Intravenous doses of 100 mg of streptomycin and 100,000 units of penicillin G were given separately and in combination to a dog with chronic enterococcal bacteremia. Serial quantitative blood cultures in standard and hyperosmolar media and simultaneous serum antibiotic assays were performed. Streptomycin alone had no demonstrable effect on the level ol bacteremia. Penicillin alone reduced lower levels of pre-antibiotic bacteremia, followed by an early rise at four to five hours after injection. With higher pre-antibiotic bacteremia, this rise was detected as early as two hours after antibiotic injection. Penicillin plus streptomycin produced a more rapid and more pronounced depression which was sustained for longer than six hours. Thisin vivokilling effect lasted longer than detectable serum antibiotic concentrations. Following penicillin administration, persistently higher bacterial counts were obtained in the hyperosmolar medium than in the standard medium. This finding suggests the production of osmotically fragile forms of the organism after single doses of intravenous penicillin. This study corroborates previous in vitro observations of synergism of penicillin and streptomycin against enterococci.

A patient with diabetes mellitus treated with the sulfonylurea chlorpropamide developed the syndrome of inappropriate secretion of antidiuretic hormone. The mode of clinical presentation was one suggesting a central nervous system disorder. A clinical study of the patient’s response to chlorpropamide was carried out Headache, systolic hypertension, serum hypo-osmolality, hyponatremia, urine hyperosmolality, fall in urinary output and continued renal excretion of sodium were demonstrated, confirming the initial diagnosis.

A review of clinical records of the past 10 years at the Cincinnati General, Cincinnati Veterans Administration, Christ, Good Samaritan, Jewish, and Holmes Hospitals, Cincinnati, yielded 25 patients with proven pulmonary blastomycosis. Eleven presented with findings suggestive of a diagnosis of primary carcinoma of the lung and one with carcinoma of the larynx and pulmonary metastases. The clinical presentation was varied; three were asymptomatic. Seven of the 12 underwent major surgical procedures before the diagnosis was apparent. An eighth was diagnosed by laryngeal biopsy. Three of the remaining four were bronchoscoped but were actually diagnosed when previously collected sputums grewBlastomyces dermatitidis.The remaining patient had pulmonary infiltrates, a bone lesion, and a positive skin biopsy for blastomycosis. An expeditious diagnostic approach is warranted when carcinoma is suspected, but prudent attention to the possibility of blastomycosis might save an occasional patient an otherwise unnecessary surgical procedure.

Septicemia withAeromonas hydrophilaandMima polymorphais described in a patient with acute myelogenous leukemia. This is the tenth reported case ofA. hydrophilasepticemia, but it is the first documented survival of septicemia with this organism in a patient with acute leukemia. This is the first reported case of a mixedA. hydrophilaandM. polymorphasepticemia in association with acute leukemia. Although it is difficult to assign importance of one organism over another, it appeared from previously reported cases and from the gastrointestinal disturbances preceding the sepsis, that the clinical course of our patient was more compatible withA. hydrophilasepticemia than withM. polymorphasepticemia. The lowered resistance of the host appeared to be a major factor in the development of the systemic infection in this patient.

The serum concentration, urinary excretion, and renal clearances of eight proteins of various molecular weight were studied in 14 patients with chronic myelogenous leukemia Values of total serum protein, gamma globulin, and the various immunoglobulins were either normal or low. Serum lysozyme exceeded normal values in seven patients. However, no correlation was found between the level of lysozyme and the presence or absence of Philadelphia chromosome. Serum transferrin was low in ten patients, ceruloplasmin was increased in seven, while ∂2 macroglobulin was usually normal or increased. The total urinary protein was generally normal except for one patient who had mild proteinuria. The urines usually contained low or normal quantities of the proteins studied, with low or normal renal clearances except for those of immunoglobulin G (IgG), immunoglobulin A (IgA), and transferrin, which were increased. Albumin constituted less than 25 per cent of the total urinary protein in nine patients. No significant impairment of renal function or electrolytic imbalance were observed. These findings indicate a substantial difference between myelogenous leukemia and mono- or myelomonocytic leukemia, in relation to the abnormalities frequently observed in the serum and urinary proteins, electrolytic imbalance, and renal function in the latter type of leukemia.

The effect of subpressor and pressor doses of angiotensin II was studied in 14 normal subjects. Ten of these received an intravenous magnesium load. Both doses caused a diminution of urinary magnesium, calcium, and sodium excretion, with reduction of the renal plasma and urinary flow. The magnesium infusion caused an increase in magnesium and calcium excretion; the absolute magnesium reabsorption remained unchanged, even when pressor doses of angiotensin II were used. A maximum reabsorption capacity for magnesium is suggested. The decrease observed in the urinary excretion of sodium, calcium, and magnesium under angiotensin II infusion could be mediated by the hemodynamic effects of the drug. The mechanism by which hemodynamically induced changes alter the fractional electrolyte reabsorption is still unclear.

Thin needle aspirations of intrathoracic masses were performed on 20 patients with x-ray or fluoroscopic guidance. Useful diagnostic information was obtained in all patients, with malignancy found in 18 of the 20 patients. Eight of these showed cytologic evidence of undifferentiated carcinoma, which was the most frequently diagnosed cell type.
