摘要:
SummaryA study was undertaken to define the microbiologic spectrum of septicemia in surgical patients with intestinal diseases, to learn more about its clinical manifestations, and to determine factors related to its pathogenesis and outcome. During a 2 1/2‐year period we detected and treated septicemia in 27 patients, of whom ten died. Results indicated that septicemia was caused by a wide variety of opportunistic bacteria or fungi and was produced by infections in a number of anatomic locations. Underlying noninfectious diseases, especially severe malnutrition and cachexia secondary to intestinal conditions and intractable renal failure, often were important determinants of a fatal outcome of infection. Early manifestations of septicemia frequently were subtle, atypical, or mistaken for expressions of other illnesses. The urinary or respiratory tracts and indwelling intravenous catheters served as apparent portals of entry for septicemia in a sizable proportion of the patients; adoption of additional preventive measures for those types of hospital‐acquired infections, hopefully, may further reduce the low incidence of septicemia in our patients in the future.

摘要:
SummaryThe cases of three patients, two with carcinoma of the cervix and one with a tumor of the left kidney treated by nephrectomy, are presented. Following irradiation to the tumor sites, two of the patients developed proctitis and rectal stricture; in one, a colostomy was established. The second is still under observation. The third patient developed obstruction of the transverse colon which necessitated partial resection and end‐to‐end anastomosis.Aspects of the pathology, clinical presentation, and surgical management of the postirradiation syndrome in the gastrointestinal tract are reviewed, and diagnostic pitfalls in its recognition are emphasized.

摘要:
ConclusionA case report of a tubular duplication of the distal ileum and entire colon with no connection to the parent organ is presented. The diagnosis was confused, making surgical management difficult. Technical aspects of the operative procedure are discussed.

摘要:
SummaryPrimary squamous‐cell carcinoma of the cecum is uncommon. Only four cases have been cited in the literature previously. The fifth case is the subject of this presentation. This tumor has no characteristics, clinical or gross, which would help to distinguish it from the usual adenocarcinoma. The diagnosis is microscopic, and can be aided by keratin and prekeratin stains. The presence of intracellular bridges and negative mucin stains are supportive evidence. These lesions may arise from embryonic nests of undifferentiated epithelium, epidermoid implantation at time of operation, inflammation, or radiation, or they may be teratomatous.

摘要:
ConclusionCarcinoma involving the cloacal zone consists of squamous, transitional and/or mucin‐secreting cells. Although the term “transitional‐cell cloacogenic carcinoma” has been used to designate this type of tumor, a conventional term, “basaloid carcinoma,” may be used to describe tumors which consist of a non‐mixed pattern of transitional cells. The prognosis of cloacogenic carcinoma is dependent upon early diagnosis and radical treatment. However, when the carcinoma involves the regional lymph nodes or there is evidence of an anaplastic pattern histologically, a poorer prognosis can be expected.

摘要:
SummaryThere are many technics for treating condyloma acuminatum. None is entirely satisfactory. The viral etiology and the probable venereal transmission make most methods difficult at best. Our method of using bichloracetic acid has given us a more satisfactory cure rate, with a minimum of discomfort, no scarring, and less prolonged convalescence. Future improvement in the management of condyloma acuminatum will depend upon the development of a suitable vaccine based on the positive identification of these growths as specific infectious lesions caused by a virus. Oral or injectable medication would attack the lesion directly, be easier to apply, be less painful, and allow vectors to be treated more easily. However, until such a vaccine is introduced, the clinician will continue to see condyloma acuminatum and will need therapeutic tools that cause few complications and minimum discomfort and have a reasonable degree of success in removing the warts.
