&NA;The management of colorectal polyps should be highly individualized. In arriving at a therapeutic decision concerning the treatment of invasive carcinomas arising in colonic polyps, one should have a basic knowledge of the polyps' malignant potential and propensity to metastasize. It is optimal to confirm the presence of invasive malignancy by excisional biopsy examination and weigh the risk of the patient's having involved lymph nodes against the risk of undertaking radical operative treatment. The results reported here give an accurate indication of the risks of metastatic nodal involvement in different types of polyps containing invasive carcinoma.

&NA;A new technique for constructing a continent ileostomy has been tested in the dog. The terminal ileum is divided 10 cm from the ileocecal valve, and the distal stump is closed. The last 35 cm of the proximal stump of ileum are used: the proximal 30 cm portion is used to prepare a reservoir ileostomy according to Kock's method, while the distal 5 cm portion is used to form the stoma.The outlet of the reservoir is sutured to the abdominal skin and is provided with a magnetic sealing device formed by a magnetic ring coated with Palacos which is implanted in the abdominal wall; the magnetic outside cover seals the stoma.Eight dogs were provided with this type of ileostomy. The observation periods did not exceed six weeks. The dogs wore the magnetic covers for 10 to 12 hours a day. Full continence was obtained in all dogs but one, in which the ring had to be explanted because of infection.Radiologic examination performed 30 days after the operation with orally administered barium demonstrated great increases in the capacities of the reservoirs and full continence of the stomas.Intrareservoir pressures recorded during infusion of fluid in the ileostomy pouches demonstrated the absence of pressure waves, even when 600 ml of fluid were introduced.

&NA;Histologic changes observed in association with gastric, mammary, and bronchial carcinomas may be considered features of colorectal carcinomas also. We interpret our results as evidence of a local host reaction to the tumor, probably antigenic. We conclude that adjuvant immunotherapy after radical surgical treatment will probably increase the chance for survival.

&NA;The results of rectal examinations of 495 patients with suspected acute appendicitis were correlated with six other variables from the patients' histories and results of physical examinations. Positive rectal examination results were recorded for 46 per cent of all patients with acute appendicitis and for 53 per cent of those with normal appendices. The diagnosis of acute appendicitis should not be based solely or primarily on the results of the rectal examination. The examination should, however, be performed in all suspected cases of appendicitis to rule out gynecologic and urologic disease. “If you don't put your finger in, you put your foot in it.” ‐ H. &NA;We report a family manifesting the cancer‐family syndrome in which 11 family members had colonic carcinomas (predominantly involving the proximal colon, in the absence of polyposis), with an average age at onset of 35 years. Three women had endometrial or endocervical cancers. The kindred is notable in that its full evaluation was predicated upon the recognition of features consistent with the cancer‐family syndrome in only two sisters. The ascertainment and evaluation of the kindred demonstrates the clinical utility of regarding such criteria (early cancer onset, multiple primary cancers, proximal colonic involvement) as a basis for selecting cases for more thorough family‐history evaluation. Although such selection criteria are not pathognomonic for the syndrome, identification of a more extensive family cancer history sometimes enables the initiation of a highly specific cancer surveillance program. Specific attention has been given to the problems of screening patients at risk for the development of proximal colonic cancer, an important feature of the cancer‐family syndrome. Innovative operative management is also indicated, such as total colectomy for initial colonic cancer, and consideration of prophylactic hysterectomy for women with colonic cancer (because of the high risk of development of endometrial carcinoma).
