A 21 year old man with mild hemophilia A (factor VIII levels of 6.9) and 4.5% ) developed a factor VIII inhibitor of 12.5 units/ml after receiving concentrated human factor VIII for a surgical procedure. The inhibitor was undetectable six weeks after it appeared. Only four previous cases of inhibitors in mild hemophiliacs have been reported and the low titers and transient nature of the inhibitors in these cases make them unlike the inhibitors that occur in severe hemophiliacs.

A search for viral, bacterial, and mycoplasmal pathogens was made during 51 episodes of pneumonia in nonrecruit military personnel hospitalized in Hawaii. In men under age 41, evidence of M. pneumoniae infection was found in at least 54% of pneumonias, while bacterial pneumonias were rare. In contrast, in men over age 40, many of whom had underlying diseases, bacterial pathogens caused half of the pneumonias, whereas M. pneumoniae infection was uncommon. During the study, a shipboard epidemic of M. pneunumiae infection was encountered, the first to be studied by radiographic, cultural, and serologic methods.

Renal hemodynamies were evaluated in 30 intact anesthetized dogs at 2.36 and 3.6 atmospheres absolute (ATA) of oxygen pressure using a dye-dilution method to measure blood flow. In 15 animals studied at 2.36 ATA the asterial pO2exceeded 1400 mm Hg. Renal blood flow, renal blood volumes, mean arterial pressure, stroke volume, renal fraction of cardiac output and the renal arteriovenous O2difference did not change significantly, while renal resistance and total systemic resistance increased 10 and 28%, respectively. In 15 animals studied at 3.6 ATA the arterial pO2exceeded 2400 mm Hg; significant mean decrease occurred in renal blood flow with no significant change in the renal fraction of cardiac output. Renal and systemic resistance increased 46 and 63%, respectively. The renal arteriovenous O2difference increased as renal blood flow fell; therefore, renal oxygen uptake was unchanged. These studies suggest a selective effect of hyperoxygenation on the renal and systemic circulation with a higher increase in the systemic resistance than renal resistance.

The history of the first known case associating hypertrophic subaortic stenosis and Friedreich's alaxia is presented along with a brief discussion of the cardiomyopathy of Friedreich's alaxia. The patient presented at the age of seven with the classical neurological findings of Friedreich's alaxia and was noted to have a cardiac murmur consistent with dynamic sabaortic stenosis. Cardiac catheterization demonstrated a resting gradient between the inflow and outflow tract of the left ventricle. The gradient increased following the administration of Isoproterenol and amyl nitrite. An elevated first derivative of the brachial artery pulse and the typical decrease in the brachial artery pressure pulse following a premature ventricular contraction was also recorded.

The effect of low amplitude, 0.5-2 ma, chest wall stimuli (CWS) upon the function of fixed rate, P-wave synchronous, QRS blocking demand and QRS synchronous standby pacers has been discussed. The role of such CWS in evaluation of the clinical function of the varied types of pacemakers has been illustrated. Chest wall stimuli provide an additional means of rapid atraumatic evaluation of normal and abnormal pacer function.

Twenty-eight patients with hypothyroidism or goiter were treated double-blind with replacement dosages of desiccated thyroid USP, thyroglobulin, liothyronine, and sodium levothyroxine. Laboratory profiles and clinical responses demonstrated that all compounds had a normalizing effect on the patients' metabolism. Bound T4(as represented by PBI concentrations) reflects serum free thyroxine levels, confirming a previous hypothesis that free and bound T4exist in an equilibrium conforming to a mass action equation. In contrast to what has previously been reported, the resin T3test was in the normal range for patients on liothyronine. There was a higher incidence of hyperthyroid symptoms on liothyronine therapy. When patients were treated with sodium levothyroxine, free T4and bound T4were high, at levels near those associated with clinical hyperthyroidism in untreated subjects.
