&NA;In assessing the results in this series, an attempt has been made to rationalize the methods of treatment and emphasize the importance of a sound anatomic basis for both classification and management. The technical methods are somewhat of a compromise. The classic methods of staged division of muscle, together with the use of the seton (for drainage only) are combined with exploration of the intersphincteric plane to eliminate the causative factor of the disease.Because these “high” fistulas are uncommon, particularly the suprasphincteric and extrasphincteric varieties, there is little information in the literature with which to compare the figures. It is hoped that the experience gained in treating these cases will act as a guide for surgeons who see complicated fistulas infrequently, and also act as a baseline for further reappraisal of treatment methods. The ultimate aim must be to obtain healing of the fistula, at the same time minimizing disturbance of function.

&NA;Fistulas manifesting in the perianal region but originating in the abdomen often have their true origin not recognized for extended periods. Examples of seven such complex cases are presented and discussed. Removal of the abdominal abnormality is mandatory before any cure can be effected; the perianal portion usually heals without any special local treatment.

&NA;A preliminary report in 19654described a conservative surgical procedure for the management of acute and chronic horseshoe anal fistulas. The operation has been used exclusively at the Ochsner Clinic for this problem since 1963. Forty‐one patients were treated from 1963 to 1973.The paper reviews the pathology of acute and chronic horseshoe anal fistulas and describes the surgical procedure for both acute and chronic horseshoe abscess anal fistulas with accompanying illustrations.The excellent results with minimal deformity of the anus and anal canal are attributed to avoidance of severing the superficial external sphincter between its coccygeal origin and the anus. Of the 41 patients treated in the period from 1963 to 1973, healing was good, and there has been no recurrence.

&NA;Carcinoma of the colorectum knows no age barrier. To date, more than 1,400 cases have been reported to occur in persons less than 40 years old. Our experience with 70 patients treated over a tenyear interval is reviewed. Signs and symptoms in the young are not distinctive. Diagnosis depends on a high index of suspicion and appropriate investigative procedures. Survival times are shorter in the patients who have mucinous and anaplastic tumors, and their incidences seem to be increased in this age segment. Overall survival does not significantly differ from our general experience (41 per cent, 5‐year survival). Early diagnosis and prompt institution of aggressive surgical treatment can be expected to produce survival equivalent to that in patients of other ages.
