摘要:
Summary5‐FU is a useful, fairly predictable drug for the palliation of advanced colonic carcinoma. When used with caution, subjective improvement can be expected in more than half the patients and objective remissions in a sixth to a quarter. Toxic side‐effects can be kept to a minimum by evaluating the patient carefully before each dose of the drug.

Summary51Cr studies of feces to determine the presence of occult blood have confirmed that small amounts of51Cr are present in the stools of normal healthy individuals. A volume representing as much as 3 ml of blood may be present in the stool of a normal person, and this probably is excreted in bile.A loss of51Cr beyond this “normal” loss represents true blood loss, and this loss we have defined as “positive” occult blood.Twelve patients with proven malignant lesions of the alimentary tract were investigated, and all 12 had “positive” occult blood in the stools passed. Similar studies of 33 patients with benign lesions of the alimentary tract which caused symptoms were made but only 11 of these had “positive” fecal occult blood. It is suggested that this investigation may help in determining whether a lesion in the alimentary tract is likely to be malignant, and it may also be of some value in screening for cancer.

ConclusionsHemorrhoidectomy by amputation and anoplasty is far more radical than any conventional hemorrhoidectomy. Smoothness of the postoperative stage is one of its greatest assets; this, however, is not sufficient reason for such extensive surgery if the hemorrhoids can be treated with a less sophisticated method. When comparing my results in cases of complicated hemorrhoids, such as those associated with prolapse of the rectal mucosa, extensive deformity of the anal outlet, multiple skin tags, “doughnut” external hemorrhoids (Figs. 12 and 13), and extensive thrombosis, with those of the ligature and excision procedure, it is clear that the anatomic reconstruction obtained by amputation and anoplasty is more satisfactory. There is no reason, however, to use a method such as amputative hemorrhoidectomy, which requires a careful technic and great experience in the management of the structures of the anal canal, in the treatment of non‐complicated hemorrhoids. In my practice, of 752 hemorrhoidectomies performed, only 199 were done by amputation and anoplasty. I advocate this method for the complicated cases which merit its use, for it provides good reconstruction of the anatomy, and in my experience its functional results are comparable to those obtained with excision and ligature.When radical hemorrhoidectomy is needed to remove disease elements responsible for local symptoms, if the patient appears satisfied with immediate as well as late results, I believe that the technical problems involved with this method, no matter how difficult they may appear, are worth the effort.

SummaryIt is important to point out that this series of 144 cases of acute diverticulitis is a prospective study, with each case documented in great detail; therefore, the study provides a good basis for the evaluation of treatment. No definite conclusions can be reached at the present time, but it can be stated that 64 patients were treated by quite simple means (i.e., drainage alone or drainage combined with closure of a perforation) and only two of the 64 died. Other methods of treatment, such as colostomy or emergency resection, have yet to prove more effective than these more simple procedures in the management of acute diverticulitis. The important thing, obviously, is to save the patient's life. The question whether more extensive surgery during this emergency period would prevent future symptoms and complications will require further careful follow up of the patients in the survey.

SummaryIn a series of 307 colostomies established in 296 patients, there were complications in 53 patients (16 per cent), of whom 41 had reoperations to correct the complications. One patient died of sepsis following operation for a perforation caused by an irrigating catheter.

SummaryA patient in whom a large radiation‐induced rectovaginal fistula was successfully repaired by a pull‐through operation is described. Normal bowel and vaginal function were restored. If repair of a rectovaginal fistula is indicated, and the fistula is large and located in the proximal vagina, the abdominal endorectal operation should be considered.
