The use of lithium as a therapeutic agent in the treatment of chronic alcoholic patients with depression has been investigated in a double blind study with active medication and placebo. Lithium appears to modify the patients' drinking habits significantly (p <0.01) when the readmission rate of the groups are compared. Of those patients who had to be readmitted for their drinking, the lithium group had fewer episodes as compared to the control group. Although both groups were less depressed at the end of one year, as compared to their depression ratings at the beginning of the project, there was not a significant difference between groups (analysis of covariance), ie, there was a placebo response which may have disguised a real difference. Other parameters studied included serum magnesium levels and thyroid function which showed that the clearance of iodine from the plasma by the thyroid gland was significantly increased (p <0.005)

The association of rheumatoid factor (RF) and hepatitis B antigen (HBAg) was investigated in four serum collections.Group 1:Fifty-eight per cent of 353 patients with miscellaneous liver diseases were sero-positive for RF. RF did not apear to be more common among HBAg-positive sera than among HBAg-negative sera from patients with liver disease.Group 2:Twenty-six per cent of 137 chronic HBAg carriers were found to be sero-positive for RF.Group 3:HAg was not associated with rheumatoid arthritis; among 74 patients with this diagnosis, only one was seropositive for HBAg.Group 3:RF was not associated with false positive reactions for HBAg on radioimmunoassay; among 30 blood donor sera giving false positive reactions for HBAg on radioimmunoassay only two (6 per cent) were positive for RF. It was concluded that RF is a frequent serologic manifestation accompanying liver disease and that its presence correlates with severity of liver injury but not with the presence or absence of HBAg

A 17 per cent incidence of hypercalcemia was found in 121 patients with hyperthyroidism. Constipation and a loss of initiative or an apathetic appearance were present in a proportionately increased number of hypercalcemic hyperthyroid patients. However, only the presence of constipation appeared to be related to the serum calcium level. Hypercalcemia, a frequent occurrence in the hyperthyroid patient, due to action of thyroid hormone upon bone, may significantly modify certain clinical features of the disease.

Five hundred consecutive patients admitted to the medical service of a large general hospital were evaluated for anemia. Decreased hemoglobin levels were detected in 23 per cent of the patients. Hypoproliferative anemia was documented in 52 per cent of the anemic patients, and in 90 per cent of the patients a probable cause was found. Iron deficiency accounted for 23 per cent, folate deficiency for 3.5 per cent, vitamin B12deficiency for 1.5 per cent, and hemorrhagic-hemolytic (including hemoglobinopathies) for 5 per cent of the anemias. Early death or discharge of some patients resulted in 15 per cent undiagnosed anemias. Early etiologic diagnosis of anemia was often helpful in guiding management of individual patients. Understanding the hematologic characteristics of the population admitted to the hospital helped the house staff to anticipate the probable causes of anemias and allowed the faculty to judge which anemias to emphasize in their teaching.

With the Doppler ultrasonic flowmeter catheter, phasic instantaneous and continuous left ventricular blood velocity was measured in the left ventricles of 163 subjects during elective right atrial and ventricular pacing. At driving rates which exceeded 120 beats/minute there was a uniform decrease of peak left ventricular blood velocities. The most profound decline of peak blood flow velocity was observed during the course of pacemakerinduced ventricular tachycardia. There was a significant degree of inverse correlation between the per cent of control peak left ventricular and ventricular rates during the tachycardias (r=−.82, p<0.001). Such reductions of left ventricular blood velocity are secondary to less forceful left ventricular contraction and shorter diastolic filling periods during rapid heart rates. It is concluded that observations regarding diminished blood flow velocities in central and regional circulations during tachycardia are linked to the basic abnormality of left ventricular blood velocity demonstrated in this study.

The occurrence of polymyositis and pernicious anemia in a 58-year-old man with acquired hypogammaglobulinemia is described. Immunologic studies showed very low values of circulating immunoglobulins by radial immunodiffusion. Only 2 per cent of the circulating lymphocytes were identifiable as B-lymphocytes by the immunofluorescence technique for the detection of membrane-bound immunoglobulin. The possible relationship of the development of polymyositis to hypogammaglobulinemia is discussed.
