Interventional radiologic techniques have been applied to the management of congenital cardiovascular abnormalities. Transcatheter embolization is now the procedure of choice in the treatment of pulmonary arteriovenous malformations. Transluminal angioplasty has been successfully employed in the management of patients with congenital pulmonary and aortic valvular stenosis and may also be the treatment of choice for postoperative restenosis of coarctation of the aorta.

Embolization of the systemic arteries of the lung (described by Remy and colleagues1in 1973) is now a useful method for the treatment of hemoptysis or hemorrhagic lesions of the lung prior to surgical treatment, or for local treatment of hemoptysis when surgery is contraindicated or unnecessary. The technique is based on the anatomy of the different divisions of the systemic circulation (bronchial and extrabronchial), which for various physiologic reasons may develop hypervascularization. The results, complications, and contraindications of systemic embolization have previously been described and the technique is now commonly practiced.2–4

Transthoracic needle aspiration biopsy (TNAB) is the procedure of choice for the diagnosis of focal pulmonary processes. TNAB can identify the entire spectrum of focal benign or malignant disease. Technique-dependent factors and the underlying pathologic processes that affect the diagnostic yield of TNAB are reviewed.

A stereotaxic guidance system has been used in conjunction with a conventional CT unit to biopsy 26 consecutive thoracic lesions. Accurate needle placement with a single manipulation occurred in 24 instances and two lesions required second passes. The technique provided adequate pathologic material with an acceptable complication rate. System advantages include the ability to approach lesions along oblique paths, controlled approach in difficult locations (mediastinal, peripheral, subscapular), capacity to biopsy small nonfluoroscopically visualized lesions, and increased speed over manual techniques.

To determine the value of transbronchial needle aspiration biopsy in the diagnosis of sarcoidosis, we reviewed a one-year experience of consecutive patients with sarcoidosis presenting with hilar and/or paratracheal adenopathy. The sensitivity of transbronchial needle aspiration biopsy in obtaining noncaseating granulomas was 90%. This yield exceeds that of most published reports of transbronchial lung biopsy and bronchial mucosal biopsy and suggests that transbronchial needle aspiration biopsy may be a valuable diagnostic tool in the evaluation of these forms of sarcoidosis.

TNB of hilar and mediastinal masses is both safe and useful. It has a high diagnostic yield, can be performed in practically all areas of the mediastinum, and appears to be no more hazardous than needle biopsy of the lung. In healthy patients, it can be performed as an outpatient procedure. TNB makes it possible to avoid surgery and mediastinoscopy in patients with unresectable malignant neoplasms and in many patients with innocuous benign mediastinal lesions. The ease and safety of TNB may make it the preferred initial procedure for diagnosing and staging patients with mediastinal masses of unknown etiology.

Fine needle aspiration of the lung is gaining wide acceptance as a fast, accurate, safe, and inexpensive method of diagnosing lung lesions. It provides information that eliminates the need for or helps guide surgery. If used optimally, pulmonary fine needle aspiration has a diagnostic accuracy approaching open biopsy with minimal and minor morbidity. Since fine needle aspiration has closely linked radiology and pathology, communication and understanding between these areas is of increasing importance.

The use of intravascular catheters for diagnostic and therapeutic purposes has resulted in an increased number of foreign body embolizations. The retained foreign body is a potential source of morbidity and mortality. In this communication we describe the different techniques that are used to remove foreign bodies from various locations within the heart or vascular tree. A brief review of the literature concerning the reported complications from retained foreign bodies is made to underline the importance of early percutaneous removal of these fragments.

The fiberoptic bronchoscope became commercially available nearly 20 years ago. This instrument has revolutionized the practice of respiratory medicine by providing the biggest single advance in diagnostic techniques for the chest in recent years. Its use is now widespread, and it has largely superseded the rigid bronchoscope. However, it is still preferable to use the rigid instrument for the investigation of suspected tracheal tumors, removal of foreign bodies and inspissated mucous plugs, for the management as opposed to the investigation of hemoptysis and, arguably, for the biopsy of vascular tumors such as adenomas. Radiologists should be aware of the uses and limitations of fiberoptic bronchoscopy, the relevance to management and diagnosis of abnormalities visible on the chest radiograph and computed tomography, and the complementary role it plays with other biopsy techniques that are used for the diagnosis of focal and diffuse lung disease.

The computed tomography (CT) findings of five benign, mature teratomas of the anterior mediastinum collected during a ten-year period are described, and their case histories are reviewed. Three of the five were primarily cystic, with only small soft tissue components. One of the five contained equal cystic and solid elements. All cysts had a higher attenuation than water. Three tumors contained small foci of dense calcification or ossification. Only two contained fat. Computed tomography accurately predicted the presence of adherence to adjacent structures in all five. Four of five patients were less than 22 years of age, and benign teratoma of the mediastinum was not a common neoplasm at any age. The origin and pathology of germ cell tumors of the mediastinum are briefly reviewed.
