Summary and ConclusionsTwo cases of left‐sided pneumatosis coli are presented. In each case the diagnosis was made during sigmoidoscopy and confirmed by biopsy and roentgenologic examination.In one case the presence of a frothy discharge at each examination suggested that the discharge had some relationship to the disease.Multiple submucosal lumps in that portion of the bowel which can be visualized during sigmoidoscopic examination should arouse suspicion of left‐sided pneumatosis coli, which may be confirmed by biopsy and roentgenologic examination.The benign course of the disease in both of the cases and complete resolution of the condition in one supports the belief that the disease may be self‐limited.

SummaryA case of pneumatosis intestinalis, secondary to an anorectal surgical procedure is presented. This sequence of events has not been reported previously. The causative factors in this case appear to be the retention of flatus in the rectum thereby giving rise to increased intraluminal pressure in the presence of a defect in the rectal mucosa. The similarity to pneumatosis cystoides intestinalis is striking. A brief review of this rather rare condition has been provided.

SummaryA fatal allergic reaction is described which followed injection of a solution into the internal hemorrhoidal area. This report is believed to be the first of its kind. A plan is suggested for the prevention of a similar experience. Whether there is a higher incidence of this condition masquerading as coronary thrombosis is a matter worthy of consideration.

ConclusionsLeiomyosarcoma of the rectum is a rare smooth muscle tumor and is found most commonly in men between 50 and 70 years of age. It may develop from a benign leiomyoma and malignancy is evidenced in the early stages by the microscopic picture rather than by early metastasis or local extension. The main symptoms are constipation and a sense of rectal fullness.A collected series of cases, and a case report of rectal leiomyosarcoma are presented to emphasize the remarkably poor prognosis especially when the lesion is treated by local excision. Radical primary abdominoperineal resection is advocated as the proper surgical procedure if there is to be any hope of prolonged survival. Radiation therapy has been used in only a few instances and the results have been consistently poor.

Conclusions1. Of the 131 uncomplicated cases, none had signs and symptoms of sufficient severity to require surgical operation.2. In the other 125 cases, it is conceivable that complications developed which might have been avoided if the involved segment of the bowel had been resected during the so‐called optional period prior to the development of complications.3. Of the 125 cases with complications, 97 developed bleeding without previous signs or symptoms.4. Therefore, it is probable that in only 28 cases were there early signs or symptoms which might have directed attention to the existence of diverticula thus providing opportunity for “elective” surgical intervention and prevention of complications.5. Impending complications cannot always be determined or even suspected, but when severe diverticulitis is revealed by a history of abdominal pain, fever, chills, diarrhea, and nausea, and by roentgenologic and sigmoidoscopic examination, complications usually occur.6. Complications require surgical operation and, when the complication is owing to rupture of diverticula or obstruction in the left segment of the colon, we believe the three‐stage procedure is safer.7. No fatalities occurred on the first admission because of bleeding but there was a high percentage of cases of recurrent bleeding.8. Surgical operation should be performed after the first episode of bleeding and should, also, be utilized more frequently as an “optional” procedure during intervals between attacks.
