&NA;Tumor immunity in 31 colonic tumor patients was studied by determiningin vitroleukocyte‐migration inhibition. In tissue culture 30 per cent inhibition of patient leukocyte migration by tumor antigen could be demonstrated in half of the patients, suggesting that they were immune. This immunity was not related to the clinical or pathologic stage of the disease and varied in different patients. It was transient in some and persistent in others. In addition, a positive response at a fixed time after operation was not found.

&NA;Advanced age, per se, is not a contraindication to colectomy and ileostomy for inflammatory intestinal disease and for multiple colonic tumors. Such operations are well tolerated if performed on an elective basis.Elderly patients accept and manage the ileostomy as well as younger patients. As in younger patients, both overall health and the quality of life improve after operation.Such medical problems as are found in elderly patients who have had ileostomy are the same ones found in patients of comparable age who have not undergone ileostomy.Given the proper indications, the surgeon should not hesitate to perform ileostomy in an elderly patient simply because of anticipated difficulties in stoma management. The elderly can manage an ileostomy as well as younger patients, and, although a slightly higher incidence of ileostomy‐related complications occurs in the aged, these complications can generally be managed without serious morbidity.

&NA;Urologic complications arose in 23.7 per cent of 569 patients who underwent abdominoperineal or low anterior resection for anorectal cancer. No randomization of the two operations was attempted, low anterior resection being performed whenever resection 5 cm below the tumor was possible. Complications were more frequent after abdominoperineal resection and in men.Preoperative intravenous pyelography was performed in the cases of 541 of the patients, including 60 who underwent palliative colostomy. The pyelograms of 30 per cent of these patients were abnormal. The abnormalities were anatomic variations of the urinary tract in 25 per cent and urologic diseases in 75 per cent. None of the postoperative urologic complications was related to an abnormal preoperative pyelogram. No relation was found between the radicality of abdominoperineal and low anterior resection and the pyelographic signs of tumor involvement.The present study gives little support to the use of preoperative pyelography as a routine. A randomized trial to elucidate this situation is warranted.

&NA;Seventy‐three patients with adenocarcinoma of the sigmoid, rectosigmoid and rectum were reviewed to determine the incidence of suture‐line recurrence. Four patients (5.5 per cent) developed sutureline recurrences despite the use of a number of techniques to reduce this problem. All four recurrences were in patients who had Dukes' C lesions of the rectum. It would appear that such recurrences are not related to implantation factors alone.

&NA;In dealing with anorectal disease of childhood, it must be remembered that the pathogenesis is similar to that of the adult: Careful examination, including endoscopy, is necessary to establish a diagnosis, and can easily be accomplished, even in the neonate. Treatment of anal fissures, hemorrhoids, fistulas and/or rectal abscesses in children parallels that of the same diseases in the adult.

&NA;Gardner's syndrome is inherited as an autosomal dominant trait. It is characterized by intestinal polyposis, bony exostoses, and multiple soft‐tissue abnormalities. Life‐threatening components of the trait are the malignant potential of the colonic polyposis and the malignant behavior of the histologically benign intraabdominal fibromatosis. Colectomy or proctocolectomy is indicated for colonic polyposis; a case illustrating the feasibility of wide resection for intra‐abdominal fibromas is presented.

&NA;We have endeavored to show the importance of iron deficiency and to cite a few examples of the systemic effects of such deficiency without anemia. We have shown the need for large‐scale screening of patients for iron deficiency and suggest that serum iron and total iron‐binding capacity determinations will adequately furnish the vehicle for this purpose. The modes of iron therapy are discussed with the belief that for the surgeon parenteral iron is the best vehicle to replace the deficiency with the least risk.
