The purpose of this study was to measure uptake of tritiated digoxin by neoplastic tissues known to have differential contents of sodium-potassium adenosine triphosphatase (Na+K+ATPase), the presumed receptor for digoxin. Tumor samples were removed at the time of craniotomy in seven patients with meningiomas (Group 1) and seven patients with more malignant central nervous system tumors (Group 2) (three astrocytomas, three glioblastomas, one meduloblastoma). Patients with meningiomas were found to have a significantly higher digoxin uptake (21.8 ± 7.3 ng/gm tumor versus 5.7 ± 5.2 ng/gm tumor; (p<0.01) and a significantly greater tissue/serum ratio (13.9 ± 11.7 versus 3.26 ± 3.7, p<0.0). This study provides the first demonstration of increased uptake of digoxin by non-cardiac pathologic tissues. The results are most likely due to differences in the number of digoxin receptor sites.

A study of coagulation disorders due to hepatitis A infection occurring during pregnancy was undertaken to determine if the unique coagulation status produced by pregnancy (elevated clotting factors and decreased fibrinolytic activity) was responsible for the increased severity of hepatitis A infections reported for pregnant women from various parts of the world. Of 49 patients studied, 12 (24 per cent) developed hepatic failure and 9 (18 per cent) died. A prolonged prothrombin time and low fibrinogen level were found to be as frequent as previously reported for nonpregnant patients with and without hepatic failure. Thrombocytopenia was less common and a long thrombin time was more common. Although intravascular coagulation was suggested by a lower mean fibrinogen level than expected in late pregnancy, mean platelet counts were similar to controls. The frequency of a positive protamine sulfate paracoagulation test for intravascular coagulation (DIC) was similar to that reported for uncomplicated pregnancy, and was of no prognostic value when performed on admission. We conclude that the severe clinical course of hepatitis during pregnancy in this epidemic was not attributable to a predisposition for DIC. However, once fulminant hepatitis occurred, DIC may have been a clinically significant factor.

Effective renal plasma flow (ERPF) and glomerular filtration rate (GFR), as determined by a radioisotopic method, were evaluated in seven heroin-overdosage patients (Group I) and 7 hypnotic-sedative-analgesic overdosage patients (Group II). No patient had evidence of shock. There was no significant intergroup differences in PaO2, PaCO2, pH, blood pressure, pulse rate, respiratory rate, cardiac output, or serum creatinine levels. Group I patients had lower ERPF and GFR compared to Group II (271 ± 182.8 vs. 580 ± 62 cc/min and 62.4 ± 32.6 vs. 102.9 ± 12.8 cc/min, respectively, p< 0.005). We conclude that compared to hypnotic-sedative-analagesic overdosed patients heroin overdosed patients have more profoundly altered renal hemodynamics.

The lymphocytes of iron-deficient anemic and nonanemic patients have a decreased response to phytomitogens. Treatment of these patients with iron is followed by restoration of the lymphocyte response to normal. These findings indicate an important nonhemoglobin function of iron. Iron deficiency may be a factor in the production or potentiation of immune deficiency.

Seventeen patients with lymphomatous involvement of the heart or pericardium were studied. The series includes eight patients with Hodgkin's disease and nine with non-Hodgkin's lymphoma. All 17 had radiologic evidence of pulmonary, pleural, or mediastinal involvement. Cardiac or pericardial disease in seven was apparently due to direct extension of other intrathoracic tumor masses. Cardiac involvement was usually a late manifestation of lymphoma with median onset 20 months after initial diagnosis. Fourteen patients had been treated for stage IV disease prior to the onset of cardiac lymphoma. Only seven of the 17 patients with cardiac involvement were diagnosed antemortem. Four of them are alive without evidence of disease 8 to 68 months after diagnosis and treatment. Because cardiac lymphomas may respond to therapy with prolonged survival, it is imperative that clinicians suspect cardiac or pericardial involvement in lymphoma patients who have radiographic evidence of intrathoracic lesions (especially adjacent to cardiac borders), unexplained tachyar-rhythmia or conduction disturbance, evidence of outflow obstruction, or signs and symptoms suggesting pericardial effusion or tamponade.

Mean 24-hour urinary excretion of copper and zinc was greater than normal in 17 patients with untreated primary hyperparathyroidism. Mean total serum copper concentration in these patients was slightly greater than the normal mean, whereas mean total zinc concentration was normal. After surgical treatment of hyperparathyroidism there was a transient further increase in urinary copper and zinc concentrations in the 13 patients so studied, with no consistent change in serum copper or zinc concentrations. Five patients were studied for several days after this postoperative peak had passed. Urinary copper and zinc excretion decreased in three in relation to surgical correction of their hyperparathyroidism and remained elevated in one with persistent hypercalcemia. In the fifth patient urinary zinc excretion increased to slightly above the normal range despite successful surgery. These results suggest that changes in urinary excretion of copper and zinc may be useful indicators in the diagnosis and treatment of patients with primary hyperparathyroidism.

Sixty-two of 1242 patients with M components were found to have lymphoma. There were 33 patients with immunoglobulin M (IgM), 20 with IgG, 5 with IgA, and one patient with Bence Jones protein M components. Three patients had biclonal gammopathy. The types of lymphoma were: lymphocytic, 31; histiocytic, 12; mixed cell, 4; stem cell, 2; Burkitt's, 1; Hodgkin's disease, 9; and unclassified, 3. All patients were in stages III or IV of lymphoma, and the average duration of disease was 29.3 months when M components were detected. Anemia, abnormal peripheral blood lymphocytes, and lymphomatous involvement of the bone marrow were especially common among patients with IgM M components. Osteolytic lesions were found in 12 patients and osteosclerotic lesions in one. A second malignancy occurred in eight patients. The level of M component was below 1.0 gm/dl in 55 per cent of patients. Significant suppression of normal immunoglobulin levels in the serum was noted in 4 and 16 patients with IgG and IgM M components, respectively. Bence Jones proteinuria was found in 19 per cent, cryoglobulinemia in 11 per cent, and cold agglutinins, all of anti-i specificity, in 10 per cent of the patients. Most of the M components decreased during therapy. Only two M components gradually increased. The mean survival of 39 patients who died was 10.4 months. The living patients have been followed for a mean period of 21.2 months. The presence of M components in lymphoma may suggest B cell origin of the tumor but the coexistence of plasma cell dyscrasia cannot be ruled out.

A rare complication, massive pulmonary hemorrhage, occurred in two patients who were receiving heparin for pulmonary thromboembolic disease. The site of pulmonary hemorrhage was not found at autopsy. The occurrence of profuse hemoptysis in both patients prior to anticoagulation suggests that this finding may be of value in predicting the risk of pulmonary hemorrhage in a given patient.

A 69-year-old man with glucose-6-phosphate dehydrogenase deficiency was treated with nitrofurantoin for pyuria. Four days later he presented with metabolic acidosis due to excess lactic acid, a decline in circulating hemoglobin, reticulocytosis, elevated serum transaminase levels, and hyperbilirubinemia. The drug was withdrawn and the hyperlactatemia subsided in three days without specific treatment.In vitro, nitrofurantoin is capable of stimulating erythrocyte glucose utilization and lactate production, and inhibiting the generation of reduced glutathione.In vivo, this drug is capable of producing hemolysis in susceptible subjects and hepatocellular injury. The temporal proximity of drug ingestion and hemolysis, increased glucose utilization, lactate excess, and hepatic insufficiency suggests that nitrofurantoin may have been responsible for precipitating the clinical and chemical abnormalities observed.
