Summary and ConclusionsRectal carcinoids are slow‐growing, potentially malignant neoplasms. They develop in the submucosa and are found most often in the anterior wall of the middle third of the rectum. The diagnosis is usually made in older patients and the occurrence is generally at an earlier age in women than in men. There are no histologic criteria by which to recognize the malignant and potentially malignant tumors. The size of the tumor is closely related to its malignant potentiality; 78 per cent of those larger than 2.0 cm. were associated with metastasis while only 8.0 per cent of the smaller lesions were malignant. Tumor invasion of the muscularis propria was noted in 78 per cent of the malignant cases and in a much smaller percentage of the benign cases. The actual incidence of invasion cannot be determined from the available data. A significant number of cases coexisted with other gastro‐intestinal neoplasms and with peptic ulcers.Abdominoperineal resection is the treatment recommended for carcinoids larger than 2.0 cm. in diameter, while smaller lesions can be adequately treated in most cases by local excision.

SummaryReports were examined of 279 cases of hemorrhage after anorectal surgical operations, treated by 97 different surgeons.The most popular method of controlling hemorrhage was by ligature, usually employed with anesthesia. Insertion of some kind of hemorrhage pack was the next most popular method of treatment.

SummaryThirty‐eight adult patients with complete rectal prolapse had rectopexy by the method of Pemberton‐Stalker. The 34 survivors followed for an average of 4.8 years (one to 9.5 years) had five recurrences, only two of which were pronounced—an incidence of anatomic recurrence of 13 per cent. In the majority of patients, however, there had been an improvement of sphincter function so that the incidence of symptomatic recurrence was only 3.0 per cent.These results are poorer than those reported following more recent methods which have been perceptibly extended by repair of the levator muscles or resection. Still, they are good enough to justify rectopexy in the present form in all poor‐risk patients.

Summary and ConclusionsTrue Hirschsprung's disease, occurring in the adult, is not reported frequently in medical literature. The patient presented herewith illustrates the essential features of Hirschsprung's disease, namely, unrelenting constipation since birth, massive dilatation of the sigmoid flexure with a distal narrowed segment, and a rectal muscle biopsy showing no ganglion cells. Rectal muscle biopsy is conclusive in making the diagnosis of Hirschsprung's disease. By microscopic examination of the muscle of the bowel wall, the extent of the disease in the colon can be determined.

SummaryIn this clinical study documented by serial color photographs, neither erythromycin, 400 mg. administered orally every six hours, nor tetracycline, 250 mg. given orally every six hours for periods of three to seven days, significantly affected wound healing. However, nitrofurazone applied topically had a demonstrably beneficial effect.

SummaryWe believe the interest shown in the Thiersch operation is justified. It is a useful procedure, used either singly or combined with other technics. To the various modifications of the original procedure we add one which consists of the insertion of a double loop of semiflexible plastic material, the ends of which can be joined by melting and fusing. In spite of the fact that the number of patients operated upon is small and the time elapsed since operation is not long, we find that this is an operation which provides good results and is worth employing more frequently.

Summary and ConclusionsA case of cecal volvulus is reported in which the diagnosis was made early and therapy was instituted quickly. Diagnosis in most cases can be made by consideration of characteristic clinical and roentgenologic features. Early diagnosis and operation are imperative if the mortality rate is to be held at an appropriate level. Although the mortality rate is still high, present day adjuncts to surgical management have reduced it to a much more desirable level. The advantages and disadvantages of cecopexy, cecostomy and resection have been summarized. Right hemicolectomy is becoming the treatment of choice and is supplanting the older and more conservative cecopexy and cecostomy.
