One hundred and fifty-one patients who presented with massive gastrointestinal bleeding were subjected to emergency endoscopic examination. Pre-endoscopic medication included diazepam (Valium) or morphine and phenobarbital. Liver function studies were compared before and after the medication was given. Those patients who received Valium were equally well or better relaxed than those who received narcotics and barbiturates. There was more amnesia after Valium with a shorter duration of action. Data showed that Valium had relatively less effect on liver function than that noted with narcotic and barbiturates. It was concluded that injectable Valium is a safe and effective adjunctive medication for use before endoscopic examination of patients presenting with gastrointestinal hemorrhage.

Nine potential risk factors were evaluated in 84 kidney transplant patients to help determine when the risks of continued therapy of a transplant outweight the chances of ultimate graft success. Seventy-five per cent of the patients with two or more rejections in the first two months post-transplant died or lost their transplanted kidney, compared to only 33 per cent of those with zero rejections; and 51 per cent of the patients receiving high dose prednisone therapy in the first two months post-transplant developed serious medical complications compared to only 14 per cent of those receiving low dose prednisone therapy. In addition, granulocytopenia was associated with a significantly higher infection rate, and patients < 10 and > 40 years of age had significantly higher mortality rates. the other risk factors examined did not correlate with mobidity or mortality rates. This analysis provides some objective guidelines to the physician to help him decide when to recommend graft removal following renal transplantation.

Post-transfusion hepatitis occurred in ten of 56 (17.9 per cent) patients in 1963–1964, and in 14 of 78 (17.9 per cent) patients in 1968–1969 at Philadelphia General Hospital (PGH). Since November 1969 all donor blood has been tested for Australia antigen (Au) by immunodiffusion and later by counterelectrophoresis, and no positive units have been transfused. all patients were screened by stringent criteria to exclude any pre-existing liver disease and had repeated follow-up examinations after transfusion, including clinical examination, as well as serial serum glutamic pyruvic transaminase (SGPT) and Au determinations, Among 204 patients receiving Au-negative donor blood who could be followed adequately for six months, 12 (5.9 per cent) hepatitis patients (four icteric) were detected. These results indicate a two thirds reduction from the 18 per cent incidence in post-transfusion hepatitis at PGH found on two previous studies. Au testing to exclude positive donors and administering only Au-negative blood, and the changes in composition of the donor population which resulted, were effective in reducing the incidence of post-transfusion hepatitis.
