In vitro comparisons of bactericidal activity of kanamycin, ampicillin and the kanamycin/ampicillin combination were made with 354 Enterobacteriaceae; including 224Escherichia coli, 51Enterobacter-Klebsiellaand 79 Proteus. Ninety-six per cent of the entire group were killed by the combination of 12.5 μg of each antibiotic per ml, 84% were killed by 3.1 μg of each per ml, demonstrating a distinct antibacterial advantage for the combination over kanamycin or ampicillin used alone. In addition, kanamycin/ampicillin was synergistic against 26% ofE. coli, 12% ofEnterobacter-Klebsiella, 42% ofProteus mirabilisand 36% of indole positive Proteus. Significant antagonism did not occur. The clinical significance of these data is discussed.

Serum iron concentrations and total iron binding capacity fall early after the exposure of volunteers to virulentFrancisella tularensis. Serum iron fell significantly and consistently below values encompassing normal changes or differences among individuals. The prospective nature of this study and the rapid development of hypoferremia, even in vaccinated subjects who remained asymptomatic, made it evident that scrum iron fell in two distinct phases. The first, or Exposure Phase Hypoferremia, represented a consistent fall of modest magnitude early in the incubation period which seemed to be an early host response to invading microorganisms, a response independent of subsequent illness or its absence. The second, or Illness (Febrile) Phase Hypoferremia, represented an exaggerated, superimposed response related in timing and magnitude to the onset and severity of illness. The possibility was raised that an endogenous mediating factor was released from neutrophilic leukocytes and served to trigger the observed changes in iron metabolism.

The rate of clearance of a drug from the circulation was measured in alcoholic subjects and in abstinent control subjects. Tolbutamide, warfarin and diphenylhydantoin were studied. All of the alcoholic test subjects had a history of recent heavy alcohol intake. The half-life of the drugs in the blood of the alcoholic test subjects was significantly shorter than in the controls for all three drugs. These observations indicate a more rapid removal rate of these three drugs known to require hepatic microsomal enzymes for their metabolism. It is possible that continued intake of alcohol induces nonspecifically, hepatic microsomal enzymes.

The peripheral circulator status of the upper extremity vascular bed has been described in patients with obstructive airways disease. An indicator-dilution method was used to estimate blood flow. Despite hypoxemia and hypercapnia with and without acidosis, the total upper extremity and forearm blood flow and vascular resistance were within the normal range in most patients studied. Correction of hypoxemia, despite enhanced hypercapnia, did not alter total or forearm blood flow. Some of the factors which might influence the measured blood flow values are discussed.

Studies were performed on eight dogs which received heart allografts in an attempt to determine the earliest onset of increased immunologic activity directedagainst the allografts. An increased rate of spontaneous transformation of peripheral blood lymphocytes to “blast” cells in culture was observed within 24 hours after surgery and remained elevated until two to five days after the rejected heart was removed. Such increased spontaneous transformation rates were the earliest indicator of rejection, occurring two to seven days prior to rejection. By contrast, electrocardiographic changes did not occur until six to 24 hours immediately prior to rejection.
