&NA;This study has confirmed that patients who have advanced colorectal carcinoma have impaired responsiveness to delayed‐hypersensitivity skin testing, and also have elevated levels of serum IgM. Serial observations of delayed‐hypersensitivity skin tests, total lymphocyte counts, T‐lymphocyte counts, B‐lymphocyte counts, and serum immunoglobulin levels failed to reveal any consistent pattern of responses in patients treatment with either chemotherapy alone or chemoimmunotherapy. In 33 patients chosen at random to receive either 5‐FU alone or 5‐FU in combination with intramuscularly administeredC. parvum, there was no evidence of objective response or influence on survival. Intramuscularly administeredC. parvum, in the dose and schedule used, produced no evident immunologic or therapeutic effect.

&NA;Two methods of management of the perineal wound after rectal excision for neoplastic disease were compared in a prospective randomized clinical trial in 65 patients. Twenty‐eight patients (Group A) were treated by partial closure of the perineal wound and cigarette drainage of the pelvic space, according to the method proposed by Gabriel. Thirty‐seven patients (Group B) were treated by complete suture of the perineal wound and suction drainage of the pelvic space according to the method proposed by Altemeier.Immediate and late results were better in patients in Group B. They achieved healingper primamin 62.85 per cent of cases and had significantly shorter hospital stays than patients in Group A. Three months postoperatively, 91.42 per cent of patients in Group B and 59.25 per cent of those in Group A had complete healing of the perineal wounds, with a statistically significant difference (&khgr;2with Yates correction =7.27, P<0.01). The development in 10 patients of Group B of perineal abscesses or hematomas necessitated reopening of the perineal wounds: however, the postoperative courses were similar to those of patients in Group A.It is concluded that healing by primary intention with use of the method described herein is the treatment of choice for managing the perineal wounds in patients undergoing rectal excision for neoplastic disease.

&NA;Colonoscopy performed in the operating room at celiotomy, but prior to colotomy, is of definite value in the intraoperative assessment of colonic disease, especially with reference to the need for and extent of resection. By avoiding colotomies, contamination of the peritoneal cavity and wound can be obviated, with concomitant diminished morbidity.

&NA;A case of an undoubted squamous‐cell carcinoma arising at the hepatic flexure in the colon of a middle‐aged man is reported. Review of the literature and exclusion of cases not strictly comparable suggest that pure squamous‐cell carcinoma of the colon is rare, this being only the fifteenth such tumor reported, and only the second recorded case of such a tumor of the heaptic flexure. Possible pathogenesis is suggested.
