&NA;Patterns of stratified squamous epithelium have been recognized recently in colorectal adenomas. Light microscopic and keratin immunohistochemical analysis of four cases in the present report suggested origin from large intestinal reserve cells, with impaired and disorderly maturation in the squamous foci. One case had an invasive adenocarcinoma separately in the same polyp, bringing the reported incidence of malignant transformation in these adenomas to seven of 48 (15 percent). Evidence is presented to support the notion that squamous differentiation may be an inherently neoplastic phenomenon in colorectal adenomas, which may be added to the list of markers for colorectal polyps at higher risk for malignant transformation.

&NA;A technique is described that has proven most effective in correction of postoperative anal stenosis. A modification of this technique also has proven effective in the treatment of symptomatic anal mucosal ectropion—a condition marked by pruritus and perianal soilage from mucosal weeping, usually associated with the Whitehead hemorrhoidectomy. The design of the flap allows primary closure of the donor site and tension‐free flap inset with a robust flap blood supply.

&NA;To evaluate the potential effect of androgens on human colorectal cancer, the prevalence and concentration of cytosolic androgen receptors were analyzed in 23 rectal and 13 cecal adenocarcinomas by a hybrid radioligand assay. Androgen receptors were detected in nine of the rectal (39 percent) and five of the cecal tumors (38 percent). Androgen receptor levels demonstrated were low, ranging from three to 17 fmol/mg cytosol protein. Dissociation constants were 1 nM or less. The presence of androgen receptors was independent of age or sex of the patient, and of the state of histologic differentiation and Dukes' staging of the tumor. Androgen receptor prevalence was similar in mucosa adjacent to rectal and cecal adenocarcinomas and in mucosa from five of ten patients (50 percent) with diverticular disease. Our findings suggest that androgen dependency does not play a major role in endocrine control of the development of rectal cancer.

&NA;The incidence of ischemic colitis in renal transplant recipients approaches 1 percent. The mortality in these patients with ischemic colitis is nearly 70 percent. Immunosuppressive agents have been implicated in the development of ischemic colitis. To study the effect of immunosuppressive agents on ischemic colitis, a 4‐cm segment of the colon was devascularized in 27 male Fischer rats. The animals were divided into one control and two treatment groups. One treatment group received methylprednisolone and the other, azathioprine, in doses similar to those of renal transplant recipients. Both experimental groups, either separately or combined, showed significantly greater areas of colonic ischemic changes than did the control group. This study demonstrates that systemic administration of immunosuppressive agents may augment the development of ischemic colitis.

&NA;Three hundred nineteen affected members of 94 Danish families with familial polyposis coli had been registered in the Danish Polyposis Register by the end of 1982. Of the 247 histologically verified cases, 168 were propositi and 79 were call‐up patients. The frequency of colorectal cancer at the time of diagnosis of polyposis was 69 percent in propositi vs. 3 percent in call‐up patients. The cumulative survival rate after ten years was 97 percent in call‐up patients, as compared to 42 percent in propositi. The prognosis of polyposis patients has improved significantly since the establishment of the Danish Polyposis Register in 1971.

&NA;In an attempt to know the exact retrograde spread of high‐dosage 5‐aminosalicylic acid enemas, we have studied eight patients with active left‐sided colitis, by adding a small amount of barium sulfate to the enemas and by checking the spread radiologically after 15 minutes, 1 hour, and 6 hours. Four grams of 5‐aminosalicylic acid in 100‐ml enemas and 4 gm in 200‐ml enemas were used. The same experiment was repeated in a subsequent attack, with enemas labeled with technetium‐99m and checked by scintiscans in five of these patients. We always have observed a volume‐dependent spread of enemas but, interestingly, in the patients studied with technetium‐99m there was always a wider spread than that which was detected with barium enemas. In all five patients, 100‐ml enemas reached the splenic flexure. In two patients with total colitis, a progression of 100‐ml technetium‐99m enemas was performed in the transverse colon, but the maximum opacity remained in the left side. We can conclude that 4 gm of 5‐aminosalicylic acid in 100‐ml enemas can be suitable for treating patients with left‐sided colitis, and will represent a valid addition for patients with more extensive colitis.

&NA;Immunoperoxidase staining for carcinoembryonic antigen (CEA) was performed on 192 colorectal carcinomas to determine: whether tissue staining can be substituted for preoperative serum CEA levels, and whether patient survival can be predicted by these parameters. The overall incidence of positive tissue staining was 75 percent, which was similar to the elevated serum level percentage of 73 percent. Both the serum CEA level and the CEA tissue stain correlated with patient survival in Dukes' stage C patients. There was no correlation between tissue CEA stain and tumor differentiation. Positive tissue stain and elevated preoperative serum CEA identified patients with poor prognosis in Dukes' stage D only. This study shows that tissue staining with immunoperoxidase may be substituted for preoperative serum levels for CEA. The combination of these two parameters, however, does not identify patients at greater risk for recurrence than either procedure alone.
