A patient with chronic meningococcemia with the unusual features of subungual splinter hemorrhages, a pericardial friction rub, and a culture positive skin lesion is presented. Group specific and homologous antibody responses were absent and the nitroblue tetrazolium test for infection was negative. The clinical features and the difficulty in explaining the mechanism of the relatively benign course that characterizes this disease are discussed.

A patient with meningococcal pneumonia which developed following therapy for an earlier episode of pneumococcal pneumonia is presented. This entity is most frequently associated with an antecedent viral respiratory illness and/or a concurrent infection with Hemophilus influenzae or Diplococcus pneumoniae. The characteristics of this disease are discussed and the possible pathogenetic mechanisms reviewed.

A severe case of trichinosis occurred following the ingestion of raw meat from a common black bear,Ursus americanus, that was shot in Butte County, California. Examination of steaks cut from the bear revealed heavy infection withT. spiralis.A good clinical response was obtained following treatment with prednisone (1 mg/kg body weight per day) and thiabendazole (50 mg/kg body weight per day); no toxic effects were observed. In comparison with the pre-treatment biopsy, a more intense inflammatory response was seen following the administration of thiabendazole. Nevertheless, a muscle biopsy taken several months after discharge still showed well-encapsulated, morphologically intact larvae. In addition to the expected immunological responses toT. spiralis, a marked rise in titer of antibodies toToxoplasma gondiiwas observed by the fluorescent antibody and Sabin-Feldman dye test methods. Since toxoplasma infection of muscle is widespread in man, it is possible that an unrelated disease of muscle could result in stimulation of anti-toxoplasma antibodies similar to the findings in polymyositis and dermatomyositis. Alternatively, the possibility that a dual infection was acquired from the bear meat cannot be excluded.

A patient with diffuse eosinophilic gastroenteritis, associated with progressive chronic interstitial pulmonary fibrosis, was followed for 29 years. His major symptoms were recurrent gastrointestinal bleeding, anemia, diarrhea, malabsorption, and increasing respiratory insufficiency. Multiple iejunal biopsies revealed eosinophilic infiltration and extensive villous atrophy. Results of a lung biopsy and pulmonary function tests were consistent with chronic interstitial pulmonary fibrosis. His gastrointestinal symptoms responded to steroid therapy, but he died of progressive respiratory insufficiency. At autopsy, villous atrophy was noted to extend throughout the whole extent of the small intestine in association with multiple “pseudopolypoidal” inflammatory lesions.

A patient with histologically proven coexistent Paget's disease of the bone and parathyroid adenomatosis is described. She developed coma associated with hypercalcemia and underwent successfully surgical removal of two parathyroid adenomata. The differential diagnosis of hypercalcemia in patients with this rare association is discussed and the importance of early surgical treatment is stressed. A review of similar reported cases is presented.

A patient with post-necrotic cirrhosis is described in whom spur cell hemolytic anemia developed eight years after splenectomy in association with worsening liver function. The presence of a spleen or splenic function is therefore not essential either for the formation of spur cells or for hemolysis of such cells. Splenectomy therefore should be regarded with circumspection in the management of patients with spur cell hemolytic anemia.
