The status of the diagnosis and treatment of Hodgkin's disease is briefly reviewed. Certain aspects–pathological identification and classification, staging and staging techniques (lymphangiogram and laparotomy), radiation treatment (dose and field size), chemotherapy, and data handling–are discussed in relation to their effects on results being currently reported. The need for further data before conclusions are reached as to the place and extent of diagnostic and therapeutic procedures is emphasized.

An additional case ofBacteroides sp.endocarditis is reported. This rare infection may go undetected because of the fastidious growth requirements of the organism. Typically, it occurs in males above the age of 40 with underlying heart disease. The oropharynx, the gastrointestinal tract, and the genitourinary tract are the principal portals of entry. The clinical manifestations do not differ appreciably from those described for endocarditis due to facultative microorganisms except for an increased incidence of thrombophlebitis and embolic phenomena. The mortality rate of 36 per cent is in part the result of the penicillin resistance ofB. fragilis.Of the antibiotics currently available, chloramphenicol appears to be the drug of choice.

Fifty adult patients were treated with 0.6 to 1.8 grams/day of parenteral clindamycin-2-phosphate for 4 to 37 days. Favorable clinical results were observed in patients with pneumococcal pneumonia, streptococcal pharyngitis, acute staphylococcal and/or streptococcal soft tissue infection, staphylococcal osteomyelitis, and actinomycosis. Three patients with mycoplasma pneumonia did not respond. Serum concentrations of clindamycin, particularly after intravenous administration, were many-fold higher than the minimum inhibitory concentrations of the clinical isolates. Patients' clindamycin-containing sera showed antibacterial activity against their own infecting pathogens in high dilution. Eight to 28 per cent of the administered clindamycin was excreted in the urine in active form. Clindamycin was well tolerated and generally nontoxic although reversible rises in SGOT and SGPT were frequently observed without other laboratory or clinical evidence of hepatotoxicity. In animal experiments simulating clinical use, wide tissue distribution of clindamycin was observed and there was no histologic evidence of hepatic cell injury.

Auditory function was evaluated in nine patients with two metabolic bone disorders, seven with idiopathic osteoporosis (IO) and two with Paget's disease of bone (PD). Five patients with IO and the two with PD were subjectively aware of hearing losses which occurred concomitant with or after the onset of symptoms of their bone disease. Audio-metric thresholds were markedly altered from normal in six of seven patients with IO and in the two with PD when compared with an age-matched patient group without bone disease. Three distinct patterns of hearing loss were observed. Acoustic resistance was elevated in each patient tested indicating some fixation of the ossicular chain. Roentgenographic analysis of temporal bones indicated increased sclerosis of cochlear and vestibular labyrinths in four of seven patients with IO. These results demonstrate that patients with two forms of metabolic bone disorders have decreased auditory function.

Prolonged bleeding time and alteration in clotting factors were observed in a patient with polycystic renal disease and cerebral artery aneurysm who received pharmacological doses of dextran-40. It is suggested that these abnormalities may have been aggravated by selective retention of the higher molecular weight fraction of the polymer, due to increased renal decompensation induced by the dextran. The dextran effect on platelet function is probably the major cause of impaired hemostasis. Other coagulation changes such as reduced Factor VIII and fibrinogen levels, shortened thrombin and reptilase times, and the possible mechanisms of action involved are discussed.

A patient with coexistent rheumatic mitral insufficiency and congenital subaortic diaphragm is reported. The association of these two conditions resulted in the absence of a significant gradient across the aortic outflow tract and the failure to appreciate the presence of a subaortic membrane. The correction of the mitral insufficiency by valve replacement resulted in the development of progressive left ventricular failure.
