Summary and ConclusionsClinical and experimental evidence indicates that barium sulfate causes minimal foreign‐body reaction and generally is well tolerated when injected into the lungs, peritoneal cavity, and subcutaneous and retroperitoneal tissues. That this is also true when it is injected accidentally into the wall of the rectum during administration of barium enemas is revealed by a review of the 15 cases reported in medical literature and our two cases.Depending on the degree of bacterial contamination, host resistance, and the site and mechanics of injection, barium introduced into the wall of the rectum may present as perianal or retrorectal abscesses which require drainage, or as ulcerated or nonulcerated tumors of the rectal wall. In the latter instances, only biopsy is required for diagnosis, and this is well tolerated. Attempts to eliminate the barium from the tissues by surgical means are futile and unnecessary.Barium granuloma of the rectum can be avoided by care and diligence while introducing tubes into the rectum prior to administration of barium enemas, by avoiding excessive pressure in the rectum when a balloon is used, and by avoiding administration of barium enemas soon after biopsy of the rectal mucosa.

SummaryA case of massive rectal bleeding from a nonspecific lesion of the rectum has been presented. The implications of this case have been discussed and the causes of massive rectal bleeding outlined.It is recommended that an aggressive approach to the management of this problem be adopted and that surgical intervention be considered early if clinical signs of reduced blood volume are present or if blood loss of more than two units has occurred.

ConclusionThe syndrome of bilateral lacerations of the broad ligament resulting from child‐birth was first described in 1955. Its clinical significance has been emphasized as an etiologic factor in such pelvic symptomatology as dyspareunia, dysmenorrhea, menorrhagia and pelvic pain. A case of a strangulated bowel secondary to a herniation through a tear in the broad ligament has been presented. To the author's knowledge this is the first reported case of a bowel obstruction complicating the Allen‐Masters syndrome.

SummaryDermatomyositis is an uncommon disease which produces, in addition to its well‐known skin and musculoskeletal symptoms, variable gastrointestinal complaints. When it first appears in adult life a diligent search should be made for an associated neoplasm, since they occur in more than 50 per cent of such patients over 40 years of age.

SummaryThe bowel must be well prepared. The anastomosis should not be jeopardized by use of too many sutures. The perineal floor is not closed; the anastomosis is left intraperitoneally where it can be protected by omentum or small bowel in the event of a separation. A tube cecostomy is a valuable adjunct in a difficult resection. Oral feedings should be allowed only after satisfactory restoration of bowel function. Bandages are removed permanently on the first postoperative day, and showers and baths are allowed as soon as the patient is sufficiently ambulatory. Colon surgery can be done effectively without the use of drains, prophylactic postoperative antibiotics, or prophylactic postoperative gastric suction.

SummaryTo avoid complication after the Whitehead radical circumferential hemorrhoidectomy, preservation of the anoderm assimpleflaps has been recommended by others. This paper describes a new method ofslidingskin flap grafts for cases of insufficient anoderm. It provides for the use of this operation with greater frequency and safety. All internal and external hemorrhoids, crypts, and redundant rectal mucosa can be removed. There is remarkably little postoperative pain and there are few complications. Passage of large‐caliber stools postoperatively, without pain, is explained by transient denervation of the flaps and protection of the sensitive sphincter muscle.Complete relining of the anal canal with flaps, and providing a large lumen, prevents overstretching by a large constipated stool and decreases the incidence of hemorrhage, stricture, fissuring, painful scars and recurrence.

SummaryTwo cases of leiomyosarcoma of the rectum are reported. One patient died of local extension and distant metastases. In the other patient the disease was palliated by cryosurgery to relieve obstruction and, eight months later, the patient was doing well.Medical literature has been reviewed. Leiomyosarcoma of the rectum is rare and extremely malignant. Ninety‐seven cases are reported in medical literature; the five‐year survival rate was 20 to 25 per cent. Invariably, local excision is followed by local recurrence. The treatment of choice is abdominoperineal resection but, even with this method, the survival rate is poor.
