperative investigation, as shown by Goligher. A new colonic anastomosis was performed in 24 cases of high and low anterior resections. The principle of forming a seromuscular cuff has been employed. The encouraging results merit further trial.

&NA;Two cases of spontaneous rupture of colicartery aneurysms are presented, one involving the left colic artery and the other the right colic artery, and the literature on abdominal apoplexy and visceral arterial aneurysms is reviewed.In view of the potential for life‐threatening hemorrhage, it is urged that, whenever feasible, visceral arterial aneurysms be resected prophylactically when discovered by angiography or at celiotomy.

&NA;A 9‐mm granular‐cell myoblastoma of the cecum found incidentally during appendectomy in a 17‐year‐old girl is reported. Electronmicroscopic findings favor origin of the granular cells from an undifferentiated mesenchymal (fibroblast‐like) cell. Review of the small number of previously reported cases revealed three involving the cecum, one each in the ascending and transverse colon and two in the rectum. Four patients were asymptomatic and their lesions were found incidentally. Three lesions simulated malignancy clinically; these patients underwent right hemicolectomy.

&NA;A 16‐year‐old Negro girl underwent exploratory laparotomy for ileocecal intussusception and was found to have moderately well‐differentiated mucin‐producing adenocarcinoma of the ileocecal valve. Specific aspects of this disease in children are discussed and an appeal for early diagnostic studies in cases of children who complain of weight loss, chronic constipation, and abdominal pain is made. Finally, on the basis of the natural history of this disease, a “second‐look” operation is recommended.

&NA;A redundant limb of ileum left after a hemicolectomy is postulated to be the causative factor in a case of volvulus of the transverse colon and distal ileum after hemicolectomy 20 years before. A case is presented and the etiology of volvulus briefly discussed.

&NA;A patient with a cancer of the colon or rectum is at increased risk for developing subsequent cancer of his remaining large bowel, particularly when associated polyps and papillomas are present, and when the initial resection is for two or more growths. Patients who develop signs and symptoms of large‐bowel tumors following colonic resections for carcinoma should be completely evaluated for another primary tumor. If it is assumed that these patients simply have recurrences of their initial cancers and therefore they are not treated, many patients would be denied a potentially curative operation. All investigators agree that this group warrants long‐term follow up, ideally with regular and double‐contrast enema studies and sigmoidoscopy.Earlier diagnosis of a second colorectal cancer should improve the resectability rate and prognosis. Those patients with intact cell‐mediated immunologic responses seem to do better after surgical treatment.
