&NA;Proctitis cystica profunda is an uncommon benign rectal condition with a range of appearances that has led to confusion in nomenclature. In a personal series of 28 patients with histologically proven proctitis cystica profunda, over 50 per cent had associated rectal prolapse. Cure of the prolapse cured the proctitis cystica, profunda in 80 per cent of the patients only. Direct surgical excision (local or segmental) has not been successful and two patients have required a stoma for relief of pain.

&NA;Anastomotic leakage is particularly common in low anterior resection. This may be due to poor blood supply of the cut ends. Eighteen angiograms of the rectum, were studied and a midline paucity of vessels was found both anteriorly and posteriorly. This may provide an anatomic explanation for dehiscence.

&NA;Anorectal motility was studied in 93 children (aged 15 days to 161/2 years) who had undergone surgery for myelocele or meningomyelocele, and 80 controls, matched for age and sex. Mean resting pressure in the anal canal of patients was markedly decreased, particularly in the presence of incontinence, and unstable. The recording of a rectoanal inhibitory reflex both in the upper anal canal and at the anal margin was associated with the presence of fecal incontinence and motor disorders of the lower extremities. Patients could be divided into four groups according to manometric pattern: 1) those with normal recordings (incontinence in 63 per cent), 2) those with spontaneous intermittent relaxations of the upper anal canal and simultaneous rectal contractions (incontinence in 91 per cent), 3) those with normal manometry except for unstable upper anal canal pressure (incontinence in 92 per cent), and 4) those with absence of the rectoanal inhibitory reflex and hypotonic anal canal (patients too young to appraise continence, all under 2 years).

摘要:
&NA;Four patients who had intravenous pyelography were found to have radiologically visible calcification of the rectocolon and urinary tract due to schistosomiasis. All four patients had mild proctocolitis colonoscopically and multiple calcified schistosomal ova were found in the submucosa histologically. The authors believe that radiologically visible rectocolonic calcification is more common than reported and that it is associated with a clinically mild or latent form of schistosomiasis.

&NA;The role of ultrasound scanning in the diagnosis of cancer of the colon under emergency conditions such as intestinal obstruction, perforation or, more rarely, heavy colonic bleeding, is discussed. In any case, ultrasound scanning can quickly and rationally indicate choices for further assessment when possible. Characteristic ultrasonic images are described, and possible differential diagnoses are discussed briefly. The method seems significantly sensitive, although nonspecific, as it can give analogous images in cases of inflammatory pseudo‐tumors, Crohn's disease, and adult intussusception.

&NA;Concentrations of ampicillin after intraincisional instillation in laparotomy wounds were measured in ten patients undergoing appendectomy. Ampicillin, 1 gm, was instilled under the fascia and 1 gm in the subcutaneous space during wound closure. Wound secretion was collected every two hours during the first 24 postoperative hours by cannulation of a fine perforated drain placed in the subcutaneous space. Ampicillin was determined by a disk diffusion method. During the first eight hours the median concentration of ampicillin in wound secretion exceeded 1000 microgram/ml; 14 hours and 20 hours after wound closure the median concentrations were 73 and 14 microgram/ml, respectively. The effect of ampicillin in high concentrations on “resistant” strains ofBacteroides fragiliswas demonstrated in anin vitroexperiment. It is concluded that in colorectal surgery the effect on wound sepsis of intraincisional antibiotics as an addition to systemic antibiotic prophylaxis should be evaluated in a clinical trial.
