ConclusionThe muscles of the pelvic floor are unique by virtue of their nervous mechanism. No other muscle will fulfill their function, which is the reason for failure of the Wredon‐Stone operation for incontinence. Without constant reflex postural activity, there cannot be continence.The electrical activity occurring in the muscles of the pelvic floor, which can be recorded by electromyographic means, has been well described by other investigators. Our efforts have been directed toward investigation of the mechanism of this activity and the means by which it can be modified. Our findings suggest that the mechanism is reflex and we propose that it be known as thepostural reflex of the pelvic floor. In addition, we have described inhibition of this reflex by rectal distention. This phenomenon is also due to a reflex arc and we suggest that it be calledthe rectal inhibitory reflex. These closely interrelated reflexes have many practical and theoretical implications, especially in relation to continence and rectal prolapse.

SummaryTwo cases of chronic ulcerative colitis occurring in brothers who ultimately developed cancer of the large bowel are presented. These are the first cases of this kind that the authors have been able to discover in medical literature. The possibility of hereditary predisposition to both ulcerative colitis and cancer is discussed.

SummaryAlthough management of chronic ulcerative colitis continues to challenge the physician, our ability to treat this devastating disease is rapidly improving. Experience at the Ochsner Clinic indicates that the incidence and clinical picture of this condition are similar to those reported from medical centers located further North. Results of medical treatment are usually satisfactory, although the disease still exhibits a pronounced tendency to relapse. The most satisfactory surgical method of handling intractable chronic or fulminant ulcerative colitis or dangerous complications is total or subtotal colectomy and ileostomy.

Summary and ConclusionsA case of disseminated atypical capillary hemangiomatosis of the colon and rectum is reported which was successfully treated by total colectomy. The tiny blood clots which were seen at proctoscopy and appeared to be attached to the mucosa were not completely evaluated during the first examination; they proved to be hemangiomas which were recognized later and fulgurated. The bleeding was so excessive that, at first, it was believed to originate at some point in the colon beyond the reach of the sigmoidoscope. It was not until after coloscopy was performed that it could be assured that the entire colon was involved.
